척추손상환자의 무력감에 관한 연구

Abstract

People who are ill often experience fellings of powerless ness. These fellings may be expressed as lack of control over various aspects of their lives. The more serious the illness the more powerless patients may feel when care usually managed by oneself must be received from a care giver. People who have suffered a spinal cord injury may feel particularly powerless due to paraplegia or quadriplegia. Nursing has a holistic view of human beings but quantitative research of powerlessness may have a limited and a culturally specific view of the concept. The purpose of the study was to provide theoretical understanding of spinal cord injury patients perceptions of powerlessness and thus to contribute defining characteristics and related factors to "powerlessness." The subjects were 20 patients with a spinal cord injury admited to the rehabilitation and neurological surgery unit of two university hospitals in Seoul. Data were collected from September 11 to 27, 1989 by nonstructured interviews by the investigation ; interviews were tape-recorded. Data analysis was done by Van Kaam's phenomenological method. The resultes of study are summarized as follows. 1. Spinal cord injury patient's moaning of powerlessness (23 raw data, 3 subthemes, 1 theme) 1) inability-no power; physical immobility; social inactivity. 2. The situation of powerlessness. (68 raw data, 4 subthemes, 2 themes) 1) disturbance of physical function-disturbance of dalily activity; loss of role. 2) inadequate support-function-oriented attitude of the helth care team; insincere altitude of family. 3. Reaction to the powerless situation (86 raw data, 8 subthemes, 2 themes) 1) emotional reaction-depression; decrease of self-esteem; self-abandonment; anger; avoidance. 2) physiological reaction-loss of appetite; sleep disturbance; fatigue 4. Coping style to powerlessness (61 raw data, 8 subthemes, 2 themes) 1) support-patients; family; health care team; visitors; religion. 2) self-conquest-self-control; positive altitude to treatment; self-reflection. The results contribute relevant defining characteristics and related factors to "powerlessness," and theoretical understanding grounded in subjective experience of spinal cord injury patients as a basis for planning future research directed at nursing intervention.;질병은 개인의 신체적, 심리적, 시회적 기능을 제한시키고 구속하므로써 무력감을 유발시킨다. 특히 척수손상환자는 신체의 부분 혹은 전신마비로 인해 신체기능의 일부 혹은 전체를 상실하게 되어 많은 무력감을 경험한다. 질병과 입원으로 야기된 무력감은 건강회복에 부정적인 영향을 미칠 수 있으므로 척수손상환자를 간호하는 간호사에게 있어 무력감은 중요한 관심사이다. 한편 무력감은 느낌, 감정이므로 수량화하기에 어려움이 있으므로 질적연구가 중요시 되어지는 개념이다. 이에 본 연구는 무력감에 대한 연구가 부족한 상태에서 척수손상환자의 직접 경험을 대상자의 관점에서 기술하고 분석하여 무력감에 대한 이해를 증진하고자 하며, 나아가 간호중재를 계획하는데 도움이 되고자 시도하였다. 연구대상은 서울시내에 있는 2개 대학병원의 재활원, 신경외과 병동에 입원한 척수손상환자 20명을 대상으로 하였다. 자료수집기간은 1989년 9월 11일부터 9월 27일까지였으며, 비구조적 면접을 통해 자료를 수집하였다. 수집된 자료는 Van Kaam의 방법을 이용하여 분석하였으며, 분석된 내용의 신뢰도와 타당도는 간호학과 박사과정생 4인, 간호학과 석사과정생 3인, 국문학과 석사과정생 1인에 의해 검정되었다. 본 연구를 통하여 얻은 결과는 다음과 같다. 1 . 척수손상환자가 지각한 무력감의 의미는 힘이 없는 상태, 신체적 부동, 역할불능으로 인한 무능력함으로 범주화되었다. 2 . 척수손상환자가 무력감을 느끼는 상황은 일상생활의 장애, 역할수행능력의 상실인 신체기능장애와 의료진의 기능중심적인 태도, 가족의 무성의한 태도인 부적절한 지지로 범주화되었다. 3 . 척수손상환자가 무력한 상황에서 나타난 반응은 우울, 자존감 저하, 자포자기, 분노, 회피의 정서적 반응과 식욕감퇴, 수면장애, 피로의 생리적 반응으로 범주화되었다. 4 . 척수손상환자의 무력감에 대한 대처양상은 환자, 가족, 의료진, 지인(知人), 종교를 통한 지지와 자제, 적극적인 치료자세, 자기반성을 통한 자기극복으로 범주화되었다. 이상의 결과에서와 같이 척수손상환자가 경험하는 무력감 현상은 신체적, 사회적으로 무능력함을 의미하고 있으며, 신체적 통제력을 상실한 상황에서 무력감을 많이 경험하는데, 이러한 상황에서는 대부분 우울과 분노 등의 정서적 반응을 나타내었다. 또한 무력감을 극복하기 위해서 자원을 통한 지지와 자신을 통한 자기극복으로 대처하고 있었다. 따라서 간호사는 환자 개인의 느낌을 표현하고 서로의 의견을 나눌 수 있는 환자 group을 형성하고, 기능중심적 태도보다는 환자의 정서상태에 관심을 기울여야 하며, 무력감을 이해하기 위한 교육프로그램에 가족을 포함시킴으로써 척수손상환자의 무력감에 대한 간호를 제공해야 할 것이다.
