요양병원의 간호인력 확보수준이 환자결과에 미치는 영향

Abstract

본 연구는 지속적으로 증가하고 있는 요양병원에서의 간호인력 확보수준이 환자결과에 미치는 영향을 검증하기 위해 기관관련 요인, 환자관련 요인, 시장관련 요인을 통제한 상태에서 간호인력 확보수준이 환자결과에 미치는 영향을 분석한 종단적-후향적 연구이다. 본 연구는 2008년 1월 1일부터 2011년 12월 31일까지 운영 및 개․폐업한 모든 요양병원 중 4년 동안 지속적으로 운영을 유지해 온 요양병원 458개, 청구건수 2,972,361건, 입원환자 304,398명을 대상으로 하여 균형패널자료로 구축하였다. 수집된 자료는 SAS Enterprise Guide 4.3 버전 프로그램을 이용하여 기술통계방법, 상관분석(correlation analysis), 일원오차성분모형-시간고정효과 모형을 이용한 패널회귀분석(One-way error component-time fixed effect model)과 일반화추정방정식(Generalized Estimating Equation) 모형을 적용하여 분석하였다. 본 연구에서 간호인력 확보수준은 연평균 환자당 간호사와 간호조무사 수의 합인 환자 당 간호인력 수와 간호인력 중 간호사인력 비율로 측정하였다. 환자결과는 욕창발생 및 악화비율, 낙상발생비율, 일상생활 수행능력 감소비율, 상급종합병원 및 종합병원으로의 입원비율로 측정하였다. 본 연구의 결과는 다음과 같다. 1. 요양병원의 간호인력 확보수준이 욕창발생 및 악화에 미치는 영향은 패널회귀분석과 일반화추정방정식 모형 모두 기관관련 요인, 환자관련 요인, 시장관련 요인을 보정하였을 때 간호인력 중 간호사인력 비율이 증가할수록 욕창발생 및 악화비율이 유의하게 감소하는 것으로 나타났다. 2. 요양병원의 간호인력 확보수준이 낙상발생에 미치는 영향은 두 모형 모두유의하지 않은 것으로 나타났다. 3. 요양병원의 간호인력 확보수준이 일상생활수행능력 감소에 미치는 영향은 두 모형 모두 기관관련 요인, 환자관련 요인, 시장관련 요인을 보정하였을 때 간호인력 중 간호사인력 비율이 증가할수록 일상생활수행능력 감소 비율이 낮아지는 것으로 나타났다. 4. 요양병원의 간호인력 확보수준이 급성기병원에 미치는 영향은 두 모형 모두 기관관련 요인, 환자관련 요인, 시장관련 요인을 보정하였을 때 간호인력 중 간호사인력 비율이 증가할수록 상급종합병원과 종합병원으로의 입원이 증가하는 것으로 나타났다. 본 연구에서 요양병원의 기관관련 요인, 환자관련 요인, 시장관련 요인을 보정하였을 때 요양병원의 간호인력 중 간호사인력 비율의 증가가 환자의 욕창발생 및 악화비율을 낮추며, 일상생활 수행능력 감소를 지연시키고, 급성기병원으로의 입원을 증가시키는 것으로 확인되었다. 이러한 결과는 요양병원의 간호사들이 입원환자의 건강문제를 신속히 확인하고 이에 대한 중재를 제공하고 있음을 보여주는 근거로 향후 요양병원에서의 간호사인력 확보에 대한 근거자료로 활용될 수 있을 것이다.;The purpose of this study is to examine the effect of nurse staffing on patient outcomes in long-term care hospitals(LTCHs), using 4-year (2008-2011) longitudinal data. The secondary data for the analysis were from the Health Insurance Review and Assessment Service(HIRA). Nurse staffing levels were measured using two indicators: number of Registered Nurses and Certified Nursing Assistants per inpatient and nursing skill-mix. Patient outcome included pressure ulcer, fall, functional decline, and hospitalization. Donabedian’s (1980) theoretical model of structure, process, and outcomes were used as the framework guiding the investigation. The balanced panel regression data analysis with one-way fixed effects model and generalized estimating equation(GEE) model were used. Based on the statistically significant relationship, the results of this study were as follows: 1. Skill mix(Registered Nurse/Nurse staffing) had significantly negative relationships with pressure ulcer in both models. 2. Nurse staffing level did not have statistically significant relationship with fall in both models. 3. Skill mix had significantly negative relationships with functional decline in both models. 4. Skill mix had significantly positive relationships with hospitalization in both models. The findings of this study supports the importance of registered nurse staffing level in maintaining and decreasing negative patient outcomes. Nursing policies should be established to ensure an adequate nursing staffing level threshold.
