위절제 수술 전 Prealbumin 수치와 수술 후 감염 합병증과의 관계

Abstract

수술 전 영양상태는 수술 후 합병증과 관련이 있다. 내장단백질은 영양평가지표로서 임상적으로 많이 활용되고 있는데, 이 중 하나인 prealbumin은 단백질 영양실조증에 민감한 지표로 알려져 있다. 이 연구에서는 위절제 수술 후 합병증의 예후인자로서 수술 전 prealbumin 수치의 유용성을 알아보고자 하였다. 2009년 8월부터 2010년 10월까지 서울대학교병원에서 양성 또는 악성 위질환으로 위절제술을 시행한 환자 중에서 수술 전 prealbumin 수치를 측정한 183명을 대상으로 관찰 연구를 수행하였다. 내장단백질인 prealbumin (구분점, 18 mg/dL) 과 albumin, 인체측정학적 파라미터인 체질량지수와 같은 영양평가지표를 조사하였고, 환자의 기본 정보들과, 수술 및 질환과 관련된 임상병리학적인 특성을 수집하였다. 임상적 결과로 수술 후 재원기간, 수술 후 30일 이내의 합병증과 사망률을 조사하였다. 수술 전 정상 prealbumin 수치를 보인 환자 (n=160) 에서 합병증 발생률은 24%였으나, 낮은 prealbumin 수치를 보인 환자 (n=23) 에서는 합병증 발생률이 52%로 유의하게 높았다 (p=0.005). 수술 전 저알부민혈증 (<3.5 g/dL) 이 있거나 체질량지수 (<18.5 kg/m2) 가 낮은 환자에서 합병증 발생률이 높게 나타났으나 통계적 유의성은 없었다. 수술 후 전체 합병증과 유의한 상관관계를 보인 변수는 당뇨병, 위절제 정도, 합병절제 여부, TNM 병기, 수술 전 prealbumin 수치로 나타났다. 다변량 분석 결과, 이들 중 기저질환으로 당뇨병이 있는 경우 (p=0.001), 합병절제를 한 경우 (p=0.001) 통계적 유의성을 나타내었다. 합병증 중 감염 합병증에 대한 단변량 분석에서는 위절제 정도, 접근방식, 합병절제 여부, TNM 병기, 수술 전 prealbumin 수치가 유의한 상관성을 보였으며, 다변량 분석에서 합병절제 (p=0.001) 와 수술 전 prealbumin 수치 (p=0.032) 가 독립적인 변수로 확인되었다. 위절제 수술 전 prealbumin 수치는 수술 후 합병증, 특히 감염 합병증 발생을 예측할 수 있는 예후인자로 활용 가능할 것으로 생각된다.;Preoperative nutritional status is associated with postoperative complications. Prealbumin, a visceral protein, is sensitive to protein malnutrition. The objective of this study is to evaluate the role of preoperative prealbumin levels as a marker for predicting complications after gastric surgery. An observational study was performed on 183 patients who underwent gastric surgery due to benign or malignant gastric disease at Seoul National University Hospital (SNUH) between August 2009 and October 2010. Preoperative prealbumin levels were also measured. Nutritional variables such as prealbumin (cutoff value, 18 mg/dL), albumin, body mass index (BMI), and clinicopathologic data were collected. Postoperative hospital stay, 30-day complications and mortality rate were obtained to investigate outcomes. The complication rate was 52% in the abnormal prealbumin group (n=23) and 24% in the normal prealbumin group (n=160; p=0.005). The complication rate was higher in patients with low preoperative albumin levels (<3.5 g/dL) and abnormal BMI (<18.5 kg/m2), but the differences were not statistically significant. Comorbidity of diabetes mellitus (DM), resection extent, combined resection, TNM stage and prealbumin levels were associated with complications. In multivariate analysis, DM and combined resection were significantly correlated with complications (p=0.001 for each). In subgroup analysis, resection extent, approach, combined resection, TNM stage and prealbumin levels were significantly associated with infectious complications. Multivariate analysis identified combined resection (p=0.001) and prealbumin levels (p=0.032) as independent variables. Preoperative prealbumin levels could be a useful marker for predicting complications, especially infectious complications, after gastric surgery.
