사시와 굴절이상과의 상관성

Abstract

눈은 예민하고 섬세한 지각적인 작용과 운동적인 작용에 의하여 올바른 위치를 유지하고 있으며 이들의 작용을 방해하는 요인에 의하여 사시가 발생하게 된다. 이 요인에는 기능적 요인, 신경적 요인, 조절성 요인, 및 해부학적 요인이 있으며 이들은 단독적으로 또는 복합적으로 작용하여 사시를 일으킨다. 저자는 사시환자 125명을 대상으로 초진시의 연령, 굴절상태, 사시의 종류 및 정도를 관찰하였으며 사시의 여러 원인 증에서 특히 굴절이상, 즉 조절성 요인과 사시 발생과의 상호작용 내지 상관 관계를 규명코자 본 연구를 실시하였으며 그 결과는 다음과 같다. 1. 공동성 사시는 120예였고 비공동성 사시는 5예였다. 2. 공동성 사시에서 내사시 60예(50%), 외사시 60예(50%) 이었다. 3. 내사시에서 조절성 내사시는 25예, 비조절성 내사시는 35예였으며 외사시에서 조절성 외사시는 26예였고 비조절성 외사시는 34예로서 내사시와 외사시 모두 비조절성 사시가 많았다. 4. 내사시, 외사시. 모두 교대성 사시가 많았다. 5. 내사시의 경우는 여자에 있어서 많았으며 외사시의 경우는 남녀사이에 큰 차이가 없었다. 6. 초진시의 교정시책은 0.7이상이 71.3%로서 양호한 시력을 보였다. 7. 사시의 정도는 20∼40△ 사이에서 가장 많았다. 8. 조절성 내사시의 경우는 +1.0D.∼+2.5D.사이의 굴절이상이 많았으며 외사시의 경우에는 - 1. 0D.~-3.0D.사이에서 많았으며 내사시와 원시, 외사시와 근시사이에서 상관성이 있음을 볼 수 있다. 9. 사시의 정도와 굴절이상의 정도사이에는 상관성이 없었다.;The author analyzed I25 strabismic patients which were treated both at Ewha Womans University Hospital and Kim's eye hospital by varieties and types of strabismus, sex, age, visual acuity, angle of deviation, refractive state of first visit. The obtained results summarized as follows : 1. Among I25 strabismus, 120 were comitant and 5 were noncomitant. 2. Among 120 comitant strabismus, esotropia were 60 cases (50%), and exotropia were also 60 cases(50%). 3. Among 60 of esotropia, 25 were accomodative type and 35 were nonaccomodative type. Among 60 of exotropia, 26 were accomodative type and 34 were nonaccomodative type. 4. Alternating type of comitant strabismus was more prevalent in both esotropia and exotropia. 5. More higher incidence in female than in male was found in comitant esotropia. 6. 0.7 or better visual acuity was 71.3% in comint strabismus. 7. Regarding to the refractive states and strabismus, more esotropia in hyperopia, and more exotropia in myopia was found. 8. No consistency was found between degrees at strabismus and refractive errors.
