일부 가족부양 노인과 양노원 의탁 노인의 기능적 손상에 대한 비교연구

Abstract

근래 현저한 사망율의 저하는 상대적으로 노령인구의 비율을 증가시켜 정상적인 노화현상에 의한 이들의 기능저하 혹은 상실을 지연시킴으로써 어떻게 하면 안락한 건강상태를 유지토록 할 수 있을 것인가의 문제가 대두되고 있다. 노년기의 간호는 그 연령에 비춘 정상적인 노화과정의 수준까지 장해된 생체의 구조와 기능을 회복시키는데 있다고 보아 기능손상에 대한 자료를 얻고자 본 연구를 시도하였다. 연구를 위한 자료수집은 1983년9월20일부터 10월10일까지 가족부양노인군 161명과 양노원의탁노인군 174명을 대상으로 Gregg A. Warshaw등이 개발한 Standardized Survey Instrument로 직접 면접 조사하였다. 수집된 자료를 백분율, t -검정, x^(2) -검정등의 통계방법으로 분석한 결과는 다음과 같다. 1 . 양노원의탁노인군이 가족부양노인군보다 정신혼란, 기억장애, 일상 생활활동의 변화등이 크게 나타났으나( p < .05 ) , 청력장애, 시력장애, 언어장애, 기동성의 변화 등은 두 그룹간에 유의한 차이가 없었다. (p>.05) 2 . 연령증가에 따라 정신상태, 청력, 시력, 기억력 등은 유의한 손상을 나타냈으나( p < .05 혹은 P < .01 ) , 언어장애와는 무관했고(p>.05 ) , 기동성 (mobility status)은 가족부양노인군의 경우는 연령증가에 따라 현저한 의존성을 보인( p < .01 ) 반면, 양노원의탁노인군에서는 층계를 오를 때 ( p < .05 )를 제외하고는 비의존적인 경향을 보였다. (p>.05 ) 3 . 일상생활 활동에서도 마찬가지로 가족부양노인군에서는 세수하는 일을 제외하고는 연령증가에 따른 의존적인 변화를 보인(p<.01 ) 반면, 양노원의탁노인군에서는 연령증가와 유의한 차이가 없음을 나타내었다. (p>.05 );Nowadays, because of the strong decreased mortality. Proportions of the geriatric population have increased. Problems emerge from thar how elderly maintain the comfortable health status because the declination of their function can be possible in normal aging phenomena. Geriatric nursing care should make elderly convalesce to level of normal aging process from impaired subject's structure and function according to age. To plan the better care services in the future in geriatric nursing, a survey was purposed to define the extent and severity of functional disability of elderly. Research data was collected from Sep. twentith to the tenth of Oct. 1983, Using the standardized Survey Instrument developed specifically by Gregg A. warshaw. The study of population included 161 family cared and 174 institutionalized older persons aging 60 years or older. The survey teams were led by four researchers with me. For each elderly a research team collected the following functional information : degree of mental confusion, sensory deficits (hearing, vision), speech impairment, and functional status (mobility, daily living activity). The survey included direct interview and assessment of each elderly. The data analized by use of percentage, t-test, Chi-square test. The results of analysis were as follows: 1. The results suggest that institutionalized older persons show more profound signs of mental confusion, impaired memory, impaired activities of daily living than family cared older persons (P〈.05), assessment of hearing, vision, speech, and mobility reveals non significant impairment. (P〈.05). 2. The result of studing age-correlated disability reveals that the impairment in mental status, hearing, vision, memory is significant, (P〈.05, or P〈.01) but the impairment in speech is non. In case of family cared older persons the mobility status relate to the upon the age (P〈.01), but in institutionalized older person, it is independent except climb stairs. (P〉.05). Activities of daily living were assessed in six areas : Feeding, Dressing, Washing, Shaving, Cooking and cleaning. The result of studing age-correlated disability is that, family cared older persons show the strong dependency in five areas except cleansing (P〈.01), but Institutionalized older persons show independent ability in all six areas. (P〉.05).
