Abstract
알코올의 호흡억제 작용으로 알코올 투여시 Pco_(2)증가와 Po_(2)감소를 보이며 이로 인하여 뇌혈관의 확장및 cerebral vascular bed의 저항이 감소하며 뇌혈류량이 이상 항진되고 이러한 현상에 수반되는 대뇌 산소 소비는 감소되며 그 결과 대뇌 glucose 수송계수가 변경된다. 또한, 알코올의 마취 작용으로 대뇌 에너지 소비가 감소하므로 해당작용이 억제된다. 이러한 일련의 과정들을 통하여 급성알코올 중독시 대뇌 과혈당 및 ketosis가 초래된다. 흰 쥐의 복강내에 알코올을 용량별 0.75, 1.5, 3.0 및 6.0g/kg로 단독 일회 투여하여 실험적 급성알코올 중독을 유발시켰을 때 혈중 알코올 농도는 각각 60, 180, 337 및 513mg/dl를 나타냈으며 대뇌 피질 에너지 대사 물질을 정량한 결과 glucose량은 정상 뇌에 비하여 용량 의존적으로 유의적인(P<0.01) 증가를 보였으며 ATP, creatine-p량은 알코올량 3.0과 6.0g/kg투여시 유의적인 증가를 나타냈다. 역으로 lactate량은 정상 뇌에 비하여 알코올량 3.0과 6.0g/kg 투여시 유의적인(P<0.01) 감소를 보였다. 한편, 알코올의 호흡 억제 작용의 생리적 parameter가 되는 Po_(2), Pco_(2) 및 pH에 있어 알코올량 0.75g/kg투여로 Po_(2) 증가와 Pco_(2) 감소 경향을 보이며 pH은 별 변화를 나타내지 않았으며 그외 실험한 알코올 용량에서는 용량 의존적으로 Po_(2) 감소와 Pco_(2) 증가 및 산성화가 유의성있게 관찰되었다. 또한 선천성고혈압쥐에 알코올투여시는 Po_(2)감소가 정상혈압쥐에서 보다 더 크게 나타났다. 이러한 결과에서 알코올 중독 증상 발현은 대뇌 피질 뇌 에너지 대사 물질의 장애와 혈액의 생리적 parameter의 변화가 한 원인이 될 수 있다.;Respiratory inhibition of acute alcohol intoxication resulting in vascular dilation, cerebral blood flow increasing and then, alteration of glucose transport parameter. This study was aimed to evaluate cerebral energy metabolism and physiological parameter of respiratory in a acute alcohol intoxication. Acute alcohol intoxicating model was made by intra-peritoneal injection of alcohol 0.75, 1.5, 3.0, 6.0, g/kg in wistar rat. The concentration of ATP, creatine - P and glucose increased after administration of alcohol 6.0g/kg and inversely lactate decreased. Mechanism of glucose increasing is, first, decreased of glucose metabolite rate, second, alteration of glucose transport parameter. The changes of physiological parameter under alcohol intoxicating caused Po_(2) decreasing and Pco_(2) increasing. It is said that alcohol-intoxicated symptom was appeared by disturbances of physiological parameter and cerebral energy metabolite.
