장기요양시설 요양보호사의 구강보건지식 및 구강보건 제공 실태조사

Abstract

본 연구는 장기요양시설 요양보호사의 구강보건지식 및 구강보건 제공 실태를 조사하고, 구강보건위생지침서와 구강보건교육이 구강보건 제공 실태에 주는 영향에 대하여 연구하여 요양보호대상자에게 보다 나은 구강보건을 제공하고, 요양보호사의 구강보건 교육의 기초자료로 활용하고자 하였다. 연구대상은 경기도 의정부시에 소재한 장기요양시설 71개소 중 본 연구의 목적을 설명하고 설문을 허락한 장기요양시설(요양원) 39개소, 요양보호사 369명 이었다. 설문은 334부(90.5%)를 회수하였고, 응답이 불충분한 28부를 제외한 306부(82.9%)를 최종 분석하였다. 설문은 인적사항 5문항, 구강보건지식문항 20문항, 구강보건 제공 실천 8문항, 구강보건교육관련 2문항으로 구성하였고, 구강보건지식과 구강보건실태를 정답 1점으로 하여 100점 단위로 점수화 하였다. 수집한 자료는 SPSS 18.0을 이용하여 연구대상자의 구강보건지식과 구강보건 제공 실태는 빈도분석을 하였고, 연령과 경력 및 대상자에 따른 구강보건지식과 구강보건 제공 실태는 분산분석과 t-test를 실시하였다. 또 구강보건 교육경험과 구강보건 위생지침서의 유무에 따른 구강보건지식과 구강보건 제공 실태는 교차분석과 t-test를 실시하였고, 연구대상자의 연령과 경력, 요양보호대상자 수, 지식이 구강보건 제공 실태에 미치는 영향은 회귀분석을 실시하고 다음과 같은 결과를 얻었다. 1. 요양보호사의 구강보건지식 점수는 100점 만점에 평균 82.8점이었고, 구강보건 제 공 실태 점수는 평균 78.8점이었다. 2. 요양보호대상자 수가 적을수록 구강위생용품 사용과 치과정기검진이 더 제공되었고, 구강보건교육경험이 있으면 구강보건 제공이 통계적으로 유의한 증가를 보였다. 3. 구강보건교육경험이 있으면 구강보건 제공 점수는 통계적으로 유의하게 높았으나, 구강보건지식 점수는 높지 않았다. 4. 장기요양시설에 구강보건위생지침서가 비치되어 있는 경우 구강보건 제공 점수가 더 높았고 통계적으로 유의하였다. 5. 구강보건교육경험과 구강보건위생지침서가 있는 경우 구강보건 제공 점수에 영향을 미치고 있었으며, 구강보건위생지침서가 통계적으로 더 유의한 상관성을 보였다.;This is to survey the oral-health knowledge of nursing home staffs working in the long-term care facilities (sanatoriums) and their effort to provide oral health services, and to investigate the relationship between the oral health care services and oral health education for the nursing staffs. The subjects in this study were 369 nursing home staffs working in 39 nursing homes that were selected from 71 long-term care facilities in Euijeongbu city, Gyeonggi Province. Of 369 staffs, 334 respondents(90.5%) returned the questionnaire. After excluding 28 incomplete answers, 306 respondents(82.9%) were analyzed. As for oral health knowledge, one point was given to the right answers and the sum scores were recalculated on the basis of 100 points. The results were tested by SPSS 18.0 package, ANOVA and t-test. The relationship between oral health education and their provision were analyzed by mutiple regression. The results of this study were as follows: 1) The nursing home staffs got 82.8/100 scores in oral health knowledge, and 78.8/100 scores in oral health care provision. 2) The less the nursing home patients per staffs was, the more oral health care services were given significantly. Also, there was significant increase in oral care services when the staffs were educated for oral health care. 3) If the staffs were educated for oral health care, the scores of care service were higher significantly but there were no significant differences in oral health knowledge scores. 4) There were no significant differences in oral health knowledge according to their age and career, but the number of recipients made significant differences in use of dental hygiene supplies and periodical dental examination. 5) The correlation between the oral health care services and oral hygiene guidebook were significantly higher than the relationship with oral health education. Conclusively, the better oral health care services were provided for elderly patients when the long-term care facilities were equipped with dental hygiene guides and when the nursing home staff members received oral health education.
