정신질환자와 비정신질환자의 사회적 지지와 스트레스에 대한 비교 연구

Abstract

과학의 발전으로 질병은 여러가지 요인들의 동시 상호작용으로 발생한다는 것이 분명해 졌다. 그리하여 질병의 원인으로 인간의 내적, 외적환경의 관계를 검토하게 되었으며, 이러한 경향에 따라 인간의 사회 환경적 요인으로서 의미있는 타인과의 상호 작용인 사회적 지지가 건강관리 사업에서 근래에 이르러 많은 관심을 받게 되었다. 그래서 그 역할에 있어서도 다양하게 기술 되었는데 특히 사회적 지지가 스트레스 사건시 대단히 효과적이라는 주장이 나오게 되었다. 그러므로 본 연구는 질병예방과 건강유지 증진의 개인적이며 환경적 요인으로 대두되고 있는 사회적 지지와 이와 관련되는 요인으로서 건강을 위협하는 스트레스를 평가하기 위하여 정신질환자와 비정신질환자를 그 대상으로 하여 사회적 지지와 스트레스를 비교분석하고 이 두 요인의 상관관계를 검토하였다. 본 연구의 자료수집기간은 1983년 3월 21일부터 4월 16일까지 였으며, 서울에 위치한 2개 병원 정신과에 입원한 환자와 외래환자 63명을 정신질환자로, 서울시내 4개동에 거주하는 정신과적인 치료의 병력이 없고 현재 신체질환도 않고 있지 않은 주민 62명을 비정신질환자로 하고 Norbeck 등이 고안한 N.S.S.Q(Norbeck Social Support Questionnaire )와 Homes 와 Rahe의 S.R.R.S (Social Readjustment Rating Scale)를 번역 보완한 질문지를 사용하여 얻은 자료를 분석한 결과는 다음과 같다. 1. 정신질환자는 비정신질환자 보다 사회적 지지 기능도, 의미있는 사람수, 그들과의 접촉빈도, 교제기간등 모든 사회적 지지항목에서 유의하게 낮음을 나타내었다. (P< .00 혹은 0.01<P<0.05) 2. 정신질환자와 비정신질환자가 최근 6개월 동안 경험한 스트레스 사건수는 유의한 차이가 없었다. (P>.05) 3. 정신질환자는 비정신질환자 보다 스트레스 사건에 대한 심각도가 높게 나타났으며 유의한 차이를 보여 주었다. (P<.05) 4. 정신질환자에서는 사회적 지지의 모든 항목과 스트레스 사건수 및 심각도 간에는 유의한 상관관계가 없었다. (r = 0.06∼0.15, P>.05) 그러나 비정신질환자에서는 사회적 지지의 모든 항목과 스트레스 사건수 및 심각도간에는 모든 항목에서 유의한 양의 상관관계를 보여주었다. (r = 0 .34 ∼ 0 .22 , P < .00 혹은 0.01<P<0.05);This study on social support and stress of psychiatric patients and non-psychiatric peoples was applied to people who was divided into two groups from March 21 to April 16, 1983. One is in-patients and out-patients of two mental hosoitals in Seoul. The other is inhabitants in four Dohgs of Seoul. This study was purposed to identify the difference of the total function of social support, network properties and the difference of stressful life events' numbers, the severity of stress between psychiatric patients and non-psychiatric peoples. And also to investigate the correlation between social support and stress in two groups mentioned above. For the measurement of the social support, the research used N.S.S.Q.(Norbeck Social Support Questionnaire)made by Norbeck J.S., A.M. Lindsey and V.C. Carrieri. For the measurement of the stress, the researcher used Homes & Rahe's S. R .R .S (Social Readjustment Rating Scale). The analysis of data was done by use of percentage, t-test, F-test, Chi-square test and Pearson Correlation Coefficient. The results of analysis were as follows; 1. Psychiatric patients had lower total function of social support and network properties than non-psychiatric peoples. (p<.00 or 0.01<p<0.05). 2. There was no significant difference in stressful life events' numbers between psychiatric patients and non-psychiatric peoples. (p>.05). 3. Psychiatric patients had higher severity of stress than non-psychiatric peoples. (p<.00). 4. There was no significant correlation between social support and stress in psychiatric patients. (r=0.06~0.15, p>.05) But, there was a positive correlation between social support and stress in non-psychiatric peoples. (r=0.34-0.22,p<0.00 or 0.01<p<0.05)
