간호사의 돌봄에 대한 환자의 느낌

Abstract

Caring is at the core of the nursing profession and dicipline. Caring has been accepted as a domain that differs from other health-related profession diciplines, thus the necessity and importance of studying has been increased. But in the mass-media there has been frequently reported a lack of caring, "medical care professions are unkind". Acutally caring has been discarded in the clinicla settings. Clients of nursnig have been ask to us the nursing practice that they can feel cared for by nurses. Therefore it is a primary goal that nurse clients have the feeling cared for by nurse. The priority to attain this goal is that nurse understnad patients' feeling carede for. However it has not been studyed before. Speaking that they feel nurse take care of them, what patients feel about caring by nurse? If we can answer this question, we can provide for clients the caring felt cared for, and caring that they can be given the more satisfaction. By base of upper mentioned statement, researcher studyed the patient's feelig cared for by nurse. Phenomenological method of Van Manen research method was used in this research. This method allow the data included richly participant's experience descrive intactly. Thus one allow understand research phenomenon in complexed context. Also, because it allow reseacher collect not only participant's description but various data, researcher can make get a great number of data. This method have the merit that we can understand the phenomenon deeper and expend the meaning of phenomenon. Research participants were conviniently selected 6 patients in A University Hospital, 4 in medical setting and 2 orthopedic surgery setting. Data were collected on researcher's experience, tracing etymological sources, participant's descriptions, experiential descriptions in art. Participant's description were collected through several interviews per one. Research findings appeared that patient's feeling cared for by nurse was <comfort>, <jung-gam> - the korean word included meaning as like 'loved' and 'warm' in english, <genuine>. 'The theme of <comfort> was included <comfort in body>, <comfort in mind), <secure>. The theme of <jung-gam> was included <warm> and <feeling a jung> - jung was a unique feeling that appeared in only korean culture; it have a meaning as like 'loved' in english. The Theme of <genuine> was included <feeling a genuineness> and <feeling a sincerity>. Researcher hopes this study helps nurses understand patient's feeling cared for and nurse provided for patients better satisfactory nursing practice and contribute to develop the body of caring knowledge.;돌봄은 간호학과 간호 전문직의 중심이다. 돌봄은 간호와 다른 건강 관련 전문직을 구분짓게 하는 독특한 연구 영역으로 수용되고 있으며 이에 따라 돌봄 연구의 필요성과 중요성이 대두되고 있다. 그러나 대중 매체는 "의료인이 불친절하다"는 말로 돌봄의 결핍을 보도하고 있다. 사실상 돌봄은 간호 실무에서 사라져 가고 있다. 간호의 소비자들은 가장 기대하는 것은 간호사가 자신을 돌보아 주었다고 느끼게 하는 것이다. 따라서 간호 대상자가 돌봄을 받았다고 느끼게 하는 것은 간호 실무의 일차적 목표라 할 수 있다. 이러한 목표를 달성하기 위해서 가장 우선해야 하는 것은 간호사가 대상자의 돌봄 느낌을 이해하는 것이다. 그러나 간호사의 돌봄에 대한 환자의 느낌은 조사된 바 없다. 간호사가 돌봐준다고 느꼈다고 말할 때, 환자는 간호사의 돌봄에서 무엇을 느꼈는가? 이와 같은 질문에 답이 밝혀질 때 우리는 환자가 간호사가 돌봐주었다고 느끼는, 곧 보다 만족스러운 간호를 제공할 수 있을 것이다. 이와 같은 근거에 의하여 간호사의 돌봄에 대한 환자의 느낌을 이해하기 위하여 Van Manen의 현상학적 방법을 이용하여 본 연구를 시도하였다. 이 방법은 현상의 의미가 변질되거나 퇴색되지 않게 하기 위하여 대상자의 경험을 풍부하게 담고 있는 자료를 그대로 서술하여 다양한 상황에서 현상을 이해를 하게 한다. 또한 대상자의 진술뿐 아니라 다양한 출처에서 자료를 수집하기 때문에 풍부한 자료를 획득할 수 있게 한다. 그럼으로써 현상을 보다 깊이 있게 이해할 수 있게 하며 현상의 의미를 확대할 수 있다는 장점이 있다. 연구 대상은 A 대학 병원에 입원한 내과와 정형외과 환자 6인이었다. 자료는 연구자의 경험, 언어적 의미 조사, 예술 작품에 나타난 주제 묘사, 대상자의 진술에서 수집되었다. 언어적 의미 조사는 연구와 관련된 단어의 사전적 의미를 조사하였으며 예술작품에 나타난 주제 묘사는 연구대상자와 비연구대상자에게 돌봄을 느낀 예술적 작품의 예를 수집하였다. 대상자 진술은 여러 번의 심층 면접을 통해 이루어졌다. 면구 결과, 간호사 제공한 돌봄의 느낌은 <편안함), <정감>, <정성을 느낌> 으로 나타났다. <편안함>의 범주에는 마음의 편안함, 몸의 편안함, 안심이 포함되었으며 <정감>의 범주에는 따뜻함과 정을 느낌이 포함되었으며 <정성을 느낌>은 성의를 느낌, 정성을 느낌이 포함되었다. 본 연구는 환자의 돌봄 느낌을 밝힘으로서 간호사가 환자의 돌봄 느낌을 이해하도록 돕고 이를 기초로 하여 돌봄을 받았다는 느낌을 주는, '곧 보다 만족감을 주는 간호 실무를 수행하도록 도울 것이며, 돌봄 지식체 개발에 기여할 것으로 기대된다.
