정시안 및 비정시안에서의 사위에 대한 비교연구

Abstract

양안이 융합으로 부터 중단될때, 안위는 정위(orthophoria)를 나타내는 것은 드물고, 정도의 차이는 있으나 대부분은 편위되어 사위(heterophoria)를 나타낸다는 것은 잘 알려진 사실이고 또 이러한 사위가 안정피로의 원인이 될 수 있고, 사시로 발전할 수 있으므로 임상에서 흔히 측정한다. 정시안과 사위의 원인중 하나로 간주되는 비정시안을 대상으로 교정전 후의 사위에 대한 연구 결과 다음과 같은 결론을 얻었다. 정시안에서는 84.4%, 비정시안 중 근시안에서는 89.2% 원시안은 91.4%의 사위가 나타나 비정시안에서 사위의 빈도가 높은 경향을 보였으며, 비정시안에서 교정후에 근시안은 85.0%, 원시안은 85.8%의 사위가 나타나 교정전보다 약간 낮았으나 통계학적으로 유의성이 없었다. 각 사위별 빈도는 근시안에서는 외사위(exophoria)가 60.5%로 월등히 많았으나 정시안과 원시안에서는 내사위(esophoria)와 외사위의 빈도에 큰 차이가 없었으며, 상사위(hyperphoria)는 정시안에서 32.6%, 근시안에서 33.5%, 원시안에서는 34.2%로 나타났고, 회선사위 (cyclophoria)는 정시안과 비정시안에서 10%내외로 비슷하게 나타났다. 비 정시안에서 교정전과 교정후의 사위의 정도를 비교시, 교정후 약간 감소 하였으나 통계학적인 유의성은 없었다. 또한 굴절이상정도와 사위정도와의 연관성을 조사한 결과 통계학적인 유의성을 발견할 수 없었다.;The purpose of this study was to examine the heterophoria in emmetropia and ametropia. The subjects were 337 healthy men and women between the age of 20-41 years. In 337 cases, Emmetropia were 135 cases, Ametropia were 202 cases. The entire group was tested for heterophoria with synoptophore. In the ametropia, this test was performed before and after correction of refractive error with glasses. The results were as follows : 1. The incidence of heterophoria was 84.4% in emmetropia, 89.2% in myopia and 91.4% in hyperopia. 2. Exophoria was most common in myopia. But in emmetropia and hyperopia, there was no significant difference between the incidence of exophoria and esophoria. The incidence of hyperphoria and cyclophoria was not significantly different between emmetropia and ametropia. 3. In the ametropia, the size of heterophoria was slightly decreased after correction, but the difference was not significant (p>0.05). 4. There was no correlation between the amount of refractive error and the size of heterophoria (p>0.05).
