일개 여자대학교 학생결핵관리 실태에 관한 조사

Abstract

In order to know the status of prevalence and care about pulmonary tuberculosis student, this study was done by analyzing tuberculosis student's records registered in Ewha Women's University Health Center during 1982-1984. And the results were as follows ; 1. Annual prevalence rate of student pulmonary tuberculosis was 8.6 per 1,000 In 1982, 6.3 in 1983 and 5.6 in 1984. 2. The annual prevalence rate according to grade; freshmen was 15.2 per 1,000 in 1982, 14.2 in 1983 and 11.6 in 1984, sophomer was 5.2, 1.6 and 4.4, junior was 4.6, 6.2 and 3.3.senior was 5.6, 1.8 and 2.7. It was seen the decreasing tendency of the annual prevalence rate in each grade. 3. During 1982-1984, the percentage distribution of cases by registered source was 44.5% in a entrance physical examination, 47.5% in a annual physical examination and 5.2% in the medical service, 1.0% from in the hospital information and 1.9% by other sources. 4. The proportion of minimal and moderate advanced pulmonary tuberculosis student was 90.0% and 10.0 % in 1982, 95.9% and 4.1% in 1983, 89.8% and 10,2% in 1984. 5. The Patient distribution of "those using drugs was 73.2% in MZ group (Ethanlbutol + INH) and 16.8% in R group (Rifampin + Ethambutol + INH) during 1982-1984. 6. The average medicated durations of the minimal pulmonary tuberculosis group were 11.5 ± 4.5 (Month) in 1983 and 10.7 ± 2.4 (month) in 1984. And those of the moderate advanced pulmonary tuberculosis group were 18.0 ± 9.2 (month) in 1983 and 16.0 ± 4.2 (month) in 1984. 7. During 1982-1984, the average medicated durations of the groups by registered source were 11.7 ± 5.8 (month) in the group by enterance physical examination and 9.9 ± 3.4(mouth) in the group by annual physical examination. In the average medicated duration, the group by enterance physical examination was longer than the group by annual physical examination.;본 조사는 대학 결핵관리를 위한 자료를 제공할 목적으로, 이화여자대학교 대학보건소에 1982년부터 1984년까지의 기간중에 등록관리받은 결핵학생 310명의 결핵관리기록을 근거로 하여 본 조사 목적으로 제작한 조사카드를 이용하여 결핵유병상과 관리실태에 관한 자료를 수집 · 분석하여 다음과 같은 결과를 얻었다. 1. 년도별 학생 결핵유병율은 1,000명당 1982년 8.6, 1983년 6.3 및 1984년 5.6으로 전체학생유병율은 년차적으로 감소경향을 나타내고 있었다. 2. 학년별 년도별 유병율은 학생 1,000명당 I학년은 1982년 15.2, 1983딘 14.1, 1984년 11.6이었고, 2학년은 각각 5.2, 1.6, 4.4이었으며, 3학년은 각각 4.6, 6.2, 3.3이었고, 4학년 은 각각 5.6, 1.8, 2.7로,매년도 공히 1학년 학생 유병율이 타학년에 비하여 3배가까이 높았고, 각학년에 있어서는 년차적으로 유병율은 감소경향을 나타내고 있었다. 3. 1982-1984년 기간중, 결핵발견계기별 환자분포는 입학신검에서 44.5%, 재학생 정기심경에서, 47.5% 진료실 진찰에서 5.2%, 타의료시설에서 발견통보 등이 2.9%였다. 4. 결핵발견시의 증후도별 환자비율은 1982년에는 경증 90.0% 중등증 10%이었고. 1983년에는 경증 95.9 %, 중등증 4.1% 이었으며, 1984년에는 경증 89.8%, 중등증 10.2%로, 경중대 중등비율은 9 : 1 범위였다. 5. 1982-1984년 기간중, 결핵학생에 대한 치료방법별 비율은, MZ그룹(Ethambutol + INH 복합치료자)이 73.2%, R그룹(Rifampin + Ethambutol + INH 복합치료자)이 26.8%로 구성비는 3 : 1 범위였다. 6. 경증환자의 평균투약치료기 간은 10.7±4.81(개월)이었고, 중등환자의 평균투약치료기간은 16.5±7.45(개월)이었다. 7. 발견시 증후정도에 따른 환자 평균투약치료기간은, 경증환자에 있어서, 입학신검에서 발견된 환자는 11.7±5.8(개월)이었고, 재학생 신검에서 발견된 환자는 9.9±3.4이었다. 한편, 중등증 환자에 있어서는 입학신검발견환자는 17.3±7.1 (개월)이었고, 재학생신검 발견환자는 14.3±6.3 (개월)이었다. 8. 경증환자에 있어서, 학생의 치료성실도에 따른 경종환자 평균치유기간은 충실도 95% 이상인 A군은 10.7±5.1 (개월)이었고, 충실도 90~95%인 B군은 13.3±0.0 (개월)이었다.
