정상 월경주기중 혈청 Testosterone 및 lH 농도의 변화에 관한 연구

Abstract

혈청 남성홀몬(testosterone)농도의 측정은 방사면역층정방법의 발달로 불임증 특히 다모증(hirsutism) 여성의 진단 및 치료에 있어서 androgen과다의 가능성을 평가할 수 있는 가장 유용한 방법이다. 정상 월경주기중 난소는 androgen을 분비하는 것으로 알고 있었지만 정상 배란성 여성의 혈청 testosterone농도의 범위와 일별(daily) 불규칙적인 파동성 분비의 변화에 대한 연구는 아직도 많이 알려지지 않았다. 그러나 일부 연구자들은 혈청 testosterone의 농도가 월경주기의 난포기 초기나 황체기 후기보다도 난포기 후기 및 배란기에 더 많이 증가한다고 보고하였다. 이에 저자는 방사면역측정방법으로 정상 월경주기중 특히 배란기중에 혈청 testosterone농도가 증가하는지 또는 뇌하수체의 성선자극홀몬인 LH(Luteinizing hormone)와 어떻게 관여하는지에 대하여 관찰한 결과 다음과 같은 결론을 얻었다. 1. 전 월경주기중 혈청 testosterone농도는 평균 0.43±0.12(S.D.) ng/㎖로 그 범위는 0.21±0.09∼0.92±0.24 ng/ml 였다. 2. 혈청 testosterone농도의 난포기와 황체기의 평균치는 각각 0.45±0.09 ng/ml와 0.37±0.12 ng/ml로 난포기와 황체기보다 더 높은 경향을 보였다. 3. 배란기때 testosterone농도는 평균 0.49±0.14 ng/ml로 난포기와 황체기에 비하여 통계학적으로 유의한 차이는 없었으나 비교적 높은 경향을 보였다. 4. 혈청 testosterone농도의 분비상태는 불규칙적인 파동성 분비를 나타냈으며 월경의 전주기를 통하여 혈청 testosterone농도의 주기적 변화를 관찰할 수 없었다. 5. 혈청 testosterone농도의 분비상태는 혈청 LH최고치에서 보는 것과 같이 급격히 상승하는 현상은 볼 수 없었다.;The purpose of this paper was to establish the level of serum testosterone and luteinizing hormone (LH) concentrations and these values were helpful to the study of the infertility, especially hirsutism. Utilizing specific radioimmunoassays, serum testosterone and LH were measured daily in the same alliquots of sera in total of 126 samples among 7 cases with normal ovulatory cycle. The results were as follows: The mean serum LH concentration was 115.9 ± 18.26mIU/㎖ at the midcycle. Serum testosterone concentrations averaged 0.43 ± 0.12ng/㎖ and ranged from 0.21 ± 0.09 to 0.92 ± 0.24ng/㎖. The mean serum testosterone concentrations in 7 women were higher around the midcycle LH peak and higher during the follicular than during the luteal phase of the cycle. Averages of daily serum testosterone concentrations for each individual women for the entire cycle, varied among the 7 subjects studied, ranging from 0.28 to 0.69ng/㎖. Coefficients of variation of serum testosterone levels in 7 individual cycles averaged 28.9%. These findings suggested that the mean serum testosterone concentrations in normal women were subjected to small but showed random variation during the course of the menstrual cycle.
