망막분지정맥폐쇄의 시력 예후에 관한 연구

Abstract

1984년 11월부터 1992년 6월까지 이화여자대학교 부속병원 안과 외래에 내원하여 검안경 검사, 형광안저촬영술 시행후 망막분지정맥폐쇄증의 진단을 받은 61명 61안을 대상으로 망막분지정맥폐쇄증의 경과와 예후에 영향을 줄 수있는 요인과 시력과의 관계를 알아보기 위해 임상과 문헌을 비교 고찰 하였다. 대상환자의 연령분포는 38세에서 83세 사이로 평균 62.3세였으며 이중 남자가 30명, 여자가 31명 이었다. 망막분지정맥폐쇄의 호발부위는 상이측 분지정맥이고, 가장 흔히 동반되는 질환은 고혈압이다. 황반부종(34안), 황반허혈(20안), 망막신생혈관(10안), 초자체출혈(8안) 및 망막박리(2안)가 합병되어 발생했다. 황반부종이 있는 환자군의 최종시력은 황반부종이 없던 환자군에 비해 저하되었고 시력호전도 불량하였으며, 황반부종이 있는 환자군에서 4분원으로 나타낸 황반부종의 크기가 클 수록 최종시력이 저하되었으며 시력호전이 불량하였다. 한편 황반허혈이 있는 환자군의 최종시력은 황반허혈이 없는 환자군에 비해 저하되어 있었으나 시력호전 양상은 차이가 없었고 황반허혈이 있는 환자군에서 4분원으로 나타낸 황반허혈의 크기와 최종시력, 시력호전 양상과는 각각 상관관계가 없었다. 따라서 형광안저촬영을 이용한 황반부의 정확한 관찰이 시력예후 결정에 중요하며 많은 예의 환자에 대한 추적관찰이 필요할 것으로 사료된다.;To evaluate the interrelationship of visual acuity and branch retinal vein occlusion(BRVO), 61 patients with BRVO who visited Ewha Womans University Hospital from November, 1984 to June 1992 were analyzed. Most commonly involved site was the superotemporal branch vein. The most commonly associated systemic disease was hypertension. Common complications associated with BRVO were macular edema(34 cases), macular ischemia(20), neovascularization(10), vitreous hemorrhage(8) and retinal detachment(2). The visual acuity of patients with macular edema was poor than that of the patients without macular edema. Also, the larger the size of the macular edema in terms of quadrant area, the worse was the final visual acuity. The visual acuity of the patients with macular ischemia was poor than that of the patients without macular ischemia but we could not demonstrate any correlation between the final visual acuity and the size of the macular ischemia in therms of quadrant area.
