한국형 베일리 영유아 발달검사 제 3판 사회-정서 척도의 표준화를 위한 예비연구

Abstract

본 연구는 베일리 영유아 발달검사 3판 사회-정서 척도(Bayley Scales of Infant and Toddler Development Third Edition(Bayley-Ⅲ) Social-Emotional Scale; Bayley, 2006)를 표준화하여 한국형 베일리 영유아 발달검사 제 3판(Korean-Bayley Scales of Infant and Toddler Development, Third Edition; K-Bayley-Ⅲ) 사회-정서 척도를 제작하기 위한 예비연구이다. 본 연구의 목적은 문항 분석을 통하여 K-Bayley-Ⅲ 사회-정서 척도 35문항에 대한 내용과 연령별 중단 기준을 검토하고, 신뢰도와 타당도를 확인하며, 임상집단변별을 위한 선별도구로써의 기초 자료를 확보하는 것이다. 본 연구의 대상은 서울 및 경기도에 거주하는 생후 15일부터 42개월 15일까지의 영유아 384명과 미숙아와 발달지체를 포함한 임상집단 26명으로, K-Bayley-Ⅲ 사회-정서 척도 예비연구본을 사용하여 검사를 실시하였다. K-Bayley-Ⅲ 사회-정서 척도의 문항 분석을 위하여 문항 숙달 비율을 검토하고, 문항 난이도 분석을 시행하였다. 신뢰도 검증을 위하여 문항 내적 일관성과 측정의 표준오차를 검토하였고, 상관 분석을 실시하여 검사-재검사 신뢰도를 알아보았다. 타당도 검증을 위하여 감각처리문항이 사회-정서 척도에 속하는 것이 타당한지, 그리고 공인 타당도를 알아보기 위한 상관 분석을 실시하였다. 마지막으로 사회-정서 척도가 임상집단영유아와 정상발달 영유아를 잘 변별하는지 알아보기 위하여 두 집단 간 독립표본 t검증을 실시하였다. 본 연구의 결과는 다음과 같다. 첫째, K-Bayley-Ⅲ 사회-정서 척도의 문항 숙달 비율 검토 결과 영유아가 해당 연령 단계 이전의 문항에 대해서는 대부분 .80~.10 사이의 문항 숙달 비율을, 해당 연령 단계의 문항에 대해서는 대부분 .50~.90 사이의 문항 숙달 비율을 보여 기능적 정서 발달 순서에 따라 문항 배치가 적절하게 이루어진 것으로 나타났다. 또한 문항 난이도 분석을 통해 살펴본 결과 문항 난이도는 ‘매우 쉬운~매우 어려운’ 수준으로 구성되어 있으며, 연령 단계별로 비슷한 난이도로 분포되어 있어 연령별 중단 기준이 적절한 것으로 나타났다. 둘째, K-Bayley-Ⅲ 사회-정서 척도의 신뢰도 분석 결과 연령 단계에 따른 Cronbach's α 계수는 .69-.94로 높은 내적 일관성을 보였고, 측정의 표준 오차는 2.78-5.16으로 문항 총점이 175점인 것을 감안하였을 때 우수한 신뢰도 구간이 예측되었다. 또한 검사-재검사 신뢰도는 .984로 본 척도가 시간의 변화에 따라 매우 높은 안정성을 가졌음을 확인하였다. 셋째, K-Bayley-Ⅲ 사회-정서 척도는 전문가 집단 자문, 검증위원회의 문항 검증, 부모 이해도 평가를 통하여 내용 타당도를 검증하였다. 또한 감각 처리 문항과 전체 사회-정서 척도의 상관이 연령 단계에 따라 .630-.963으로 유의미하여 감각 처리 문항이 사회-정서 척도와 한 검사로 묶일 수 있음을 확인하였고, 공인타당도의 확인을 위해 실시한 K-ASQ:SE와의 상관은 -.605, CARS와의 상관은 -.556으로 모두 유의미한 상관을 보여주어 척도의 타당성을 확인하였다. 임상집단을 대상으로 한 변별 타당도 분석에서 임상집단 영유아와 정상발달 영유아의 사회-정서 척도 점수 차이가 유의미한 것을 확인하였으며, 임상집단 분류 기준점에 따른 분석 결과 사회정서와 관련된 장애의 위험이 있어 심층 진단이 필요한 영유아의 경우 환산 점수 6점 이하의 기준점에서, 자폐 스펙트럼 장애 영유아의 경우 환산 점수 4점 미만의 기준점에서 높은 분류 일치도를 가지고 있는 것으로 드러났다. 본 연구 결과는 K-Bayley-Ⅲ 사회-정서 척도의 신뢰도와 타당도를 검증하여 본 검사가 영유아의 사회정서 발달을 평가하기에 신뢰롭고 타당한 검사라는 것을 확인하였다는 점에서 의의가 있다. 또한 국내 표준화를 위하여 번안 과정에서 내용 타당도를 확보하였고, 연령별 중단점을 확인하여 한국형 발달검사로써의 시사점을 제공하였다. 추후 연구를 통하여 한국 규준을 개발하는 작업과 함께 임상집단 변별을 위한 기준점 산출과 선별도구로써의 타당성 검증이 이루어져야 할 것이다.;The purpose of this Study was to standardize the Korean version of the Social-Emotional Scale of the Bayley Scales of Infant and Toddler Development, 3rd Edition(K-Bayley-Ⅲ) by analyzing the reliability and validity of the Scale and examining the adequateness of each item. The subjects for this study included 384 Korean infants and toddlers in age range of 15 days to 42 months 15 days residing in Seoul and Kyonggi province. Also 26 infants and toddlers in age range of 2 to 42 months participated in the clinical sample study. Item response analysis was done with the BILOG-MG3 and statistical analysis was done with SPSS 21.0. Results of the study were as follows: 1. As a result of analysing the degree of difficulty of the items, ti was shown that the degree was similar arranging in the child's age groups. 2. Result of the inter-item consistency reliability demonstrated a Cronbach's α of .69-.94 in the social-emotional scale, showing high reliability. In terms of the Standard Error of Measurement(SEM), the scale showed an SEM range of 2.78-5.16, implying a low level of error in the K-Bayley-Ⅲ Social-Emotional Scale. The Test-Retest reliability level was .984 reflecting extremely high level. 3. The Social-Emotional Scale of the Bayley-Ⅲ was translated English to Korean by the student of developmental psychology, and the bilingual, who can use Korean and English, did a back-translated. In order to examine content validity, experts in psychology and neuropsychiatry verified all items. The sensory processing items and the total Social-Emotional Scale have a correlation of .630-.963, depends on age-group. In terms of concurrent validity, the Social-Emotional scale of K-Bayley-Ⅲ and that of the K-ASQ:SE showed significant correlations of -.605, and also showed significant correlations with the CARS, a correlation of -.556. Also the performance of children from a clinical group provides important validity evidence for the K-Bayley-Ⅲ Social-Emotional Scale. The difference of mean performance of the developmental delay group to normal developmental group was significant. This study demonstrates that the preliminary version of the K-Bayley-III Social-Emotional scale contains items that are properly translated and applied to assess the social emotional development sequences and skills of Korean infants and toddlers. Further studies should aim for a greater number of participants in order to adequately standardize such instrument based on normally developing Korean young children, which will finally provide a norm for effectively assessing the social emotional developments of Korean infants and toddlers.
