Abstract
본 연구는 입원환자의 초기영양상태를 국내병원의 환경을 고려하여 신속 정확하게 평가하기 위한 새로운 영양검색도구를 개발하고, 타당성을 검증하기 위해 수행되었다. 입원환자 903명을 대상으로, 의무기록 및 개별면담을 통해 성별, 연령, 혈중 알부민, 총임파구수, BMI, 헤모글로빈, 헤마토크리트, 총단백질, 혈중 콜레스테롤, 체중변화, 섭취변화의 자료를 수집하였다. PGSGA (Patient-Generated Subjective Global Assessment)를 통해 양호군과 영양불량군 두 군으로 범주화 한 뒤 로지스틱 회귀분석 후진법을 통해 회귀식을 산출하여, 새로운 영양검색도구인 ISNS(Initial Screening for Nutritional Status)를 개발하였다. 연령, BMI, 혈중 알부민, 혈중 콜레스테롤, 총단백질, 헤마토크리트, 체중변화, 섭취변화여부가 최종 영양평가지표로 선정되었다. ISNS의 영양평가결과 총 903명 중 양호군은 620명(68.7%), 영양불량군은 283명(31.3%)으로 나타났으며, 양호군과 영양불량군사이에 연령, 혈중 알부민, 총임파구수, BMI, 헤모글로빈, 헤마토크리트, 총단백질, 혈중 콜레스테롤, 재원일수 모두 유의적인 차이를 보였다. 타당도 검증과정에서 ISNS는 PGSGA와 민감도 94.4%, 특이도 88.4%, 카파비 0.747로 ‘Substantial agreement’를 보였으며(P<0.001), NRS2002 (Nutrition Risk Screening 2002)와 민감도 96.1%, 특이도 79.5%, 카파비 0.505로 PGSGA보다는 낮으나, ‘Moderate agreement’를 보였다(P<0.001). 결론적으로 본 연구에서 새로 개발한 ISNS는 의무기록 및 단답형의 질문으로 파악 가능한 연령, BMI, 혈중 알부민, 혈중 콜레스테롤, 총단백질, 헤마토크리트, 체중변화, 섭취량변화가 영양평가지표로 구성되어 영양평가 방법이 간단하고 시간이 적게 걸리므로 평가의 용이성, 신속성을 만족한다. 또한 PGSGA, NRS2002와의 높은 일치도를 통해 타당도가 검증되었으며, 재원기간과 유의적인 차이를 보여 유용한 도구로 평가된다. 이 ISNS와 합병증의 유무, 사망률, 재입원률, 병원비용 등의 연관성검증을 위한 추가적인 연구가 필요할 것으로 사료된다.;The present study was designed to develop a new screening tool for initial nutritional status (ISNS) quick and accurate for hospitalized patients in Korean domestic hospital and to validate it. The subjects of this study were 903 patients hospitalized in a clinic located in Seoul and the data regarding gender, age, albumin, TLC, BMI, hemoglobin, hematocrit, total protein, cholesterol, weight changes and intake changes were collected by electronic medical records and individual interviews. After categorizing them into normal group and malnutrition group by PGSGA (normal plus malnutrition stage 1 VS malnutrition 2/3), the variables for ISNS were selected by the backward logistic regression analysis. The final selection of variables for ISNS included age, BMI, albumin, cholesterol, total protein, hematocrit, weight changes and intake changes. 620 patients (68.7%) belonged to the normal group and 283 patients(31.3%) belonged to the malnutrition group. Between normal and malnutrition patients, age, albumin, TLC, BMI, hemoglobin, hematocrit, total protein, cholesterol, and LOS showed significant differences. In the process of validation, the comparison of nutritional screening by PGSGA and ISNS at admission showed ‘Substantial agreement’ (sensitivity 94.4%, specificity 88.4%, k=0.747). Also, the comparison of nutritional screening by NRS-2002 and ISNS at admission showed ‘Moderate agreement’ (sensitivity 96.1%, specificity 79.5%, k=0.505), indicating that ISNS held high validity. In conclusion, ISNS newly developed in this study seems relatively easier and quicker to apply, since the most of variables are available in electronic medical records and by short-answer questions. Compared with PGSGA and NRS2002, ISNS has high validity and it is evaluated as a useful tool with significant differences with length of stay in hospital. Additional investigations are need to evaluate the relationship between ISNS and other risks such as disease complications, mortality, rehospitalization, and costs.
