dc.description.abstractBackground and Purpose: The microenvironment of breast cancer has been increasingly considered as a therapeutic target or biomarker for diagnosis and prognosis. Several populations of tumor microenvironment are shown to be related to support or reduce malignancy. Among those cells, a great deal of attention has been paid to the unique contribution of macrophages in breast cancer development. Macrophages are characterized by high plasticity and polarized into M1 and M2 type. M1 macrophage has been known to be antitumoral, whereas M2 is protumoral. Tumor associated macrophages (TAMs) are a subpopulation of M2 macrophages, which enhance tumor growth by stimulating angiogenesis, secreting growth factors, suppressing other immune responses, and facilitating invasion and metastasis. The purpose of this study is to evaluate prognostic impacts of degree of infiltration of M1 and M2 macrophages, which are separately quantified as intratumoral or peritumoral in breast cancer. Also, the study aims to evaluate the relation of M2 macrophage in tumor angiogenesis, and detect cancer-M2 dual positive cells in human breast cancer. Materials and Methods: A total of 193 cases of surgically resected breast cancer were collected from Ewha Womans University (EUMC) Mokdong hospital (2003-2007). In the review of the clinical record & pathology report, age, tumor size, nodal status, nuclear grade, histologic grade, hormone receptor status, Her2 status, Ki67 proliferation index, p53, recurrence and survival factors were determined. Molecular classification of breast cancer was also performed. Immunohistochemistry for CD163 as a M2 marker, iNOS as a M1 marker, and CD34 for microvessel density was performed using a representative whole tumor tissue block. Double immunohistochemistry of CD163 and pan-cytokeratin were performed to detect dual positive cells in the tumor. Morphometric evaluation for CD163 & iNOS expression in macrophage was performed by manual calculation of positive cells in 3 fields of the hot spots at 400x magnification, which was separately done at intratumoral and peritumoral areas. Tumor cell expression of CD163 and iNOS was also evaluated semiquantitatively. Microvessel density was counted using CD34 immunostain. The results were compared with clinicopathological parameters and statistical analysis was done. Results: M2 macrophage infiltration in both intratumoral and peritumoral areas was significantly increased compared to the M1. The M2/M1 ratio was 4.46 in the intratumoral area. M2 counts were significantly correlated with positive nodal status (p<0.01), higher nuclear grade, higher histologic grade (p<0.001), hormone receptor negativity (p<0.001), high Ki67 labeling index (p<0.02), and overall survival (p<0.008). In contrast, M1 was not associated with any prognostic factors except high histologic