dc.description.abstractThe high rate of maternal death remains a major public health concern in Tanzania. Most maternal deaths are caused by factors attributed to pregnancy, childbirth and poor quality of health services. More than 80 percent of the Tanzanian population lives within 5km from the health facility. However, regardless of this relative coverage of health facilities, access to reproductive health for pregnant women is still a challenge (MoHSW 2007). Access to good quality reproductive health care and effective interventions is very significant to women in the reduction of maternal mortality in developing countries (WHO 2003). The goal of the paper is twofold: First, it analyzes the factors for the failure of policy for increasing universal accessibility to reproductive health service. Second it suggests policies that could be adopted by the health sector, women and community and research institutes to accelerate progress towards reduction of maternal death in Tanzania. This paper argues that increased access to reproductive health alone has not helped to reduce maternal mortality in Tanzania since women’s access to reproductive health service is being affected by factors such as insufficient professional workers, weak referral systems, insufficient equipment and supplies which have critically limited health center’s capacity to deliver the service for the needy. Also socio-cultural factors affect women from going to the health centers in rural Tanzania. This includes: women not being allowed to go to hospital until they get permission from their spouse, some societies practices such as early marriages. Further, lack of education limits pregnant women from going to the hospital. These are the major factors that constraints women’s access to reproductive health leading to high maternal mortality in Tanzania. Maternal mortality can be prevented if the following factors are considered: strengthening the health system by providing good working conditions for workers in the rural areas. In addition, well organized health referral system, upgrading rural health centers with quality and necessary equipment, operations research addressing maternal health challenges for informed decision making in policy formulation, commitment to the policy and enhancement of health sector planning should be undertaken. Lastly, empowerment of women, families and community at large is very important for the active participation in the projects and programmes aiming at improving reproductive health as well maternal mortality.;높은 산모 사망률은 탄자니아의 주된 공공 보건 과제로 남아있다. 대부분의 산모 사망의 원인은 임신, 출산과 열악한 보건 서비스의 질과 관련이 있다. 탄자니아 인구의 80% 이상이 건강 시설로부터 5km 이내에 거주하고 있지만, 보건시설의 상대적인 보급률과는 무관하게도 산모의 출산 건강에 대한 접근은 여전한 과제로 남아있다. 우수한 수준의 출산 건강 보호에 대한 접근과 효과적인 개입은 개발 도상국에서 산모의 사망률을 감소시키기 위해 매우 중요하다. 본 논문은 두 가지 목적을 띤다. 첫째, 출산 건강 서비스의 보편적 접근성을 향상시키는 데 실패한 정책의 원인을 분석한다. 둘째, 본 논문은 탄자니아에서 산모 사망률 감소를 촉진하기 위해 보건 분야, 여성, 지역사회, 그리고 연구소가 채택할 수 있는 정책을 제안하고자 한다. 본 논문의 연구 결과, 향상된 출산 보건 접근성 만으로는 탄자니아의 산모 사망률을 감소 시킬 수 없었다. 이는 여성의 출산 보건 서비스에 대한 접근성이 불충분한 전문 인력, 열악한 위탁 시스템, 도움이 필요한 이들을 위한 보건시설의 역량을 현격하게 제한시킨 불충분한 장비와 물품과 같은 요인에 영향을 받기 때문이다. 또한, 사회 문화적 요인은 탄자니아 농촌 지역의 여성이 보건 시설을 이용하는데 영향을 끼친다. 예를 들면, 여성은 배우자의 허락을 받기까지는 병원에 갈 수 없다. 그리고 조기 결혼과 교육의 결여와 같은 사회적 관습은 임신한 여성이 병원을 이용하는 것을 제한시키고 있다. 이는 탄자니아의 높은 산모 사망률로 이어지는 여성의 출산 보건 접근성을 제한시키는 주된 원인들이다. 다음과 같은 요인이 고려된다면, 산모 사망률은 예방될 수 있다. 농촌 지역의 근로자들에게 좋은 근무 환경을 제공함으로써 보건 시스템을 강화하는 것. 또한, 잘 정비된 보건 위탁 시스템, 필요한 좋은 품질의 장비를 갖춘 향상된 농촌 보건소, 산모 건강과 관련된 정책 수립 시 현명한 의사 결정을 위해 필요한 수학적 분석 방법, 그리고 보건 분야의 계획을 향상시키기 위해서는 정책을 실행하는 데 있어서 헌신과 노력이 선행되어야 한다. 마지막으로, 여성, 가정, 그리고 지역 사회 전체의 역량을 강화하는 것은 산모 사망률 감소와 출산 건강 향상을 위한 프로젝트와 프로그램의 적극적인 참여를 위해 매우 중요하다.-
