Abstract
간은 포도당 대사 작용에 중요한 역할을 하는 장기로서 간경변증 환자에서는 높은 내당능 장애와 당뇨병의 유병률이 보고되어 있다. 이러한 포도당 대사이상에 주로 간에서 생산되어 포도당 대사에 동화작용을 하는 펩티드인 Ⅰ형 인슐린양 성장인자(insulin-like growth factor Ⅰ ; 이하 IGF-Ⅰ)의 감소가 미치는 영향에 대한 연구는 많지 않다. 이에 본 연구에서는 간경변증 환자에서 간기능 장애의 정도에 따른 IGF-Ⅰ 농도를 비교하여 당뇨병 및 내당능 장애와의 연관성을 살펴보고 비정상적 내당능 상태에 영향을 미치는 위험요소를 분석하고자 하였다. 당뇨병 이외의 다른 만성, 소모성 질환을 동반하지 않은 간경변증 환자 32명을 대상으로 하였다. 간기능에 따른 분류는 Child-Pugh criteria에 따라 Child A군은 대상성 간경변증 환자군으로, Child B군과 Child C군은 비대상성 간경변증 환자군으로 분류하여 대상성 간경변증 환자군은 14명, 비대상성 간경변증 환자군은 18명이었다. 공복으로 IGF-Ⅰ, 혈당 및 간기능 측정후 기존의 당뇨병이 없는 환자 및 공복 혈당 126 mg/dL미만의 환자를 대상으로 75g 경구 포도당부하후 2시간 혈당을 측정하였다. 내당능 상태에 따라 정상 내당능 상태와 당뇨병, 내당능 장애 및 당뇨병의 과거력이 있는 환자군을 비정상 내당능상태로 분류하여 다음과 같은 결과를 얻었다. 1) IGF-Ⅰ은 대상성 간경변증 환자군 276.3±180.3 ng/ml, 비대상성 간경변증환자군에서 151.8±71.7 ng/ml로 간기능 장애가 심할수록 유의하게 감소하였고(p=0.048) IGF-Ⅰ과 알부민(r=0.43, p<0.05), 프로트롬빈 응고 시간(r=0.43, p<0.05)이 IGF-Ⅰ과 유의한 양의 상관관계를 보였다. 2) 전체의 75%가 비정상 내당능 상태의 소견을 보였는데 이중 대상성 간경변증 환자군은 57.1%, 비대상성 간경변증 환자군은 88.9%으로 비대상성 간경변증 환자군에서 비정상 내당능 상태가 유의하게 많았다(p=0.040). 3) 비정상 내당능 상태의 간경변증 환자군과 정상 내당능의 간경변증 환자군을 비교시 연령만이 유의한 차이가 있었으며(p=0.010) 성별, 원인별 및 간기능을 대별하는 생화학적 인자들에 따른 차이는 없었다. IGF-Ⅰ은 비정상 내당능 상태의 간경변증 환자군에서 더 낮았으나 통계적 의미는 없었다. 4) 간경변증 환자에서 당뇨병 및 내당능 장애의 위험요인을 분석하기 위하여 다변량 회귀분석을 시행하였으나 알부민, 빌리루빈, 프로트롬빈 응고시간, IGF-Ⅰ및 연령 중 유의한 인자는 없었다. 결론적으로 IGF-Ⅰ은 간의 합성 능력을 평가하는 유용한 인자로서 간기능 장애가 심할수록 IGF-Ⅰ은 감소하였고 내당능 장애 및 당뇨병 발생률이 증가하였으나 IGF-Ⅰ의 감소가 유의한 영향인자는 아니었다. IGF-Ⅰ이외의 알부민, 빌리루빈, 프로트롬빈 응고시간 및 연령 중 당뇨병 및 내당능 장애의 위험요인으로 설명될 수 있는 인자는 없었으며 내당능 상태에 영향을 주는 인자는 다른 복합적인 요인이 작용할 것으로 보인다.;The liver plays a central role in carbohydrate metabolism and high prevalences of glucose intolerance and diabetes were reported in patients with liver cirrhosis. Insulin like growth factor-I(IGF-I) is a polypeptide which have important anabolic actions on carbohydrate metabolism and the majority of circulating IGF-I is synthesized in the liver. So, the present study aimed to evaluate IGF-I according to hepatic dysfunction and examine the relationships between IGF-I and the severity of hepatic dysfunction, and glucose tolerance status in patients with liver cirrhosis. Thirty-two cirrhotic patients were involved in this study. They were devided in patients with compensated(n=14, Child A) and decompensated liver cirrhosis(n=18, Child B+C). After measuring fasting serum IGF-I & glucose, we performed oral glucose tolerance test in patients with less than 126 mg/dL fasting glucose and without past diabetes history. (1) Serum IGF-I levels were significantly lower in patients with decompensated liver cirrhosis(p=0.048). And, IGF-I was also correlated well with Albumin and prothrombin time(Υ-0.43, r=0.43). (2) Prevalences of abnormal glucose tolerance status were 75% in total cirrhotic patients and 57.1% in compensated, 88.9% in decompensated liver cirrhosis(p=0.040). (3) Abnormal glucose tolerance group(n=24) were compared with normal glucose tolerance group(n=8). IGF-I levels were not significantly lower in patients with abnormal glucose tolerance and only age was significantly associated(p=0.010). (4) Multivariate logistic regression analysis showed that IGF-I, albumin, bilirubin, prothrombin time and age were not risk factor for glucose intolerance in patients with liver cirrhosis. In Conclusions, Serum IGF-I was a marker of hepatic synthetic capacity and lower in decompensated cirrhotic patients. Glucose intolerance and diabetes in cirrhotic patients were highly prevalent according to severity of hepatic dysfunction but, abnormal glucose tolerance status in cirrhotic patients cannot be independently explained by serum IGF-I level. Another hepatic functional paramerters and age were also not risk factor for diabetes and glucose intolerance in patients with liver cirrhosis.
