Abstract
본 연구는 류마티스 관절염을 가진 여성들의 신체적 기능장애와 사회적 지지가 심리사회적 적응에 미치는 영향을 파악하여 다른 만성질환에 비해 사회복지적 관심이 미진한 류마티스 관절염을 가진 여성들에게 제공할 실질적인 사회복지 서비스체계 수립의 기초 자료를 마련하는데 기여하고자 하였다. 류마티스 관절염은 관절을 침범하는 만성적이면서도 재발하는 전신성 염증 질환으로 확실한 원인을 알 수 없으며, 관절염 가운데 가장 파괴적인 질환이다. 적절한 치료시기를 놓칠 경우 신체적 기능장애를 가질 수 있다. 류마티스 관절염은 여성에게서 남성보다 3~4배 정도 높은 발생율을 보이며, 퇴행성 관절염과 달리 어느 연령층에서나 발생한다. 류마티스 관절염을 가진 여성들은 가정에서는 배우자 역할, 부모 역할, 가정 돌보기 역할 등과 직장이 있는 경우 직장생활에서 자신이 해야 할 역할을 제대로 수행하지 못하고 있다고 생각하는 등 남성보다 더 많은 갈등을 경험하고 있다. 하지만, 사회문화적으로 아픈 사람을 돌보는 역할은 남성보다는 여성에게 적합한 것으로 간주되어 환자 역할로의 이행을 주저한다고 한다. 따라서, 본 연구는 류마티스 관절염을 가진 대상자들 가운데 여성만을 대상으로 하여 그들의 일반적 특성과 신체적 기능장애, 사회적 지지 및 심리사회적 적응에 대해 살펴보았다. 본 연구의 분석에 사용한 자료는 서울에 위치한 2개 병원에서 치료를 받는 18세 이상 류마티스 관절염 여성 가운데 2004년 4월 12일부터 4월 30일까지의 조사기간 동안 설문지에 응답한 연구대상자로부터 수집한 102개의 설문지이다. 조사도구로는 신체적 기능장애 척도, 사회적 지지 척도, 심리사회적 적응 척도를 사용하였고, 수집된 자료는 SPSS(Statistical Package for Social Science) 10.0 통계 프로그램을 이용하여 기술적 분석, t검증, ANOVA, 단순상관관계 및 다중회귀분석을 실시하였다. 측정도구의 신뢰도 검증을 위해 Cronbach's alpha를 구하였다. 본 연구의 결과는 다음과 같다. 총 102명의 연구대상자 가운데 40대와 50대가 55%를 차지하였고, 교육수준은 고졸이상이 69.7%로 나타났다. 결혼상태는 66.7%가 유배우자로, 종교유무는 종교가 있는 경우가 72.5%로 절반 이상을 차지했다. 연구대상자의 직업은 74.5%가 주부로 나타났고, 류마티스 관절염으로 인한 퇴직 및 전직경험 유무에 대해서는 25.5%가 경험이 있다고 보고하였다. 월평균 치료비에 대한 부담도에 대해서 54.9%가 ‘부담스럽다’고 보고하였고, 가구 월평균 수입은 200만원-300만원 미만이 27.5%로 가장 많았고, 200만원 미만이 40명으로 전체의 39.2%를 차지했다. 유병기간은 5년 미만이 51.0%로 가장 많았고, 15년 이상도 15.7%나 되었다. 연구대상자의 신체적 기능장애 정도는 0-51점(0-60점 척도)으로 분포하였고 평균 9.89(±12.15)로 심하지 않은 것으로 나타났으며, 연령, 교육수준, 가구 월평균 수입에 따라 유의미한 차이를 나타냈다. 연구대상자가 지각하는 사회적 지지는 29-168점(24-180점 척도)으로 분포하였고 평균 91.73(±31.44)으로 높지 않게 나타났으며, 연령, 결혼상태, 가구의 월 평균 수입에 따라 유의미한 차이를 나타냈다. 연구대상자의 심리사회적 적응은 77-186점(39-195점 척도)으로 분포하였고 평균 132.12(±24.13)로 나타났으며, 종교유무, 가구 월평균 수입에 따라 유의미한 차이를 나타냈다. 본 연구대상자의 신체적 기능장애와 사회적 지지의 상관관계는 통계적으로 유의미하지 않았다. 반면에 신체적 기능장애 정도가 낮을수록, 사회적 지지 정도가 높을수록 심리사회적 적응 수준이 높았다. 연구대상자의 신체적 기능장애와 사회적 지지가 심리사회적 적응에 미치는 영향을 파악하기 위한 회귀분석결과 모델은 설명력 45.1%로 유의미하게 나타났다. 신체적 기능장애의 Beta값은 -.325, 사회적 지지의 Beta값은 +.204로 나타나 신체적 기능장애가 사회적 지지보다 심리사회적 적응 수준을 예측하는 영향력이 큰 것으로 나타났다. 또한 일반적 특성 가운데 종교유무의 Beta값은 +.231, 가구 월평균 수입의 Beta값은 +.381로 나타났다. 본 연구를 통하여 논의할 수 있는 임상적·정책적 함의를 살펴보면 다음과 같다. 첫째, 현재 사회복지적 관심이 미진한 류마티스 관절염에 대한 사회복지사들의 관심을 증대시켜 류마티스 관절염을 가진 대상자들에 대한 프로그램 개발을 촉진시키고, 이에 대한 기초자료를 제공한다. 둘째, 본 연구를 통하여 류마티스 관절염 여성의 신체적 기능장애가 대상자들의 심리사회적 적응에 미치는 영향력이 큰 변수로 나타났는데, 이는 사회복지사로 하여금 신체적 기능장애로 인해 낮아진 대상자들의 심리사회적 적응 수준을 향상시키는데 기여하는 프로그램 개발의 중요성을 인식하게 한다. 셋째, 본 연구를 통하여 류마티스 관절염 여성의 연령 및 결혼상태는 사회적 지지 정도에 차이를 나타내는 유의미한 변수임을 알 수 있었다. 이러한 결과는, 의료사회복지 실천현장에서 대상자의 연령 및 결혼상태를 고려한 사회적 지지 프로그램 다양화에 기여한다. 넷째, 본 연구에서 건강전문인의 사회적 지지와 연구대상자들의 심리사회적 적응의 상관관계가 통계적으로 유의미하게 나타났는데, 이는 사회복지사로 하여금 건강전문인과 만성질환을 가진 대상자 사이에서 옹호자로서의 역할을 위한 팀웍 접근의 중요성을 인식하게 한다. 다섯째, 본 연구를 통하여 류마티스 관절염 여성가족의 월평균 수입은 신체적 기능장애와 사회적 지지에 유의미한 차이를 나타내며, 심리사회적 적응에 영향을 미치는 중요한 예측요인임을 알 수 있었다. 이러한 결과는 류마티스 관절염 여성이 질병과 경제적 부담으로 인한 이중부담을 갖지 않도록 만성질환에 대한 국민건강보험법의 합리적인 적용과 같은 정책의 반영을 요청케 한다. 여섯째, 본 연구를 통하여 류마티스 관절염 여성의 교육수준은 신체적 기능장애에 유의미한 차이를 가지는 변수임을 알 수 있었다. 이러한 결과는 교육수준이 높을수록 보다 적절한 대처를 하고 있으며, 이는 류마티스 관절염에 대한 적절한 대처가 신체적 기능장애를 예방할 수 있음을 의미한다. 질병발생 후 사후개입은 그 효과의 감소뿐만 아니라 질병이외의 다양한 문제의 원인으로 작용하여 신체적 기능장애의 중증화 및 다양한 문제를 유발케 한다. 따라서 장기적으로 의료사회복지 정책의 큰 흐름을 예방정책으로 전환하고 예방프로그램을 개발, 강조함으로써 류마티스 관절염과 같이 신체적 기능장애를 동반하는 만성질환에 대해 신체적 기능장애의 중증화 및 그로 인한 새로운 문제의 발생을 미연에 방지하는 예방정책의 중요성을 인식하게 한다. ;Rheumatoid arthritis as one of the chronic illness requiring management in long period of time puts great burden to patients, their family and society. For clients with chronic illness, providing a political social-welfare system and social support is important. The incidence of rheumatoid arthritis (RA) is 3-4 times greater in women compared to men. Women usually struggle with burden of their multiple roles of a wife, a mother, and when they are employed, of an employee. This burden may be doubled when they acquire a chronic disease, such as RA, and may influence their psychosocial adaptation in a great deal. The