Abstract
Prolactin은 시상하부의 영향에 의하여 뇌하수체의 호산성 세포에서 분비되는 폴리펩티드 호르몬으로 비임산부에서 수술과 마취, 운동의 stress에 의해 증가되나 임산부에서는 진통중 저하되어 분만시 최저치를 보였다가 분만후 다시 증가된다. 한편 Cortisol은 진통의 stress에 의하여 점차 증가하여 분만시 최고치에 달하게 된다. 본 연구는 1991년 1월부터 1991년 9월까지 9개월간 이화여자대학교 부속병원 산부인과에 분만을 목적으로 입원한 만삭 임산 과정 중에 아무런 이상이 없었던 자연경질 분만례 20명, 유도 분만례 20명, 선택적 제왕절개 분만례 16명, 응급 제왕절개 분만례 16명 등 총 72명을 대상으로 분만전과 분만후 산모와 태아 혈중 Prolactin 치와 Cortisol 치를 측정하여 비교한 바 다음과 같은 결과를 얻었다. 1. 선택적 제왕절개군의 수술시작전 모체 혈중 Prolactin 평균치(297.91±30.81 ng/ml)는 경질분만군과 응급 제왕절개군의 진통시작전 Prolactin 평균치(291.13±26.30 ng/ml, 305.37土21.78 ng/ml)와 유의한 차이가 없었다. 진통시작전 혹은 수술시작전 모체 혈중 Cortisol 평균치는 경질분만군(34.95土3.34 ㎍/dl), 선택적 제왕절개군(36.15土2.55 ㎍/dl) 그리고 응급 제왕절개군(33.10±3.19 ㎍/dl) 사이에 유의한 차이가 없었다. 2. 경질분만군과 응급 제왕절개군에서는 분만이 진행되는 동안 모체 혈중 Prolactin 치가 유의하게 감소하였으나, 선택적 제왕절개군에서는 모체 혈중 Prolactin 치가 증가하였다. 분만이 진행되는 동안 모체혈중 Cortisol 치는 경질분만군과 응급 제왕절개군에서는 유의하게 증가하였으나, 선택적 제왕절개군에서는 유의한 변화는 볼 수 없었다. 3. 경질분만군에서 분만 진행에 따른 모체 혈중 Prolactin 치의 감소와 Cortisol 치의 상승은 경산부보다 초산부에서 유의하게 변화하였다. 4. 제대 혈중 Prolactin 치는 경질분만군, 선택적 제왕절개군과 응급 제왕절개군사이에 유의한 차이는 없었다. 그러나 선택적 제왕절개군의 제대 혈중 Cortisol 평균치(6.48±1.92 ㎍/dl)는 경질분만군과 응급 제왕절개군의 제대 혈중 Cortisol평균치(20.90±4.89 ㎍/dl, 19.95±2.29 ㎍/dl) 보다 유의하게 낮았다.;The effect of stress during labor and delivery on the plasma concentration of prolactin and cortisol was studied in 72 healthy parturient women and their 72 newborn infants. The plasma concentration of prolactin and cortisol was measured by radioimmunoassay method in 20 patients who were in spontaneous labor, 20 patients who were electively induced at term with oxytocin, 16 patients undergoing elective cesarean section without labor and 16 patients undergoing emergency cesarean section with labor. Determination of maternal plasma prolactin and cortisol levels in the spontaneous or induced labor groups and emergency cesarean section group was made before onset of labor and repeated immediately after birth. Maternal determination in the elective cesarean section group was made just prior to the procedure and immediately after birth. Cord blood was taken immediately after birth, with no attempt to separate it into venous and arterial components. The results were as follows: 1) The mean initial prolactin level in the elective cesarean section group (297.91±30.81 ng/ml) was not significantly different from the mean initial level of the vaginal delivery group(291.13±26.30 ng/ml) or the emergency cesarean section group (305.37±21.78 ng/ml). And there were no significant differences in the mean initial cortisol levels among the vaginal delivery group (34.95±3.34μg/dl), the elective cesarean section group (36.15±2.55μg/dl), and the emergency cesarean section group (33.10±3.19μ9/dl). 2) Significant fall in maternal prolactin levels was noted immediately after birth in all of the spontaneous, induced labor group, and emergency cesarean section group, whereas significant rise of maternal plasma prolactin level was noted in the elective cesarean section group. And marked rises of maternal plasma cortisol levels were noted immediately after birth in all of the spontaneous, induced labor group, and emergency cesarean section group, whereas slight change occured in the elective cesarean section group. 3) During labor and delivery, a considerable fall in plasma prolactin level and a considerable rise in plasma cortisol level were more marked in primiparous than multiparous patients. 4) There were no significant differences in the mean cord prolactin levels among the vaginal delivery group, the elective cesarean section group, and the emergency cesarean section group. But the mean cord cortisol level in the elective cesarean section group with no prior labor(6.48±1.92μg/dl) was significatly less than the value in the vaginal delivery group(20.92 ±4.87μg/dl) or the emergency cesarean section group(19.95±2.29μg/dl). These results give futher support to the hypothesis that severe stress during labor and delivery may lead to reduced concetration of prolactin and increased concentration of cortisol in the maternal plasma.