grade, and Ki67 labeling index in intratumoral area. According to the molecular subtypes, M2 was significantly low in luminal A type in both intratumoral and peritumoral areas (p<0.001). M2 score in both intratumoral and peritumoral areas was significantly correlated with microvessel density (p<0.001). Tumor cell expression of CD163 was associated with higher histologic grade, negative hormone receptor status, high KI67 index, and triple negative breast cancer. The iNOS expression in tumor cells was not correlated to any prognostic parameters. Conclusion: M2 macrophage infiltration is increased in breast cancer tissue compared to M1 and significantly associated with poor prognostic factors, microvessel density, and aggressive molecular subtype. Selective suppression of TAM is a novel candidate for targeted therapy in aggressive molecular type of breast cancer. Key words: breast cancer, tumor associated macrophage, CD163, iNOS, tumor microenvironment;연구배경 및 목적 : 종양 미세환경이 종양의 발생과 성장, 침습과 전이에 미치는 영향에 대하여 여러 분야에서 활발한 연구가 진행되고 있으며, 미세환경 조절을 통한 종양억제는 종양치료와 관련한 주요 연구과제가 되고 있다. 종양 미세환경의 다양한 구성요소 중 골수의 단핵구로부터 분화되는 탐식구는 높은 유연성을 가지며 M1 또는 M2 유형으로 극화되는데, M1은 종양을 억제하고 M2는 신생혈관 생성 촉진, 성장인자 분비, 여러 항 종양성 면역기전 억제 작용을 통해 종양의 침습과 전이에 기여한다. 종양 미세환경과 탐식구의 세포연관성은 유방암을 비롯하여 폐암, 기저암 등 여러 종양에서 연구되고 있으나 탐식구의 침윤 정도를 정량화하여 M1과 M2 유형으로 나누어 예후와 비교.분석한 사례는 없었다. 이 연구는 유방암에서 M1과 종양관련 탐식구인 M2의 침윤 정도를 정량하고 이를 여러 예후인자 및 미세혈관밀도와 비교.분석함으로써 종양관련 탐식구의 침윤 정도와 예후의 상관관계를 살펴보고 종양치료 관련 유용성에 대해서 검토해 보고자 계획되었다. 연구재료 및 방법 : 2003년부터 2007년까지 이대목동병원에서 유방암으로 진단되어 수술이 시행된 사례 중 침윤성 관암종이나 특수한 조직유형이 아닌 경우로 진단되고, 임상정보 수집이 가능한 193예를 무작위로 수집하였다. 병리기록과 임상기록을 토대로 종양의 크기, 림프절 전이여부, 핵등급, 조직등급, 호르몬수용체 여부, Her2, Ki67 index, p53, 재발과 생존여부를 분석하고 결과를 토대로 분자 분류를 시행하였다. M2 탐식구에 대한 표지자로 CD163, M1 탐식구에 대한 표지자로 iNOS, 미세혈관밀도 측정을 위해서 CD34를 이용해 면역염색을 시행하였으며, 종양세포 중 CD163 발현 세포를 찾기 위하여 CD163과 pancytokeratin에 대한 이중 면역염색을 시행하였다. 형태계측을 위하여 면역염색 양성 탐식구가 집중 침윤된 곳을 종양 내부와 종양 주변부에서 각각 3곳씩 선정하여 400배 고배율시야에서 양성 세포를 계수하고 침윤 정도에 따라 등급화 (0~3 등급) 하였다. M2 탐식구와 M1 탐식구의 침윤 정도와 여러 예후 지표 및 미세혈관밀도와 상관관계를 분석하였고, 종양세포에서 CD163과 iNOS의 발현 여부도 반정량적인 방법으로 분석하여 여러 지표들과 비교하고 통계학적인 분석을 시행하였다. 결과: M2 탐식구의 침윤 정도는 종양 내부와 종양 주변부에서 모두 M1에 비해 현저히 높았고 종양내부의 M2/M1 비율은 4.46이었다. M2 탐식구의 침윤 정도는 종양 주변부에서 22.09, 종양내부에서 18.23으로 종양 내.외부에서 유의한 차이를 보이지 않았다. M2 침윤 정도는 림프절 전이(p<0.01), 높은 핵등급 및 조직등급(p<0.001), 호르몬 수용체 음성(p<0.001), 높은 Ki67 측정지수(p<0.02), 전체생존율과 의미있는 상관관계(p<0.008)를 보였으나 종양의 크기, 무병생존율과는 무관했다. M1 침윤 정도는 어떠한 예후인자와도 상관관계를 보이지 않았다. 유방암의 분자유형과 M2 침윤 정도를 비교해보니 luminal A 유형에서 M2 탐식구의 침윤 정도가 종양 내부와 종양 주변부 모두 유의하게 낮게 나타났다 (p<0.001). 미세혈관밀도는 M2의 침윤 정도와 의미 있는 상관관계를 보였다 (p<0.001). 19예에서 종양세포에 CD163이 발현되었는데 이는 높은 조직등급 및 호르몬수용체음성, 높은 Ki67 지수, 삼중음성암과 상관관계를 보였다. 반면 iNOS는 59예에서 발현되었으나 예후인자와 의미 있는 관련성을 보이지 않았다. 결론: 유방암 조직에서 M2 탐식구의 침윤 정도는 M1에 비해 높고 불량한 예후인자와 높은 미세혈관밀도, 공격적인 분자유형과 관련이 있었다. 이는 종양관련 탐식구에 대한 선택적 억제가 종양치료에 있어 효과적인 방법이 될 수 있음을 시사하는 결과라고 할 수 있다.