purpose of this study was to identify physical, psychological, and social factors that influence the psychosocial adaptation of women with RA. The findings from this study may provide important information for the social service for the women suffer an RA. General characteristics of the women with RA, physical disability, social support, and psychosocial adaptation were described and the relationships among these factors were examined. A survey was conducted with 102 women diagnosed as RA using a structured survey tool between April 12th and 30th 2004. Data were analyzed using SPSS (Statistical Package for Social Science) 10.0 for the descriptive statistics, t-tests, ANOVA, correlation and multiple regression. The internal consistency reliability was tested by Cronbach's alpha. Major age groups of the subjects were 40's and 50's (55%). 69.7% of subjects had the high school or higher level of education. About 66.7% of subjects were currently married, and 72.5% had a religion. Major portion of subjects (74.5%) were house wives, and 25.5% had ever quitted or transited their job because of the RA. More than half of the subjects (54.9%) were burdened by the cost of treatment. Forty (39.2%) subjects earned less than two million won, and 27.5% of subjects earned more than two million but less than three million won per month. The number of years since they diagnosed as an RA varied from less than five years (51.0%) to more than 15 years (15.7%). The physical disability ranged from 0 to 51, and the average was 9.89(±12.15) appearing that less severe than previous studies. Several characteristics were significantly associated with the physical disability: patients in their sixties compared thirties, patients with no education compared to middle or higher education, patients earned less than one million compared to patients earned three millions or more reported severer physical disability. The social support ranged from 29 to 168, and the average was 91.73(±31.44). The age, marital status, and monthly income were associated with patient's perceived social support. The psychosocial adaptation ranged from 77 to 186 and the average was 132.12(±24.13). Whether they have religion and their monthly income were associated with their psychosocial adaptation. There was no significant correlation between physical disability and social support. However, there was a significant correlation between physical disability and psychosocial adaptation(r=-.434, p<.01), and between social support and psychosocial adaptation (r=.363, p<.01). Entering physical disability and social support into the model significantly improved the prediction of psychosocial adaptation: 45.1% of the variance of psychosocial adaptation was attributed by the physical disability (Beta=-.325) and the social support (Beta=.204). Additionally, the religion (Beta=.231) and monthly income (Beta=.381) were significant predictors of the psychosocial adaptation. Based on the findings from this study, several conclusions and suggestions were made as follows: (1) Since the physical disability was significantly associated with the psychosocial adaptation, social workers need to develop programs to improve physical disability of the clients. (2) Marital status and age need to be considered when the programs are developed. (3) Since the social support was a significant predictor of the psychosocial adaptation, social workers need to pay more attention to their roles as a support provider. (4) Additionally, as shown in the relationship of monthly income with physical disability, a social support, an adequate financial status is an essential for the psychosocial adaptation of women with RA. Therefore, social workers who are involved in health insurance policies and social welfare policies must pay more attention to the vulnerable population such as women suffer a chronic illnesses and lack of social support.
