입원환자의 영적간호요구에 관한 연구

Abstract

본 연구는 기독교인 입원환자가 바라는 영적간호요구의 내용을 파악하여 이에대한 기대정도와 실제 간호원으로부터 중재받는 수혜정도, 기독교신앙의 성숙정도와의 관계및 일반적 배경과 관련된 제변수와의 관계를 알아봄으로서 영적간호중재를 위한 기초자료로 제시하고자 시도되었다. 먼저 영적간호요구와 기독교신앙의 성숙정도를 측정하기위한 도구작성이 선행되었다. 이를위한 자료수집은 1987년 2월 7일부터 4월 10일까지의 53일동안 세단계에 걸친 과정을 통하여 이루어졌다. 도구작성을 위해 참여한 대상자는 간호교육과 실무에 있어서 이 분야의 전문가라고 할 수 있는 간호학교수및 임상간호원, 성직자, 평신도, 입원환자중에서 선정되었다. 첫번째 과정에서는 "만일 귀하자신이 중환으로 입원하셨다고 가정하셨을때 간호원이 어떤 영적간호를 해주면 좋을지 바라시는 내용을 모두 구체적으로 말씀해주시겠읍니까? "와 "기독교신자의 신앙이 얼마나 깊은지 혹은 성숙한지를 측정하기위해 포함되어야 할 내용에는 어떤 것들이 있다고 생각하십니까? "라는 두가지 개방식 질문지를 사용하였다. 두번째 과정에서는 첫번째 조사로 얻은 자료를 의미별로 분류한후 비슷한 내용끼리 모아서 내용의 타당도를 조사하기위한 폐쇄형 질문지를 만들어서 사용하였다 이를 통해 얻어진 응답내용과 코멘트를 근거로 삭제, 첨가, 수정및 보완한 예비도구를 작성하였다. 마지막으로 예비도구의 내용에 대한 타당성과 이해력을 Likert형의 5점 척도로 조사, 평가하는 과정을 거쳐 최종도구로 확정하였다. 이러한 과정을 거쳐 작성된 도구의 내적신뢰도를 검정하기 위하여 Cronbach's Alpha Coefficient Analysis를 사용하여 문항 분석한 결과 총 20문항으로 구성되어있는 영적간호요구 측정도구는 α = .8671이었으며 총 16문항으로 구성되어있는 기독교신앙의 성숙정도 측정도구는 α = .9327 이었다. 이렇게 구성된 36개의 문항과 대상자의 일반적 특성에 관계된 제변수를 포함하여 총47문항으로 구조화된 질문지를 연구도구로 사용하였다. 자료수집기간은 1987년 4월 20일부터 30일까지 11일간이었으며 수집된 자료는 평균, t - Test , ANOVA, Pearson Correlation Coefficient등의 통계방법으로 분석하였는데 그 결과는 다음과 같다. A . 영적 간호요구 1 . 영적간호요구의 내용 예비조사를 거쳐 영적간호요구를 측정하기위한 최종도구로 확정된 총 20개문항의 영적간호요구 내용은 부록Ⅰ- 3과 같다. 영적간호요구의 내용구성은 간호원의 자질및 태도에 관한 4문항과 구체적인 영적간호활동에 관한 16문항으로 되어있다. 2 . 영적간호요구의 기대정도 총20문항에 걸친 영적간호요구내용중 "기도나 묵상, 명상을 하거나 편히 쉴 수 있는 환경조성에 힘써주었으면 좋겠다"에 대한 기대가 최대평점 5점 만점에 4.367점으로 가장 높았으며 그 다음으로는 "예의있고 능숙한 처지로서 육체의 고통을 이해하고 해소하고자 내입장에서 최선을 다해 노력해주면 좋겠다( 4.327점 ) " , "내가 기도하고 말씀 묵상하는것을 격려해주고 나 자신과의 싸움에서 포기하지 않도록 격려해주면 좋겠다( 4.296점 ) " , "희망을 갖도록 용기를 북돋아주기 원한다( 4.286점 ) " , "나에대한 관심을 가지고 미소로서 친절하게 대해주면 좋겠다( 4.245점 ) " , "나의 이야기를 잘 들어주면 좋겠다( 4.194점 ) " , "나를 한 인간으로서 존중해주고 존경과 존엄성을 가지고 대해주면 좋겠다( 4.193점 ) " , "병원규칙을 최대한 완화하여 안정감있는 환경, 가정과 비슷한 분위기를 만들어주면 좋겠다( 4.163점 ) " , "조용하고 여유있는 태도로 대해주면 좋겠다( 4.133점 ) " , "간호절차를 친절히 설명해주면 좋겠다 ( 4.082점 ) " , "나를위해 혹은 나와함께 기도해주면 좋겠다( 4.051점 ) " , "안심시켜주고 위로해주면 좋겠다( 4.020점 ) "의 순으로 12개의 문항이 4.0점이상의 높은 평균값을 나타내보였다. 그리고 가장 낮은 기대정도를 보인 문항은 3.296점을 얻은 "내가 원할때 함께 있어주면 좋겠다"였다. 전체적으로볼때 대상자들이 바라는 영적간호요구의 기대정도는 평균 3.949점으로서 비교적 높은편으로 나타났다. 3 . 영적간호요구의 수혜정도 총20문항에 걸친 영적간호요구 내용중 간호원으로부터 받은 수혜정도가 가장 높게 나타난 문항은 최대평균 5점 만점에 4.214점을 얻은 "나에대한 관심을 가지고 미소로서 친절하게 대해준다"였으며 그 다음으로는 "조용하고 여유있는 태도로 대해준다( 4.133 ) " , "나의 이야기를 잘 들어준다( 4.061 ) "의 순서로 높게 나타났다. 수혜정도가 가장 낮은 문항은 "내가 원할때 옆에서 성경을 읽어준다( 1.531점 ) " 였으며 그 다음으로는 "내가 원할때 직접 찬송가를 불러주거나 찬송가 테이프를 틀어준다( 1.551점 ) " , "나를 위해 혹은 나와 함께 기도해준다( 1.816점 ) "등이었다. 이와같이 일반적 내용의 영적간호요구에 대한 수혜정도는 높게 나타났으나 기독교신앙과 관련된 내용의 영적간호요구에 대한 수혜정도는 낮게 나타났다. B. 영적간호요구와 제변수와의 관계 영적간호요구의 기대및 수혜정도와 기독교 신앙의 성숙정도간에는 순상관관계가 있는 것으로 나타났다. 즉 기독교신앙이 성숙한 사람일수록 영적간호요구에 대한 기대가 높으며 간호원으로부터 영적간호중재를 많이 받은 것으로 여긴다는 사실을 나타내보여 주었다. 영적간호요구의 기대정도와 일반적 제변수간에는 통계적으로 유의한 관계가 없는 것으로 나타났으며 수혜정도와 유의한 관계가 있는 것으로 나타난 것은 입원기간과 수술후기간이었다. 즉 입원기간이나 수술후기간이 오래되었을수록 간호원으로부터 제공받는, 영적간호요구의 수혜정도가 높아지는 것을 알려주었다. 본 연구의 결과는 기독교신자인 입원환자가 바라는 영적간호요구의 기대정도는 그가 가진 신앙의 성숙정도와 정비례하며, 전체적으로 높은편이나 기독교신앙과 관련된 내용의 영적간호요구에 대한 수혜정도는 그에 미치지 못하고 있음을 보여주었다. 그러므로 대상자가 기독교신자인 경우, 그가 가진 기독교신앙의 성숙정도에따라 그의 신앙과 관련된 내용의 영적간호요구에 더욱 관심을 가지고 적절하게 중재한다면 보다 효과적인 영적간호를 제공하는데 도움이 될 것이라고본다.;The problem addressed by this study was to identify Christian impatient's spiritual nursing care needs, the degree of expected spiritual nursing care and the actual care being received. Another objective was to determine the relationships between expected and received spiritual nursing care and the degree of maturity in the Christian faith and other general characteristics of Christian inpatients. The purpose was to contribute to the theory relates to spiritual nursing care. A pilot study was carried out to