알레르기성 비염의 임상적 연구

Abstract

For diagnosis of allergic rhinitis, various kinds of diagnostic procedures are available, such as characteristic history, physical examination, X-ray, eosinophil in peripheral blood, eosinophil and basophil in nasal secretion, serum IgE level, skin test, nasal provocation test and RAST. To evaluate the diagnostic significance of each procedure in allergic rhinitis, a clinicostatistical study was done with 80 cases who had visited the Dept. of Otolaryngology, N.M.C. presenting symptoms of allergic rhinitis during the period between February 1987 to August 1988. The results were obtained and summarized as follows : 1) Among 80 cases, male to female ratio was 2.2:1 and peak age incidence was in the age group 11 to 20 (41.25%). 2) The most frequent rhinoscopic finding was congested swelling of nasal mucosa (48.75%), and the next was pale nasal mucosa (18.75%). 3) Total serum IgE value of allergic rhinitis group was average 623.65IU/ml and that of normal control group was 124.70IU/ml. 4) Blood eosinophilia was seen in only 21.25% and it has no diagnostic significance in allergic rhinitis. 5) Positive rate of nasal smear for eosinophil and basophil was 95.00%. 6) Positive rate of allergic skin test was 58.75%. The most common antigens were in the following order : House dust 31.45%, Dermatophagoides farinae 16.13%, Dermatophagoides pteronyssinus 13.71%, Dog hair 9.68%. 7) There was intimate relationship between eosinophil and basophil count of nasal secretion (r=0.74). 8) There was intimate relationship between severity of clinical nasal symptoms and basophil count of nasal secretion (r=0.87). 9) Among 80 cases of allergic rhinitis, 86.25% showed nasal smear eosinophilia, and 66.67% of these revealed positive allergic skill test.;알레르기성 비염은 알레르기성 질환중 대부분을 차지하는 것으로, 발작성재채기, 수양성 비루, 비폐쇄 및 비강소양증을 주증상으로 하고 국소소견상으로는 비갑개점막의 종창과 창백을 나타내며 비점막의 산도 및 lysozyme의 감소로 인한 2차 감염이 용이하며, 비용의 발생, 이관의 종창에 따른 장액성 중이염을 유발시키는등 이비빈후과 영역에서 가장 중요한 질환의 하나이다. 이러한 알레르기성 비염의 근본치료로서 회피요법과 면역요법을 들 수 있으며, 효과적인 치료를 위해서는 그 알레르기성 질환에 대한 정확한 진단이 우선되어져야 할 것이다. 이에 저자는 알레르기성 비염의 소견을 가진 80명을 대상으로, 문진, 이학적검사, 혈청 IgE치, 말초혈액내 호산구수, 비즙내 호산구 및 호염구수, 피부반응검사를 실시하여 다음과 같은 결과를 얻었으며 그 결과들을 상호 비교·관찰하여 각각의 진단적 의의에 관하여 분석하였다. 1. 남녀의 비는 2.2 : 1이었으며, 연령별로는 11∼20세가 33명으(41.25%)로 가장 많았다. 2. 비강검사상, 비점막의 종창만 발견된 경우가 39명(48.75%)으로 가장 많았으며, 비점막이 창백한 경우는 15명(18.75%)이었다. 3. 조사군 총 80명의 혈청 총 IgE치는 평균 623.65 IU/ml 로서, 정상대조군 20명의 평균치인 124.70 IU/ml와는 통계적으로 유의한 차이를 보였다(p＜0.05). 4. 혈액내 호산구 증가는 17명(21.25%) 에서 관찰되었다. 5. 비즙내 호산구 및 호염구는 조사군 총 80명중 76명(95.00%)에서 발견되었으며, 10개이상의 세포수 증가를 보인 예도 호산구의 경우 69명(86.25%), 호염구의 경우 64명(80.00%)에서 관찰되었다. 6. 피부반응검사에서 양성이 47명(58.75%) 이었으며, 기인성 항원의 빈도를 보면 집먼지가 31.45%로서 가장 많았고, 집먼지 진드기인 Dermatophagoides farinae 와 Dermatophagoides pteronyssinus가 각각 16.13%, 13.71%이었다. 7. 비즙내 호산구수와 호염구수 사이에는 통계적으로 유의한 상관관계가 있었다(r = 0,74). 8. 비즙내 호천구수와 임상증상의 정도사이에는 통계적으로 유의한 상관관계가 있었다 (r = 0.87). 9. 비즙내 호산구수와 혈액내 호산구수의 사이에는 유의한 상관관계가 없었다 (r = 0.18 ). 10. 비즙내 호염구수와 혈청 총 IgE치 사이에는 유의한 상관관계가 없었다(r = 0.16). 11 .비즙내 호산구증가를 보인 경우는 조사군 총 80명중 69명(86.25%)이었으며, 이들의 단자검사 양성반응율은 66.67%(46명)로서, 비즙내 호산구 증가가 없는 11명(13.75%)에 대한 단자검사 양성반응율인 9.09 %(1명) 보다 높았다.
