Abstract
본 연구는 분만과정시 남편의 참여가 초산부의 상태불안에 미치는 영향을 파악하여 남편의 분만과정에의 참여를 권장하며 나아가서 부부를 위한 산전교육 프로그램을 실시하는데 도움을 주고자 시도하였다. 연구목적은 분만을 위해 입원한 초산부와 그 남편에게 분만의 정상경과에 관한 교육을 실시하고 남편이 분만과정에 참여한 초산부(실험군)와 교육을 받지 않았으나 남편이 분만과정에 참여한 초산부 (대조군Ⅰ) 그리고 교육도 받지 않고 남편이 참여하지도 않은 초산부 (대조군Ⅱ)사이에 상태-특성불안반응에 유의한 차이가 있는지 알아 보기 위함이다. 연구방법은 1983년 9월 1일부터 10윌 10일사이에 이화여자대학교 부속병원 분만실에 입원한 초산부 90명을 대상으로 실험군, 대조군Ⅰ, 대조군Ⅱ, 각각 30명씩으로 나누어 설문지를 통해 자료를 수집하였다. 연구도구는 spielberger의 상태-특성불안측정도구로서 40문항으로 구성된 설문지를 사용하였으며, 초산부와 남편에게 실시한 분만의 정상경과에 관한 교육은 연구자가 교안을 작성하여 분만을 위해 입원한 때에 1회 20분간의 개별교육을 실시하였다. 수집된 자료는 백분율, t검정, ANOVA 등의 통계방법으로 분석하였으며 그 결론은 다음과 같다. 1. 분만의 정상경과에 관한 교육을 남편과 함께 받고 분만과정동안 남편이 참여한 실험군과 교육을 받지 않고 남편이 분만과정에 참여한 대조군Ⅰ과의 상태불안에는 유의한 차이가 없었다(P = 0.07) 2. 분만의 정상경과에 관한 교육을 남편과 함께 받고 분만과정동안 남편이 참여한 실험군과 교육도 받지 않고 남편이 분만과정동안 참여하지도 않은 대조군Ⅱ와의 상태불안반응에서는 실험군이 대조군Ⅱ보다 불안반응이 낮게 나타났다(P = 0.004). 3. 교육을 받지 않았으나 남편이 참여한 대조군Ⅰ과 남편이 참여하지도 않은 대조군Ⅱ간의 상태불안에는 유의한 차이가 없었다(P = 0.364). 4. 실험군, 대조군Ⅰ, 대조군Ⅱ간의 상태불안반응의 비교에서는 실험군이 대조군Ⅰ, Ⅱ보다. 상태불안이 낮았다(P = 0.022). 5. 연구대상 전체 초산부의 연령, 학력, 종교, 직업, 임신주수, 산전진찰, 결혼생활만족도, 임신수용여부, 분만제 1 기 소요시간, 자궁수축제, 진통제 투여유무등에 따른 상태불안반응에서는 결혼생활이 만족하다는 초산부군에서 불안반응이 낮았으며(P = 0.033), 현재의 임신을 원하였던 초산부군에서 불안반응이 낮았다(P = 0.034). 6. 실험군, 대조군Ⅰ, 대조군Ⅱ사이의 초산부의 일반적 특성 및 제변수들에 따른 상태불안반응에서는 실험군에서 산전진찰을 정기적으로 받은 집단에서 불안반응이 낮았으며(P = 0.037), 대조군Ⅰ에서 분만제 1기 소요시간이 길수록 불안반응이 높았으며(P = 0.032) 대조군Ⅱ에서 결혼생활이 만족한 집단에서 불안반응이 낮았다(P=0.035) 7. 실험군, 대조군Ⅰ, 대조군Ⅱ사이의 남편의 연령, 학력, 직업에 따른 상태불안반응에서는 실험군에 있어서 남편의 교육정도가 높을수록 불안반응이 낮았다(P = 0.047).;The purpose of this study was to examine the effects on anxiety of primiparous women as related to husband's presence in the delivery process and childbirth education. The sample consisted of 90 primiparous women and husbands who were measured during the period of labor at one general hospital. The primiparous women and husbands were divided into three groups: those who took planned childbirth education that was provided by researcher and were present in the delivery process (experimental group), those who were present in the delivery process but didn't take education (control group Ⅰ) and those who neither took education nor were present in the delivery process (control group Ⅱ). Research data was collected from the 1st of Sept. to the 10th of Oct. 1983 measuring by scores on the State-Trait Anxiety Inventory (STAI). Summary of the results of study were as follows: 1. The State-Anxiety of primiparous women between the experimental group and control group (Ⅰ) aren't different. (P=0.07) 2. The State-Anxiety of primiparous women are lower in the experimental group than control group. (Ⅱ) (P=0.004) 3. The State-Anxiety of primiparous women between the control group (Ⅰ) and control group (Ⅱ) aren't different. (P=0.364) 4. The State-Anxiety of primiparous women in the experimental group are the lowest of all the groups. (P=0.022) 5. In the State-Anxiety of primiparous women as related general characteristics and other variables, those who are satisfied in the marital life are lower than the unsatisfied. (P=0.033) And those who are planned pregnancy are lower than the unplanned. (P=0.034) 6. The State-Anxiety of primiparous women as related general characteristics and other variables are different among the three groups. In the experimental groups, those who have regularly antenatal care are lower the anxiety than not regularly. (P: 0.037) In the control group (Ⅰ), the longer primiparous women have labor, the higher they have anxiety. (P=0.032) And, in the control group (Ⅱ), those who are satisfied in the marital life are lower than the unsatisfied. (P=0.035). 7. In the State-Anxiety of primiparous women as related husband's general characteristics, the more husbands have education, the lower primiparous women have anxiety (P=0.047) Therefore, social support during labor is a critical factor in reducing anxiety of pregnant woman. Increasing the availability of childbirth education for couples and encouraging greater support from husband during birth could improve birth experience of pregnant woman.
