환자의 간호요구에 대한 환자와 간호원의 지각일치에 관한 연구

Abstract

The congruence between the patients need and nurse's perception of their needs is an important basis for the individualized nursing care. This study was attempted to find out the basic data for the individualized nursing care and to measure the amount of congruence between the nurses and patients about the patients physical and emotional need during the hospital experiences. By using Questionaire, developed by Y.M. Williamson, the informations about the patient's physical and emotional needs were surveyed. This study was carried out from Sept. 21st to Oct. 21st, 1983 at the E . Hospital. A total of 67 patients and 30 nurses, who wrote the nursing history at the time of patient's admission and discharge, were selected for the study. The results were analized by computer using S.P.S.S. program and summerized as follows; 1. At the time of admission, there were no significant diferences between perceiving patient's emotional need and physical need (p>.05) 2. At the time of admission, the level of importance of emotional needs were higher than physical need as perceived by the nurses. (p= .002) 3. At the time o f discharge, the level of importance of emotional needs were higher than physical need as perceived by the patients. (p=. 046) 4. At the time of discharge, the level of importance of emotional needs were higher than physical need as peceived by the nurses. (p=. 026) 5. At the time oa admission and discharge, patients and nurses had no change in the relative of perceiving the patients need. (P>.05) 6 . There were direct relationship between the patients perceptions of how well their needs were met (65.4%) and how satisfied they were with the nursing care they received. (R=.8O9 P=. 000) 7. There were direct relationship between the nurses perceptions of how well patient's need were met (58.5%) and how satisfied in giving nursing care to the patients . (R=. 765 P=. 000) 8. (p>.05);인간은 내·외적인 환경변화에 따라 요구의 양상과 요구에 대한 지각의 변화를 가져온다. 환자는 입원을 하고있는 동안에도 요구의 양상이 끊임없이 변화하고 그 요구를 해결하기 위해 신체적 정서적으로 간호원의 도움을 요구하게 된다. 간호원은 환자의 간호요구를 해결해 주기 위해 환자와 동일하게 환자의 요구를 지각하는 것이 중요하다. 본 연구는 간호과정을 통해 환자와 간호원이 환자의 요구에 어느 정도 일치된 간호요구를 지각하는가를 확인하여 양질의 개별화된 간호를 제공하기 위한 하나의 기초자료를 얻고자 시도하였다. 자료수집은 1983년 9월 21일부터 10월 20일까지 E대부속병원에 입원한 환자 67명과 그 환자의 간호력을 작성한 30명의 간호원으로부터 수집하였다. 연구도구는 Yvonne M. Williamson이 개발한 도구로서 환자의 신체적 정서적 요구의 지각에 대한 정보를 얻도록 고안된 설문지를 사용하였다. 수집된 자료는 S.P.S.S에 의하여 백분률, t검정, F검정등의 방법으로 분석하였고 그 결과는 다음과 같다. 1. 환자 입원시, 환자는 신체적 요구와 정서적 요구에 대한 지각간에서 유의한 차이가 없었다( P=0.582 ). 2. 환자 입원시, 간호원은 환자의 신체적 요구와 정서적 요구에 대한 지각간에서 정서적 요구의 중요도가 높았다( P=0.002 ). 3. 환자 퇴원시, 환자는 신체적 요구와 정서적 요구에 대한 지각간에서 정서적 요구의 중요도가 높았다( P=0.046 ). 4. 환자 퇴원시, 간호원은 신체적 요구와 정서적 요구에 대한 지각간에서 정서적 요구의 중요도가 높았다( P=0.026 ). 5. 입원기간동안 환자와 간호원은 환자의 간호요구에 대한 지각의 변화에 유의한 차이가 없었다(P>0.05 ). 6. 환자의 간호요구에 대한 충족된 정도에 따라 환자는 65.4%의 만족정도를 보였고, 충족정도가 높을수록 만족정도가 높은 것으로 나타났다( R=0.809, P=0.000 ). 7. 환자의 간호요구에 대한 충족된 정도에 따라 간호원은 58.5%의 만족정도를 보였고 충족정도가 높을수록 만족 정도가 높은 것으로 나타났다( R=0.765, P=0.000 ). 8. 환자와 간호원은 일반적 특성과 만족도간에서 유의한 차이가 없었다( P>0.05 ). 결론적으로 환자와 간호원은 환자의 신체적 요구보다 정서적 요구에 높은 중요도를 두는데 일치감을 보였다. 따라서 본 연구의 결과는 간호원이 환자의 간호요구를 지각하는데 활용할 수 있으리라 보여, 좀더 다양한 도구의 개발이 필요하다고 본다.
