한국 정신의료사회사업가의 전문적 機能 및 業務에 관한 硏究

Abstract

社會가 고도로 발달하여 전문직의 존립이 불가피해졌고, 이에 부응하여 정신의료사회사업 (Psychiatric Social Work)도 환자를 全人的으로 이해하고 치료도 全人的인 接近을 시도하게 됨으로써 환자의 사회환경적인 要因과 정서적인 측면에의 전문적인 써어비스로 인정받게 되었다. 따라서 정신의료영역에서 사회사업가들은 타 전문직과 대등한 입장에서 活動해 오고 있다. 고도의 전문적인 업무분화가 현대사회의 전문직들간에 요청되고 있으며, 이 문제로 각 전문직들간의 갈등이 야기되어지고 있다. Secondary Setting에서 사회사업을 目的으로 活動하게 되는 정신의료사회사업 역시 이와같은 갈등상황을 갖고 있다고 믿는 본인은 한국에서의 정신 의료사회사업이 환자의 전과정을 통해서 크게 필요로 되고있는 전문직이라고 생각했으나, 실제로 근무하고 있는 정신의료 영역의 사회사업가들은 一般 사회사업직에 비해서 사회사업 이외에도 정신의료영역의 더 많은 전문적 지식과 訓練이 요구되고 있는 반면, 전문적 주체의식은 더 낮다고 생각되었다. 이와같은 생각은 6년반동안 Primary Setting에서의 실무경험과 일년간의 정신과 병동에서의 임상경험을 통해서 비롯된 것이다. 따라서 전문적 주체성에 변수로 작용하고 있는 구체적인 면들을 발견하고 살펴보기 위한 수단으로 한국에서의 정신의료사회사업에 관한 인식과 사회사업가의 전문업무, 역할 및 자격에 관한 조사를 현재 한국에서 정신의료사회사업가로 근무중이며 전국 의료사회사업가협회에 가입된 총22명과, 現在 그들과 같이 근무중인 정신과의사 총 120명중 44명을 선정하여 실시했다. 본 조사에서 발견된 점들을 요약하면 다음과 같다. 첫째, 정신의료 영역에서 근무중인 사회사업가는 一般 사회사업직에 비해 女性이 많고, 연령도 전체적으로 낮아서 20代가 73%를 차지하고 있다. 둘째, 한국에서의 정신의료사회사업가는 정신과가 있는 전체 setting에 비해 볼때, 활동범위나 그 숫자 모두가 전체 정신의료영역의 15% 이하에 불과하였으며, 지역분포도 수도권 지역에 거의 한정되어 있었다. 셋째, 정신의료사회사업이 정신과 영역에서 필요한가에 관한 질문에 사회사업가와 정신과의사 두집단은 모두가 그 필요성을 인정하고 있었으며, 정신과의사의 경우 정신의료사회사업에 관하여 많이 알고 있을수록 사회사업가의 必要性을 더 많이 느끼고 있었다. 넷째, 정신의료사업에 있어 환자의 전과정에 걸친 사회사업가의 機能은 두집단이 모두 인정하고 있었으나 구체적인 業務遂行에 있어서는 각각 상이한 견해로 맞서고 있음을 볼수 있었는데 그것은 크게 a. Intake 및 진단과정 b. 문제해결 및 치료과정 c. 사후관리 및 follow-up의 세과정으로 나누어 살펴 볼때, 두 번째 단계인 문제해결 및 치료과정에서의 업무에 있어서 두집단은 크게 견해를 달리하고 있었다. 구체적으로 한국의 정신과의사들은 사회사업가의 업무중 문제해결 및 치료 과정에서의 전문적인 써어비스를 인정하지 않으려는 경향을 보이고 있다는 것이다. 다섯째, 사회사업가들의 업무수행능력은 연령이 높을수록 업무수행에 곤란을 덜 느끼고 있는 것으로 나타났다. 따라서 사회사업가의 연령이 업무수행에 있어서 하나의 중요한 변수로 作用하고 있음을 볼 수 있었다. 여섯째, 사회사업가의 役割에 있어서도 두集團은 상이한 견해를 가지고 있었는데 90%이상의 정신과의사는 사회사업가를 치료자론 인정하지 않고 있었다는 것이다. 이것은 치료(Therapy)라는 개념에 있어서 각 전문직이 갖는 해석 및 범위에 있어서 정도의 차이가 있다고 인정하더라도 위의 결과는 상당한 의미를 갖는다고 해석할 수 있다. 일곱째, 정신의료영역에서의 사회사업가의 資格에 있어서는 두 集團이 모두 이 분야에서의 전문적인 訓練을 요구하고 있었으나 실제 사회사업가들의 水準은 모두 미달되고 있다고 보겠다. 이상에서 밝혀진 상황과 문제점들을 中心으로한 해결책을 提示해 보면, 첫째, 專門的 주체성의 확립을 위하여 먼저 見解를 달리하는 정신과의사와의 업무분배에서 생겨나는 葛藤을 解決하기 위한 努力이 必要하다고 본다. 이 問題의 해결을 위해서 사회사업가는 자신에 대한 지각 내지는 평가를 정확히하여 그 評價를 토대로 한 자신의 전문적 업무로 接近해야 할것이며, 둘째, 지속적인 사회사업가의 능력향상을 위하여 계속교육(Continuing Education)이 학교와 현장에서 강력하게 進行되어야한다고 생각한다. 셋째, Secondary Setting에서 계속적으로 야기되는 타 전문직의 기대상황과 사회사업가 자신의 전문적 주체성 確立에 있어서 적절히 대처하기 위해서 사회사업가들은 같은 영역에서의 사회사업가들과 정규적인 회합을 통한 단체적인 힘을 통하여 해결해 나가야 할 것으로 믿는다. 넷째, 이후의 한국 정신의료사회사업의 發展을 위하여 전문요원의 배출이 무엇보다 시급한 課題로 생각되는데 이 問題의 해결을 위해서는 사회사업과가 있는 대학들은 대학별로 그들의 전문방향을 모색해야 할 것이며, 전문기관이 설립되어야 하며, 자격증제도를 만드는 등의 具體的인 方法이 요청되어 진다. 물론 뒤의 두가지 方法은 정부의 개입으로 제도적인 뒷받침이 가능하면 훨씬 빠른 速度로 해결되어 나갈수 있을 것이라고 믿는다. 다섯째, 사회사업계는 他 專門職과 一般大衆에 대한 정신의료사회사업의 소개와 弘報를 위하여 努力해야 할 줄로 믿는다.;As society advanced, many kinds of complicated problems developed, necessitating the profession. And in the devision of services between the professions, conflict occurved when boundaries of the devision were trespassed upon. Though in the past social workers had worked in the psychiatric settings successfully, they began to face conflict stemming from this division of services. Through this study, it was my intention to discover the variables influnce and reflect in the professional identity of psychiatric social workers in Korea and to define the conflict that exists between psychiatrists and social workers. For these purposes, the author relied on questionaires which distributed in the practical field. The questionaires were distributed among 2 proups; First, among psychiatrists who are working with psychiatric social workers in hospital and/or clinic setting. Second, to psychiatric social workers who is a member of the Association of Korean social workers in the medical field The survey revealed these finding. 