dc.description.abstract중간엽줄기세포 (Mesenchymal stem cells; MSCs)는 1) 특정조직의 세포로 분화할 수 있는 능력, 2) 주변세포의 생존, 증식 및 기능회복을 촉진하는 다양한 성장인자를 분비하는 파라크린 효과, 3) 면역반응 조절능력을 가지고 있어 질병치료에 잠재력을 가지는 중요한 세포치료제 공급원으로 인식되고 있으며, 최근 몇몇 세포치료제가 허가를 받았다. 그러나 효과적인 이식세포의 선택, 이식경로, 세포전달시스템 및 치료효과를 지속시킬 수 있는 방법 등은 여전히 불분명하다. 본 연구에서는 현재 진행되고 있는 중간엽줄기세포의 치료효과 및 적용 범위를 증대시킬 수 있는 방법을 모색하고자 하였다. Part I에서는 지방세포로 분화된 중간엽줄기세포의 적용범위를 자가이식에서 동종이식으로 확대하기 위하여 동종이식 가능성 및 안전성을 in vitro에서 임상시험에 이르기까지 포괄적으로 평가하였다. 우선, 지방세포로 분화된 인간유래 중간엽줄기세포는 제2군 주조직적합성복합체 및 보조자극인자인 CD40, CD80 및 CD86를 발현하지 않았으며, 제1군 주조직적합성복합체의 발현은 분화되지 않은 중간엽줄기세포보다 현저하게 감소하였다. 지방세포로 분화된 생쥐 및 인간 유래 중간엽줄기세포는 동종의 면역세포와 공동 배양하였을 때 면역반응이 나타나지 않았다. 또한 in vitro 및 in vivo에서 지방세포로 분화된 중간엽줄기세포로 미리 감작한 면역세포를 다시 동일 세포에 노출하여도 면역반응이 나타나지 않았다. 이상의 결과는 지방세포로 분화된 중간엽줄기세포가 면역원성이 없음을 시사하였다. 지방세포로 분화된 마우스 유래 중간엽줄기세포를 면역시스템이 보존되어 있는 동종 마우스의 피하에 이식하였을 때 면역거부반응의 증거 및 이식부위에서 면역세포의 침윤은 관찰되지 않았으며, 동계 마우스의 피하에 이식하였을 때와 거의 동일한 정도의 지방조직을 형성하였다. 마지막으로 지방세포로 분화된 중간엽줄기세포의 안전성을 평가하는 제1상 임상시험 결과 면역거부반응의 증거는 관찰되지 않았다. 이상의 결과는 지방세포로 분화된 중간엽줄기세포가 주조직적합성복합체의 일치 여부와 상관 없이 손상된 연부조직의 치료를 위하여 “보편적인 공여세포”로 사용될 수 있음을 제시하였다. PART II에서는 이식된 줄기세포의 손상된 부위에서의 손실을 막고 생존을 높이기 위해서 사용되는 피브린글루가 중간엽줄기세포의 생존과 파라크린 효과에 미치는 작용을 평가하였다. 피브리노겐 농도에 따른 중간엽줄기세포의 생존 및 성장인자 분비능력의 차이는 관찰되지 않았으므로, 본 연구에서는 이식부위에서 세포가 장기간 유지되도록 하기 위하여 고농도의 피브리노겐을 사용하였다. 중간엽줄기세포는 피브린글루 내에서 14일 동안 생존하였으며, 중간엽줄기세포에서 분비되는 대표적인 성장인자인 혈관내피세포성장인자와 간세포성장인자를 지속적으로 분비하였다. 피브린글루 내에서 중간엽줄기세포는 염증에 노출되었을 때 성장인자 및 프로스타글란딘 E2의 분비가 현저하게 증가하였으며, 염증반응을 효과적으로 억제하였다. 또한 피브린 글루는 산화성 스트레스로부터 중간엽줄기세포를 보호하였으며, 그 결과 극심한 스트레스에 직접 노출된 섬유아세포까지 보호하였다. 반면에 중간엽줄기세포는 피브린글루 내에서 거의 이동하지 않았다. 이상의 연구결과는 중간엽줄기세포를 피브린글루와 함께 이식하면 피브린글루가 중간엽줄기세포의 생존을 용이하게 하여 파라크린 효과를 장기간 지속시킬 수 있음을 규명하였다. 본 학위논문에서는 중간엽줄기세포의 치료기전 중 분화능력을 이용하여 개발된 지방세포로 분화된 중간엽줄기세포의 임상적 적용을 확대하기 위한 동종이식 가능성을 제시하였다. 또한 세포전달물질로 사용되는 피브린글루가 중간엽줄기세포의 또 다른 치료기전인 파라크린 및 면역조절능력에 미치는 영향을 평가하였다. 본 학위논문의 결과는 중간엽줄기세포의 치료적 적용을 확대하고 발전시키는 데 활용될 수 있을 것이다.;Mesenchymal stem cells (MSCs) possess the therapeutic capacity to differentiate into various cell lineages; to modulate the immune response; and to secrete a wide array of paracrine factors crucial for survival, proliferation, and functional recovery of resident cells. These abilities make them a potentially important source for treating intractable diseases such as GvHD, osteoarthritis, cardiovascular disease, fistulas caused by Crohn’s disease. In order to improve the therapeutic function of MSCs and increase the use of MSC-based cell therapy, a few issues need to be considered. In PART I, the safety and feasibility of the allogeneic application of adipogenic differentiated MSCs