dc.description.abstract손이나 손목의 수술을 받는 환자에서 시행되는 겨드랑 접근법을 통한 팔신경얼기차단은 외래수술 마취 시에도 조기 퇴원을 용이하게 하는 부위마취방법의 하나이다. 초음파 영상을 통해 차단술을 시행하면 필요한 국소마취제의 양도 줄이면서 신경차단의 질을 높이고 차단 효과를 빠르게 나타나게 할 수 있다. 최근에는 초음파 유도하 팔신경얼기차단 시에 굳이 신경을 하나하나 찾아서 차단하지 않고, 팔신경얼기와 겨드랑동맥, 겨드랑정맥을 감싸는 겨드랑 신경혈관집(axillary sheath) 내에 약물을 주사하는 방법이 시도되고 있다. 이에 우리는 초음파 유도하 겨드랑 접근법을 통한 팔신경얼기차단술에서 정중신경, 자신경, 노신경을 각각 찾아서 차단하는 목표신경근접주위 주사법과 겨드랑 동맥(axillary artery)과 신경을 포함한 신경혈관집 내에 약물을 주입하여 차단효과를 얻는 겨드랑동맥주위 주사법 2가지(동맥 주변 1회 또는 동맥 주변 2회)를 비교하여 시술 및 마취에 소요되는 시간과 신경차단 성공율 및 합병증을 알아보고자 하였다. ASA 1-2에 해당하는 전완부, 손목, 손의 수술이 예정된 78명의 환자를 대상으로 이들을 무작위로 3 군으로 나누었다. 세 군에서 사용한 국소마취제는 2 % Lidocaine 29 mL로 모든 환자에서 동일하게 사용하였다. PN 군(perineural injection group)의 환자들에게는 정중신경, 자신경, 노신경 각각의 신경에 근접하여 각 8 mL의 약물을 주사하였다. PV1 군(perivascular 1-injection group)에게는 겨드랑동맥의 12시 방향에 24 mL의 약물을 주사하였다. 마지막으로 PV2 군(perivascular 2-injection group)에서는 겨드랑동맥의 6시 방향과 12시 방향에 각각 12 mL씩 약물을 투여하였다. 세 군에서 근육피부신경은 5 mL의 약물을 사용하여 별도로 차단하였다. 차단술에 소요된 시간과 효과발현시간을 기록하였다. 일차평가변수는 총 부위마취소요시간으로 이는 차단술에 소요된 시간과 효과발현시간을 합친 것이다. 차단술에 소요된 시간은 PN 군이 PV1과 PV2 군에 비해 길었다. 효과발현시간은 세 군에서 차이가 없었다. 총 부위마취소요시간은 PN 군이 PV1과 PV2 군보다 오래 걸렸다. 성공률, 추가 차단술이 필요했던 환자의 비율은 세 군간에 차이가 없었다. 초음파 유도하 겨드랑 접근법을 통한 팔신경얼기차단술에서 혈관주위에 약물을 주사하는 기법은 목표신경근접주위에 주사하는 것과 효과는 비슷하면서 더욱 시간 효율적인 방법이다. 따라서, 각각의 신경을 찾기 어려운 환자의 경우에 목표신경근접주위 주사법에 대체하여 혈관주위 주사법을 사용해볼 수 있을 것이다.;The axillary brachial plexus block (ABPB) provides a reliable anesthesia for upper extremity surgery. Using ultrasound in peripheral nerve block helps to reduce amount of local anesthetics, improve quality of block and expect faster onset. Recently, instead of conventional technique of blocking 4 individual nerve of axilla by ultrasound (perineural technique), another injection technique such as injecting on periphery of axillary artery in axillary sheath (perivascular technique) is used in axillary brachial plexus block. Thus, we compared perineural technique which is blocking every 4 nerves of axilla individually and perivascular 1-injection and 2-injection techniques of ultrasound-guided axillary brachial plexus block. ASA 1-2, 78 patients undergoing surgery of the forearm, wrist or hand were enrolled after written informed consent. Seventy eight patients were randomly allocated to three groups. The local anesthetic agent (2 % lidocaine) and total volume (29 mL) were identical in all subjects. Patients who were allocated to PN group (perineural injection group) receive injections at median nerve, ulnar nerve, radial nerve respectively, using 8 mL of local anesthetics (LA) for each nerve. For PV1 group (perivascular 1-injection group), injection was done at the 12-O’clock position using 24 mL of LA. For the last, patients of PV2 group (perivascular 2-injection group) got two injections of 12 mL LA each at direction of 12-O’clock and 6-O’clock of axillary artery. For all 3 groups, the musculocutaneous nerve was identified and 5 mL of LA was deposited around the nerve. The performance time and the onset time were recorded. The primary outcome was the total anesthesia-related time (sum of performance and onset time). Secondary outcomes were the success rate of the block, the need rate of rescue block, and incidence of adverse events. PN group showed longer performance time than PV1 and PV2 group. Onset time showed no differences among three groups. The total anesthesia-related time was shorter in PV1, PV2 group than PN group. There were no differences in success rates, percentage of patient who needed rescue block among three groups. An ultrasound-guided perivascular injection technique for axillary block is as effective as, and more time efficient than, perineural injection technique. Therefore, perivascular injection technique may be alternative method for axillary block in case of difficult block related to individual anatomical variation or equivocal nerve location.-
