미숙아 성장발달을 위한 이유(離乳) 교육 프로그램 개발

Abstract

최근 만혼 및 출산 연령이 늦어지고 있고 난임부부에 대한 인공임신시술이 증가하면서 미숙아와 저체중아의 출생률은 과거에 비해 꾸준히 증가하였다. 신생아학의 발전으로 미숙아의 생존율도 현저히 향상됨에 따라 미숙아가 건강하게 성장할 수 있도록 관리하는 것은 중요하다. 미숙아 및 저체중 출생아의 대부분은 잠재적으로 성장장애와 성장지연을 경험할 수 있어 이를 위해 정확한 영양학적 중재가 퇴원 후에도 이루어져야 한다. 그러므로 미숙아 어머니에게 올바른 영양공급을 할 수 있도록 미숙아 이유의 구체적이고 실질적인 교육프로그램 개발이 필요하다. 이에 본 연구는 미숙아를 분만한 어머니에게 미숙아의 이유에 대한 지식을 향상시켜 올바른 이유 진행을 잘 실시할 수 있도록 안내하는 교육프로그램을 개발하고자 하였다. 문헌고찰을 통해 교육내용의 구성요인을 도출하고, 미숙아를 분만한 어머니에게 설문조사로 이유 실태, 지식 및 교육 요구도를 파악하여 미숙아를 위한 이유 교육프로그램을 개발한 방법론적인 연구이다. 프로그램 개발은 Seels & Richey(1995)의 교수체제개발 모형인 ADDIE모형을 근거로 수행하였다. 프로그램 구성은 미숙아 성장에 필요한 영양, 이유 단계, 이유진행시 유의사항의 3영역으로 분류하여 각 영역마다 세부지침을 구성하고 구체적인 교육프로그램을 개발하였다. 프로그램 내용은 전문가 집단 6인, 미숙아 어머니 5인의 평가를 통해 수정, 보완하여 e-Book 형태로 개발하였다. 본 연구는 미숙아 이유에 관한 자료와 연구가 부족한 실정에서 미숙아 어머니들에게 이유 실태, 지식 및 교육 요구도를 측정하고 문헌고찰을 토대로 프로그램을 개발한 것에 의의가 있다. 또한 교육내용은 e-Book형태로 CD-ROM에 담아 미숙아 어머니에게 제공하여 향후 이유를 시행할 때 자가 학습을 할 수 있도록 도움을 줄 수 있으며 원할 때마다 자료를 확인할 수 있다는데 연구의 의의가 있다. 추후 이 교육 프로그램을 미숙아 어머니들에게 제공하여 효과를 검증하고 어머니들이 이를 통해 적절히 이유 실천을 함으로써 미숙아 따라잡기 성장에 기여할 수 있을 것으로 기대한다.;Recently, the birth rate of premature and low-birth-weight infants has steadily increased because artificial insemination is becoming more common among subfertile couples who marry late and because couples are deciding to have children later in life. However, the survival rate of these infants has also notably increased due to the advancement of neonatology. Therefore, the management of premature infants to promote healthy growth is crucial. Because most premature or low-birth-weight infants are at risk of growth disorders or growth delays, proper nutrition is required after the infant is discharged from the hospital. Hence, the development of specific and practical education programs for mothers of premature babies is necessary to ensure that mothers provide adequate nourishment to their babies. The objective of this study was to develop a new education program to help mothers of premature babies improve their knowledge on how to properly wean their babies. We performed methodological research using a questionnaire to obtain information about infants from their mothers. This study was based on the ADDIE model of Seels & Rechey(1995). Specifically, we developed the major components an education program based on the results of our actual weaning survey and a literature review. We spilt this program into three sections about nutrition to promote the growth of premature infants: nutrition, weaning stages, and cautions. We also developed a specific roadmap and education program for each section. Based on an assessment by six experts and five mothers of premature infants, we are providing the education program in the form of an e-Book on a CD-ROM. This study is significant because it developed a new education program despite the limited number of existing studies. This program is based on a literature review and information from mothers of premature infants regarding these mother's weaning practices, and knowledge and their demand for education. Additionally, because this program is delivered in the form of an e-Book on a CD-ROM, mothers of premature babies can easily access the program and study whenever they wish. I propose further studies to demonstrate the effectiveness of this program, and I hope that this program will be a great contribution to catch-up growth for premature infants.
