Comparison of Occlusal contact areas of Class I and Class II molar relationship at finishing using 3D digital models

Abstract

The aim of this study was to compare the occlusal contact areas of Class I and Class II molar relationship at finishing from their initial, ideal set up and final models. 41cases clinically diagnosed as having Class I (22 case) or full cusp Class II (19 case) molar relationship after orthodontic treatment were evaluated. For Class I finishing group, final models and ideal set up models were Class I molar relation, which showed the same molar relation at the initial and had all 4 first premolar extraction. For Class II finishing group, final models and ideal set up model were Class II molar relation, which showed the same molar relation at initial and had extraction of only maxillary first premolars. A 3D laser scanner was used for scanning the models and the data was imported into Rapidform 2004® software to measure the occlusal contact area. The intersecting area of the total posterior segment and individual teeth was calculated. The program automatically calculated the total intersecting contact area in mm². The collected data was processed by computerized statistical analysis using SPSS (Statistical Package for the Social Sciences : SPSS Inc., Chicago , USA ) Ver. 20.0 statistical package program. t-test was conducted for the difference between Class I finishing and Class II finishing. Repeated measured ANOVA was conducted for finding the difference in initial, set up and final models. The posterior occlusal contact area of set up models showed statistical difference between Class I and Class II finishing.(P<0.05) The Class I finishing posterior occlusal contact area showed greater area than in Class II finishing. The molars of set up and final models showed statistically significant differences between Class I and Class II finishing.(P<0.05) The final model showed greater area In Class I finishing. However, the second premolar showed similar contact area between Class I and Class II finishing, not showing any statistical difference. In conclusion, the occlusal contact area of Class I finishing models were greater than Class II finishing group therefore it is deemed to have better occlusal function. From the point of viewing the importance of functional occlusion, this result can be considered when diagnosing orthodontic treatment cases.;본 연구의 목적은 교합력이 교합면적과 관련이 있다고 판단되어, I급과 II급 치료 교합력의 차이가 있을 것이라는 가정 하에, 치료 전, 이상적인 치료 종료 셋업 (이하 셋업), 치료 종료 모델을 이용하여 교합면적을 비교하는 것이다. 1993년부터 2012년까지 이화여자대학교 의료원 부속 목동병원 치과 교정과에 내원한 환자들 중 41명을 대상으로 치료 종료 후 구치 관계가 I급인 환자 22명과 II급인 환자19명의 치료 전, 셋업, 치료 종료 모델을 이용하였다. 스캐너로 각각의 모델을 스캔 하였고 Rapidform 2004® 소프트웨어로 구치부의 전치열 제2소구치, 제1대구치, 제2대구치 교합면적의 합), 각 치아 별 교합면적(제2소구치, 제1대구치, 제2대구치)을 측정하였다. 수집된 자료는 SPSS(Statistical Package for Social Science) Ver 20.0 통계패키지 프로그램을 사용하여 전산통계 처리하였으며, t-test, repeated measured ANOVA를 실시하였다.(p < 0.05) I급 종료군과 II급 종료군의 구치부 교합면적을 비교하였을 때, 셋업 모델에서는 I급 종료군의 구치부 교합면적이 높게 나타났다. I급 종료군과 II급 종료군을 비교하였을 때 셋업 모델과 치료 종료 모델의 구치 교합면적은 I급 종료군에서 더 높게 나타났으며, 통계적 유의하였다.(p < 0.05) 제 2 소구치의 교합면적은 I 급, II급 종료군 모두, 치료 전 모델과 셋업 모델, 치료 종료 모델에서 통계적으로 유의한 차이를 보이지 않았다. 본 연구에서는 II급 종료군보다 I급 종료군에서 더 큰 교합면적을 나타내었다. 따라서 이는 I급 종료군에서 더 기능적인 교합을 보인다고 말할 수 있다. 이러한 결과는 심미적뿐 아니라 기능적 교합이 중요시 여겨지는 요즘에 교정치료 계획 수립 시 도움이 될 수 있을 것이다. 셋업과 치료 종료 모델에서 낮은 교합면적이 나타난 것에 대한 추후 보정 기간 동안의 교합면적 변화 평가와 더 많은 표본을 포함한 연구가 추가적으로 이루어져야 할 것으로 보인다.
