정상 월경주기에 있어서 혈청 성선자극홀몬(FSH. LH) 및 성스테로이드 홀몬(Estradiol, Progesterone) 의 분비에 관한 연구

Abstract

Patterns of the serum gonadotropin (FSH, LH) secretion from pituitary gland and steroid hormones (estrogen, progesterone) secretion from ovary have been studied with numerous publications since radioimmunoassay developed by fallow et al in 1959. The serum levels of FSH, LH, estradiol, and progesterone were measured daily by radioimmunoassay in ten women with ovulatory menstrual cycles. The results were as follows: 1. Menstrual cycles of ten subjects studied were considered to be normal. 2. During the follicular phase of the cycle, the secretion of FSH showed an initial rise followed by a progessive fall toward to the midcycle LH surge with a mean peak value of 24.56mIU/m1. Thereafter, the secretion of FSH was sharply decreased. 3. LH secretion showed a gradual rise before the midcycle or LH peak. During the late follicular phase, the LH secretion was rapidly increased and mean peak level was rapidly increased and mean peak level was 153.26mIU/m1. Following the midcycle LH and FSH surge, LH level was sharply decreased as well as FSH level. Therefore, LH and FSH were remained at lower level during the luteal phase rather than during the follicular phase. 4. Plasma estradiol was gradually increased during the follicular phase and it was reached the maximal peak level (218.49pg1m1) prior to midcycle LH surge. Following the midcycle peak levels of estradiol were sharply dropped and then rised again from 3rd day of cycle showing mean 96.22 pg/ml during the midluteal peak. 5. Plasma progesterone levels were increased to the maximal concentration (mean 16.22 ng/ml) after LH surge occured and it remained elevated from day 5 to day 10 following LH peak occured. Thereafter, the progesterone levels were decreased rapidly up to the onset of next menses.;최근에 방사면역측정법이 개발됨에 따라 이를 이용하여 월경주기중 뇌하수체에서 분비되는 성선자극홀몬(FSH, LH)과 난소에서 분비되는 성스테로이드홀몬(estrogen, progesterone)의 혈중내의 측정이 가능하여짐에 따라, 각 홀몬의 주기적 변화양상이 파악됨은 물론 시상하부-뇌하수체-난소축에 있어서의 음성 및 양성 되먹이기 기전의 동적해석이 가능하게 되었다. 그러나 우리나라에서 방사면역측정법을 이용하여 한국여성의 정상월경주기중 성선자극홀몬 및 성스테로이드홀몬의 변화에 관한 보고가 아직 적으므로 저자는 기초체온측정에 의하여 정상배란성 월경주기를 가진 것으로 확인된 10예의 여성을 대상으로 월경주기를 난포기, 배란기, 황체기의 3기로 나누어 각 주기에 따른 성선자극홀몬과 성스테로이의 혈청치의 변화를 측정하여 다음과 같은 결론을 얻었다. 1. 대상자 10예의 월경주기는 모두 정상 배란성으로 판정되었다. 2. FSH는 초기 난포기에 비교적 높은치를 유지하였으며(9.47±0.23 mIU/ml), LH surge와 동시에 최고치 24.56±3.56 nIU/m1에 달한 후 황체기에는 다시 감소하여 6.11±0.68 mIU/m1 이었다. 3. LH는 난포기에 큰 변화없이 평균치 19.67±1.47 mIU/ml이었고, -2일부터 급격히 상승하여 0일에 최고치 153.26±27.58 mIU/ml 에 이르고 급격히 하강하여 황체기에는 12.92±1.26 mIU/ml에 이르렀다. 4. Estradiol은 난포기에 평균치 62.71±13.51 pg/ml로 낮은치를 유지하였다가, LH surge가 일어나기 3일전부터 급격히 상승하여 -1일에 평균치 218.49±53.99 pg/ml로 최고에 달하였으며 그 후 감소하였다가 +3일이후부터 다시 상승하여 +9일까지 증가상태를 유지하였으며 평균농도는 96.22±5.75 pg/ml이었다. 5. Progesterone은 난포기동안 0.32±0.08 ng/ml로 미량 존재하였으나 LH surge이후 상승하기 시작하여 +10 일까지 계속 유지하여 평균치 16.22±1.15 ng/ml이었다. 6. 본 연구에서 정상 월경주기동안 난포기 후기때의 estradiol의 증가가 LH-FSH surge을 유발하였고 estradiol 및 progesterone의 적절한 분비가 적합한 황체기를 유지 한다는 것을 알게 되었다.
