유지혈액투석환자에서 부갑상선 초음파검사의 의의

Abstract

이차성 부갑상선기능항진증은 만성신부전증에서 흔히 동반되는 합병증으로 여러가지 원인에 의하여 부갑상선호르몬이 보상적으로 과다분비되는 상태를 말한다. 부갑상선기능항진증을 진단하기 위한 여러가지의 검사방법증 부갑상선 초음파 검사가 비관혈적인 방법으로서 비교적 검사하기가 용이하고 검사의 예민도와 특이도가 높다고 보고되고 있다. 이에 저자는 만성신부전증으로 유지혈액투석치료를 받고 있는 환자에서 초음파검사상 부갑상선비대의 빈도를 알아보고 이의 의의를 알아보고자 본 연구를 시행하였다. 만성신부전증으로 지속적 혈액투석 치료를 받고 있는 관자 29명을 대상으로 부갑상선 초음파 검사를 시행하였고 혈청 부갑상선 호르몬은 면역방사선 계측법(immunoradiometric assay)을 이용하여 intact PTH를 측정하였으며, 혈청 칼슘, 혈청 무기인 및 혈청 alkaline phosphatase도 함께 측정하였다. 수부(手部)의 단순방사선촬영과 골 및 관절통 유무에 대한 조사도 갈이 시행하여 다음과 같은 결과를 얻었다. 1) 부갑상선 초음파검사에서 부갑상선비대가 관찰되었던 환자를 제1군, 부갑상선을 관찰할 수 없었던 환자를 제2군으로, 분류하였는데, 대상환자 중 제1군은 8명(27.6%)이었다. 2) 제1군과 제2군 사이에 임상적. 방사선학적, 생화학적 지표에서 통계학적으로 유의한 차이는 없었다.(p>0,05). 3) 혈청 칼슘과 혈청 부갑상선호르몬간에 제1군에서는 상관관계가 없는데 반하여 제2군에서는 역상관관계(r=-0.72, p<0.001)를 보였다. 4) 혈액투석기간과 혈청 부갑상선호르몬간에는 제1군에서는 정상관관계(r=0.74, p<0.05)가 있었으나 제2군에서는 상관관계가 없었다. 이러한 결과로 부갑상선 초음파검사에서 부갑상선비대가 관찰되었던 군에서는 혈청 칼슘에 의해 조절되는 정상적인 부갑상선호르몬의 분비조절기전에 이상이 생겼을 것으로 생각할 수 있었고, 일단 부갑상선호르몬의 분비조절 기전에 이상이 초래되면 혈액투석기간에 비례하여 부갑상선호르몬의 분비가 증가한다는 사실을 알 수 있었다.;Secondary hyperparathyroidism is a complication of chronic renal failure caused by various causes and the status of compensatory hypersecretion of parathyroid hormone. In view of its relatively high sensitivity, specificity and simplicity, ultrasonography of the parathyroid glands may be the method of choice to evaluate possible hyperparathyroidism in hemodialysis patients. This study was designed to assess the prevalence and the significance of enlarged parathyroid glands assessed by ultrasonography in maintenance hemodialysis patients. Twenty-nine patients on maintenance hemodialysis for end-stage renal disease were examined by ultrasonography of the parathyroid glands. Laboratory data included serum intact parathyroid hormone(immunoradiometric assay), alkaline phosphatase, inorganic phosphatase and calcium. Clinical symptoms such as bone and joint pains and plain radiography of the hands were also checked. Based on the presence or absence of ultrasonographically enlarged parathyroid glands, the patients were divided into two groups, and the respective parameters were compared. The results were summerized as follows: 1) In eight(27.6%) patients, parathyroid sonography revealed one or several (up to four) enlarged parathyroid glands. 2) Patients with enlarged parathyroid glands(group Ⅰ), compared to those with undetected parathyroid glands as assessed by ultrasonography(group Ⅱ), had no statistically significant difference of clinical, biochemical, radiological parameters. 3) No significant correlations existed between the serum calcium levels and serum intact parathyroid hormone in group Ⅰ, but negative correlation(r=-0.72, p<0.001) existed in group Ⅱ. A positive correlations(r=0.74, p<0.05) existed between the duration of hemodialysis and serum intact parathyroid hormone in group Ⅰ,but no significant correlations existed in group Ⅱ. This indicates the development of abnormal calcium-related secretion of parathyroid hormone in patients with enlarged parathyroid glands assessed by ultrasonography.