1. Regarding the necessity of the psychiatric social worker's intervention in psychiatric setting, both of the 2 groups recognized the need of psychiatric social work services and psychiatrists who were more knowledgeable on psychiatric social work answered high in the item which psychiatic social work service is really necessary in psychiatric setting. 2. In the exact function of the social worker in the psychiatric setting, psychiatrists had a different opinion regarding the definite division of services with social worker, as detailed below, The author devided the process of social work function for psychiatric pt. a. The process of Intake and Diagnosis. b. The process of Problem solving and Treatment c. The process of Aftercare and Follow-up. More than 50% of psychiatrists did not want the intervention of social worker in the second process. They insisted that the process is their function in all cases, and considered that social workers should extend their services to the first and third services to the first and third processes. 3.Younger social workers indicated that they experienced considerable conflict with other professions more so than did the older social workers. 4. Regarding the role go the psychiatric social worker, the 2 groups also had a different opinion. All of psychiatrist except 2 persons answered that they did not believe that psychiatric social workers were capable of becoming therapists. Even though the 2 groups ma posses a different idea regarding the concept of therapy, this is a significant issue. 5. Both of the 2 groups responded game on qualification of psychiatric social worker in the item which the practical experiences in psychiatric setting for 2 o more years is very important. And, the group of psychiatrist did not show any significant difference on academic background of social worker. That menas in Korea, psychiatrist does not consider on whether psychiatric social worker has B.A Degree or M.A. Degree and but, 73% of social worker answered M.A degree is necessary for the job of psychiatric social worker. After having given thought to those problems mentioned above, the author would now like to present some suggestions that might lead to their solutions. 1. All of social worker should approach for their professional job and service, having correct perception and evaluation on their real ability. 2. Continuing education in school and practical field is necessary to develop capacity of social workers. 3. Regular meeting and asscoiation of psychiatric social worker is recommendable to solve the stress and conflict in their practical field, to consult ct's problem, etc. Regular meeting or association will be much helpful in solving serveral kinds of problems which occurs in the field, developing the society of psychiatric social work. 4. Producing well-trained person is most important to develop psychiatric social work in korea. For the above, first, all of university which has department of social work should decides their own direction of professionally oriented on social work which is based on reality. Second, the foundation of professional institute is recommendable. Third, institutional support for school, institute and certificate of qualification from government will be powerful to develop psychiatric social work. 5. The society of social work should invest for introdusing about the importance of psychiatric social work service in psychiatric setting. That is the way to extend the field of psychiatric social work in korea.
