신생아 중증황달에 대한 교환수혈의 임상통계학적 관찰

Abstract

최근 중증황달환자의 발병율과 이를 치료하기위한 교환수혈의 빈도가 감소하는 경향이다. 이대부속병원 소아과학 교실에서 1968년부터 1976년까지 중증황달이 있는 신생아에게 실시하였던 교환수혈의 결과에 대하여는 이미 보고된 바 있다. 저자는 1977년 1월부터 1981년 12월까지 실시한 신생아황달에 대한 교환수혈의 결과를 분석하여 보고하는 바이다. 1. 이번 실시한 저자의 통계적 관찰에 의하면 교환수혈실시율이 0.24 %이었다. 즉 교환수혈율이 감소하는 경향이었으나 만삭아의 경우는 증가하는 경향이었다. 2. 중증황달의 원인중 ABO 부적합에 기인한 황달의 빈도가 가장 많았다. 3. 교환수혈의 성공율은 98.6% (141/143)였다. 4. 교환수혈로 인한 사망율은 1.1%(1/92)로 유의성있는 감소를 보였다.;It has been noted, over the Last decades that the incidence of severe neonatal hyperbilirubinemia as well as the exchange transfusion has been decreasing due to multiple factors. I reviewed medical records of cases of hyperbilirubinemia who had exchange transfusion from Jan. 1977 to Dec. 1981, on the annual incidence, etiology, outcome, and mortality. 1. Incidence of exchange transfusion in infants who were born in Ewha hospital is 0.24% of livebirths. The annual incidence of exchange transfusion is decreasing gradually since 1977. Among total patients who had exchange transfusion, 74% was born outside, and 26% was born in Ewha Hospital. 2. ABO incompatibility was the most common etiology of neonatal hyperbilirubinemia and unknown origins sepsis, and prematurity followed in order of frequency. 3. The rate of technical failure to complete transfusion was 1.1%(1/9). 4. There was no case of immediate complication of exchange transfusion and the mortality rate was 1.1 (1/92).