were evaluated through comprehensive studies to enlarge their therapeutic use. Adipogenic differentiated MSCs did not express major histocompatibility complex (MHC) class II molecules and costimulatory molecules. Additionally, MHC class I expression was markedly decreased in adipogenic differentiated MSCs. Adipogenic differentiated MSCs from both mice and humans failed to elicit an immune response when cocultured with allogeneic lymphocytes. Splenocytes primed with allogeneic differentiated adipocytes in vitro and in vivo also did not elicit the immune sensitization when re-exposed to the same antigen. Adipogenic differentiated MSCs formed new adipose tissue without immune rejection in the subcutaneous region of MHC-mismatched mice. In addition, no indication of an immune response was observed in a phase I clinical study to evaluate the safety of adipogenic differentiated MSCs on allogeneic recipients. These results suggested that adipogenic differentiated MSCs are not immunogenic, therefore, can be used as a “universal donor” for treating depressed scar in MHC-mismatched recipients. Fibrin glue has been employed in various clinical applications of MSCs because they can facilitate the survival and differentiation of MSCs. However, the effect of fibrin glue on paracrine function of MSCs is largely unknown. Therefore, in PART II, the influences of fibrin glue on paracrine function of MSCs were evaluated. Fibrin glue of high concentration is non-toxic for MSCs and does not affect the growth factor secretion potential of MSCs. MSCs in fibrin glue survived and secreted growth factors such as vascular endothelial growth factor and hepatocyte growth factor over 14 days without additional nutrients. These growth factors and immune modulators, including prostaglandin E2, secreted from MSCs in fibrin glue dramatically increased under inflammatory conditions. Consequently, MSCs in fibrin glue effectively suppressed immune reactions. Fibrin glue additionally protected the MSCs from oxidative stress and prevented human dermal fibroblasts from death by exposure to extreme stress. MSCs within fibrin glue hardly migrated. These results suggest that fibrin glue may facilitate survival of transplanted MSCs, thereby sustaining their paracrine function. The fact that allogeneic application of adipogenic differentiated MSCs is pre-clinically and clinically safe could enlarge the therapeutic use of MSCs. The fact that MSCs in fibrin glue functionally survive well and have therapeutic effects through paracrine mediators provides the helpful information to improve the therapeutic effect of MSCs.-