develop two tools to measure spiritual nursing care needs and the degree of maturity in the Christian faith using an adaptation of the Delphi Technique. Three rounds of data collection were processed from February 17 to April 10, 1987. The target population for the pilot study were Christian nurses those who were experts in the field of nursing education and practice, clergy, layity and inpatients. The questions, "Would you tell me the kinds of spiritual care you want to receive from a nurse if you are hospitalizied due to severe illness?" and "Would you tell me what components to use to measure the maturity or depth of Christian faith?" were used for round Ⅰ. Closed questionaires which were designed to assure reliability were used for rounds Ⅱ and Ⅲ. Reliability of the tools were examined by Cronbach's Alpha Coefficient Analysis. The results were as follows : 1. Alpha of the tool to measure spiritual nursing care needs was .8671. 2. Alpha of the tool to measure the degree of maturity of Christian faith was .9327. The subjects of this study were 98 Christian inpatients hospitalized in 6 different general hospitals in Seoul. Data were collected from April 20 to April 30, 1987. throug a structured questionaire. Collected data were analyzed by use of Mean, ANOVA, t-test and Pearson Correlation Coefficient through an SPSS^(X) Program. The results included the following : A. Spiritual Nursing Care Needs, expectations and actual care. 1. Spiritual nursing care needs about the nurse's attitude (4 items) and about concrete activities (16 items) were identified. 2. Out of 20 items which were designed to measure the degree of expected spiritual nursing care, "I want the nurse to exert herself to provide an appropriate environment for me to pray, to mdeditate and to rest" obtained the highest point, 4.367 out of 5. The Lowest point, 3,296 was "I want the nurse to stay with me when I need it". As a whole, expected spiritual nursing care averaged 3,949 points in a range from 1 to 5. 3. Out of 20 items which were designed to measure the degree of spiritual nursing care being received, "The nurse showed kindness smiled and paid attention to me" obtained the highest point, 4.133 out of 5. The lowest point, 1,531 was "The nurse reads Bible to me when I want it." As a whole, the degree of received spiritual nursing care averaged 3,031 points in a range from 1 to 5 but the degree of received spiritual care related to worship or religious ritual was low (1.9938). B. Relationships between spiritual nursing care needs and other variables 1. There was a positive correlation between the expected and received spiritual nursing care and the degree of maturity in the Christian faith. The greater the degree of maturity in the Christian faith, the greater the degree of expected and received spiritual nursing care. 2. No significant difference was found between the expected spiritual nursing care and the general characteristics of the subjects. 3. There was a significant relationship between the degree of actual spiritual nursing care received and the admission date or date of operation. The longer the hospitalization or the time since the operation, the greater degree of actual spiritual care received. Results of this study showed the greater the degree of maturity in the Christian faith, the greater the expectations of spiritual nursing care. The expectations of spiritual nursing care were hish, but the actual spiritual nursing care received related to Christian worship or ritual was low. Effective spiritual nursing care should take into account the importance of the patient's maturity in the Christian faith
