手術後 腸癒着 예방을 위한 Dextran의 실험적 연구

Abstract

수술후 장유착의 발생기전 및 예방법에 관한 연구는 많이 있었으나 아직 확실히 밝혀진 바 없다. 그러나 장유착은 조직의 손상과 아울러 염증 반응을 일으키고 섬유소 물질이 침착하게 되어 일어난다고 한다. 그 예방으로는 수술시 조직에 철저하고 섬세한 조작술 및 지혈이며 여러 약물을 복강내 투여함으로 그 효과를 기대하고 있다. 약물중 Dextran은 장유착 방지를 위한 부체(flotation bath)역할 및 전위(electric potential)를 가지고 있으며 섬유소의 증식을 억제한다고 한다. 이에 본 연구는 Dextran을 복강내 투여한 다음 수술 후 1주 및 2주에 도달하며 대조군과 비교 관찰한 결과 Dextran을 투여한 군에서 유착의 정도가 월등히 적었음을 관찰하였다.;The mechanism of adhesion formation and it's prevention are not clear, but the amount of adherenee on intestinal surface may be diminished in some ways, such as minimal initial injury, prevention of coagulation of exudate, reduction of prolonged contact of surface, removal of fibrin and slowing of fibroblastic proliferation. Dextran has been used to prevent intestinal adhesion by flotation bath and siliconizing effect in various experiments. The degree of adhesion are measured in 30 white rate at postoperative 1 week and 2 weeks in three groups - control, saline and dextran. The following results are obtained; 1. The degree of adhesion are not significantly differed among three groups at postoperative 1 week. 2. The degree of adhesion are significantly decreased in dextran group than control and saline group at postoperative 2 weeks. On conclusion of this study, the application of dextran is very useful for prevention of postoperative adhesion, and it should be opened for furthur study in large experiemental group.
