dc.description.abstractBACKGOUNDS: Although benign childhood epilepsy with centrotemporal spikes (BCECT) is known to have good prognosis with more than 80% of spontaneous remission eventually, patients often manifest neuropsychological abnormalities. The purpose of the present study is to investigate neuroanatomical and electrophysiological basis of cognitive dysfunction in children and adolescence with BCECT. METHODS: Nineteen patients with BCECT and 25 normal subjects aged 6 to 16 year were recruited from the Neurology Department of Ewha Womans University Hospital. For all subjects, neuropsychological tests were assessed by using Korean version of the Wechsler Intelligence Scale for Children (K-WISC-III), Stroop and Trail-making tests (TMT), verbal and visual memory, verbal fluency and picture naming tests. Magnetic resonance image (MRI) and diffusion tensor image (DTI) were performed using a Philips Achieva 3T MR scanner. Cortical thickness and white matter integrity were measured by FreeSurfer and FSL (FMRIB's software library) softwares. Spectral analysis and phase coherence of electroencephalograph (EEG) were analyzed for evaluation of functional connectivity by using MATLAB. RESULTS: Patients with BCECT had significantly lower intelligence scores compared to control subjects. Abnormal changes in cortical thickness and DTI was detected in BCECT patients compared to controls. Cortical thickness was reduced in bilateral orbitofrontal, lateral frontal, mid- and posterior lateral temporal, and part of parietal regions. Abnormalities in DTI (increased axial and mean diffusivity values) in the left inferior fronto-occipital fasciculus were associated with lower intelligence scores. There was no difference between groups in spectral power, but decreased coherence was observed especially in the bilateral frontal and temporal areas. Decreased coherence in the bilateral frontal and temporal areas was associated with lower intelligence in the patient group. CONCLUSION: Neuroanatomical and functional connectivities were altered in patents with in BCECT patients, which might lead to their intellectual abnormalities in certain cognitive domains. Interestingly, white matter microstructural changes were also contributed to cognitive impairments in these patients.;소아의 양성롤란도뇌전증 (benign rolandic epilepsy 또는 benign childhood epilepsy with centrotemporal spikes, 이하 BCECT)은 80% 이상이 저절로 소실되는 우수한 예후를 보이는 것으로 알려져 있으나, 환자들은 종종 인지기능이상을 보이기도 한다. 본 연구의 목적은 BCECT로 진단된 소아 및 청소년 환자에서의 인지기능저하와 관련된 신경해부학적 및 전기생리학적 근원을 알아내고자 시행되었다. 연구참여 대상자는 6세에서 16세 사이의 BCECT 환자 20명과 연령, 성별이 비슷한 정상인 25명을 이화여자대학교 부속 목동병원 신경과에서 모집하였다. 신경심리검사를 위해 아동용 웩슬러 지능검사, 집행기능 검사, 언어성 및 시각 기억 검사, 언어 유창성 검사 등이 수행되었다. 자기기능공명 영상과 확산텐서 영상을 이용하여 얻은 뇌영상으로부터 대뇌피질두께와 대뇌백질의 미세구조를 측정하였다. 뇌영상의 정량적 분석을 위해 컴퓨터 소프트웨어인 FreeSurfer와 FSL (FMRIB's software library)을 사용하였다. 뇌파 데이터는 파워스펙트럼 분석과 신경연결성을 MATLAB을 이용하여 정량적으로 분석하였다. 연구 결과, 환자군은 정상군에 비해 유의하게 낮은 지능지수를 보였다. 정상군에 비해 환자군에서 대뇌피질 두께와 대뇌백질구조의 비정상적인 변화가 관찰되었다. 환자군은 정상군에 비해 양측 안와전두피질 및 외측두엽, 및 일부 두정엽 부위에서 대뇌피질두께의 유의한 감소를 보였다. 또한 환자군에서 전두엽을 다른 뇌부위로 연결하는 대뇌백질의 섬유다발에서 미세구조의 이상이 발견되었다. 특히 대뇌백질의 미세구조의 변화는 환자군의 지능지수의 저하와 유의한 상관관계가 있었다. 파워스펙트럼 분석에서 환자군과 정상군 사이에 차이는 없었으며, 전두엽과 측두엽 사이의 신경연결성이 환자군에서 유의하게 감소되었다. 환자군에서 신경연결성의 감소는 지능지수의 저하와 유의한 상관관계가 있었다. 본 연구의 결과를 종합하면, BCECT 환자에서 인지기능의 저하와 관련된 뇌의 구조적 및 기능적 연결성의 변화를 발견하였다. 특히 국소적 대뇌백질구조 및 신경연결성의 비정상적인 변화가 BCECT 환자의 인지기능 저하와 관련이 있었다.-
