콘택즈렌즈 착용이 누액분비에 미치는 영향

Abstract

각막전 눈물층은 지질층, 수성층, 그리고 점액층의 3층으로 구성되어 있으며 각막에 영양 공급과 항균 작용 및 윤활 작용을 하여 각막을 보호하는데 중요한 역할을 하는 층이며 아울러 굴절에도 큰 역할을 하고 있다. 콘택트렌즈를 착용할 때 눈물층이 빨리 배출되어 수성층이 얇아지게 되고 콘택트렌즈가 각막 상피 세포의 기능에 영향을 주어 여러 합병증이 야기될 수 있다. 저자는 이화여자대학교 의과대학 3학년과 4학년 중 안과적 질환을 동반하지 않은 178명중 콘택트렌즈 착용군 70명과 콘택트렌즈를 착용하지 않은 대조군 108명을 대상으로 특수 여과지(Whatmann filter paper No. 41)를 이용하여 누액의 분비량을 검사하는 Schirmer test를 실시하여 콘택트렌즈가 누액 분비에 미치는 영향에 대하여 조사한 결과 다음과 같은 결과를 얻었다. A. 누액 분비의 결과는 콘택트랜즈 착용군(20.5±8.0mm/5min)이 대조군(21.2±8.2mm/5min)에 비하여 다소 감소를 보였으나 통계학적으로 유의한 차이는 없었다(P>0.05). B. 연성 및 경성 콘택트렌즈 착용군의 누액 분비의 결과는 연성 콘택트렌즈 착용군(21.±8.8 mm/5min)이 경성 콘택트렌즈 착용군(15.3±8.7 mm/5min)보다 증가하였으나 두군의 대상자수의 차이가 심하여 두군 사이의 상관 관계를 볼 수 없었다. C. 콘택트렌즈의 일일 착용 시간에 따른 누액 분비와의 상관 관계의 변화는 별로 없었다(P>0.05). D. 콘택트렌즈의 착용 기간에 따른 누액 분비와의 상관 관계 변화는 별로 없었다(P>0.05). 이상의 실험 결과로 보아 콘택트렌즈의 착용은 누액 분비에 별 영향을 미치지 않는 것으로 나타났다.;Tear film consists of lipid layer, aqueous layer, and mucinous layer, and it acts as protection of cornea, nutritional supply, and refraction. The assumption of this study is that the use of contact lens may induce the rapid drainage of tear film, and may make it become thin, and may affect the function of corneal eithelium, and may induce several complications. The author analysed 178 volunteers of medical students. The volunteers consisted of junior and senior grades in Ewha Womans University were classified into two groups. One group was the users of contact lenses(70), and the other group was non-users of contact lenses who have not any other occular problems Control group (108). The purpose of this study was to evaluate the influence of tear secretion in fitting contact lens. The results were as follows ; 1. Mean value and standard deviation of Schirmer test showed ; Contact lens group was 20.5±8.0mm/5min, and control group was 21.2±8.2mm/5mm. There was not statistical significance between two grups. 2. The author could not obtained the statistical difference between the users of soft contact lenses and the users of hard contact lenses, because the number of the users of hard contact lenses was so small in contrast to that of the users of soft contact lenses. 3. There was no significant correlation between daily wearing time and the result of Schirmer test. 4. The correlation between the wearing period of contact lens and the result of Schirmer test showed ; The longer wearing period of contact lens was, the longer wetting length of Schirmer test paper was, but it was not statistically significant. The author concluded that the use of contact lens does not affect the secretion of tear.
