Abstract
연구배경 기능성 위장질환의 정신사회적 요소에 대한 관심이 증가하고 있는 가운데 최근에는 물리적 인자인 방사선도 면역체계에 영향을 주어 과민성 장증후군을 유발한다는 주장이 있다. 본 연구는 방사선 노출과 정신사회적 스트레스가 동반될 것으로 예상되는 의료기관의 방사선 작업 종사자들에서 기능성 소화불량과 과민성 장증후군의 유병률을 조사하고 질병의 유병에 영향을 미치는 인자를 알아보고자 하였다. 방법 이화여자대학교부속 목동병원에서 근무하는 방사선 작업 종사자를 대상으로 자가 보고형 설문조사를 실시하였고, 선량계를 이용하여 연간 방사선 노출량을 측정하였다. Pittsburgh Sleep Quality Index와 Brief Encounter Psychosocial Instrument를 이용하여 수면의 질과 스트레스의 수준을 측정하였다. 병원 방사선 작업 종사자에서 방사선 노출량, 스트레스의 수준 등이 기능성 소화불량과 과민성 장증후군의 유병과 연관이 있는지 분석 하였다. 결과 전체 대상자 70명(남성: 58.6%, 평균연령: 37±9세)의 연간 평균 방사선 노출량은 0.56 ± 0.92 mSv이었고, 기능성 소화불량과 과민성 장증후군의 유병률은 각각 31.4%, 22.9%이었다. 다변량 분석에서 기능성 소화불량의 유병과 연관된 요소는 높은 스트레스였다(교차비 6.37, 95% 신뢰구간 1.38-29.53). 과민성 장증후군의 위험인자에 대한 단변량 분석의 결과 통계적으로 의미 있는 위험인자는 없었다. 연간 방사선 노출량이 상대적으로 높은 상위 30%군(≥ 0.6 mSv)과 상대적으로 노출량이 적은 군(70.0%, <0.6 mSv)을 비교하였을 때, 양 군 간에 기능성 소화불량(28.6% vs. 32.7%, p=0.84)과 과민성 장증후군(19.0% vs. 24.5%, p=0.83)의 유병률의 차이는 없었다. 3년간의 누적 방사선 노출량을 확인할 수 있었던 48명을 대상으로, 3년 누적 방사선 노출량의 상위 20%에 해당하는 9.4 mSv 이상(10명) 군과 9.4 mSv 미만(38명) 군으로 나누어 비교했을 때, 양 군 간에 기능성 위장질환(30.0% vs. 31.6%, p=0.92)과 과민성 장질환(40.0% vs. 15.8%, p=0.09)의 유병의 차이는 없었다. 결론 방사선 작업 종사자에서 기능성 소화불량과 과민성 장증후군의 유병률이 유의하게 증가하지는 않았고, 높은 스트레스가 기능성 소화불량의 발생과 연관이 있었다.;Background and Aims: The importance of psychosocial factors has been emphasized as the cause of functional gastrointestinal disorders. On the one hand, physical factors like ionizing radiation have been reported to generate symptoms of IBS. Health care workers inevitably become to encounter many physical hazards including ionizing radiation, and have an increased level of psychological disturbance. This study was designed to investigate the prevalence of functional gastrointestinal disorders of hospital radiation workers and also determine significant factors associated with those results. Methods: From June 2010 to May 2011, radiation workers who are working as technologists and nurses at Ewha Womans University Mokdong Hospital were recruited as subjects. Their whole body doses of radiation were evaluated using the electronic dosimeter. The prevalences of functional dyspepsia (FD) and irritable bowel syndrome (IBS) were assessed by the validated bowel symptom questionnaire. The quality of sleep and stress levels were measured by using PSQI (Pittsburgh Sleep Quality Index) and BEPSI (Brief Encounter Psychosocial Instrument). Classifying the subjects according to the radiation dose, sleep quality, and psychological stress level, their prevalences of FD and IBS were comparatively analyzed. Results: A total of 70 subjects (men: 58.6%, mean age: 37.2±0.9 years) were recruited. Average radiation dose per exposed worker for 1 year was 0.56 ± 0.92 mSv. Each prevalence of FD and IBS was 31.4% and 22.9%, respectively. Between the workers who received individual annual doses ≥ 0.6 mSv (relatively high-dose exposed group, 30.0%) and < 0.6 mSv (relatively low-dose exposed group, 70.0%), there wasn’t any significant difference in their work experience and length of work. There was no difference of prevalence in IBS (19.0% vs. 24.5%, p=0.83) and FD (28.6% vs. 32.7%, p=0.84) between the high-dose and low-dose exposed group. However, compared to the high-dose exposed group, the low-dose exposed group showed higher PSQI scores. But, no difference of stress level was revealed. By multivariate logistic regression analysis, the significant factor associated with the prevalence of FD was their high level of stress (odds ratio =6.37, 95% confidence interval=1.38-29.53). Between the radiation workers with IBS and those without IBS, there was no significant difference of their rotating shift work, radiation exposure level, sleep quality, and stress level. Conclusions: Occupational exposure to low levels of radiation does not seem to be significantly related with IBS and FD, but high stress level seems to be related with FD.
