응급기도관리 교육방법에 따른 간호사의 자기 효능감, 임상수행능력

Abstract

본 연구는 응급기도관리에 대한 강의식 교육 방법과 시뮬레이션기반 교육 방법에 따른 간호사의 자기 효능감과 임상수행능력 정도에 대한 차이를 비교하기 위하여 시도되었다. 연구설계는 집단 비교연구로 자료수집은 2012년 4월 28일부터 2012년 5월 12일까지였으며 응급기도관리에 대한 강의, 소그룹 워크숍, 팀시뮬레이션을 적용한 시뮬레이션군과 응급기도관리에 대한 강의를 받은 강의군으로 나누어 진행하였다. 연구 도구로는 응급기도관리에 대한 자기 효능감은 천희숙(2001)이 개발한 응급간호 수행력에 대한 효능감의 도구를 연구자가 수정 보완한 도구를 사용하였고, 임상수행능력 측정 도구는 김윤희(2010)가 개발한 심폐응급간호 임상수행능력 측정도구를 연구자가 수정 보완하여 사용하였다. 수집된 자료는 SPSS WIN 18.0을 사용하여 빈도, 백분율, 평균, 표준편차, t-test, Chi-square test, Fisher’s exact test, Mann-Whitney Test 등의 통계방법으로 분석하였다. 연구 결과는 다음과 같았다. 1. 제 1가설인 ‘시뮬레이션기반 응급기도관리 교육은 받은 시뮬레이션군은 강의식 교육을 받은 강의군보다 자기 효능감이 유의하게 높을 것이다’는 시뮬레이션군(4.04±.55)이 강의군(3.34±.32)보다 통계적으로 유의하게 높아 지지되었다(t=5.985, p<.001). 2. 제 2가설인 ‘시뮬레이션기반 응급기도관리 교육은 받은 시뮬레이션군은 강의식 교육을 받은 강의군보다 임상수행능력이 더 높을 것이다’는 시뮬레이션군(4.26±.57)이 강의군(3.51±.47)보다 통계적으로 유의하게 높아 지지되었다(t=5.532, p<.001). 이상의 결과를 통해 시뮬레이션기반 응급기도관리 교육은 간호사의 자기 효능감, 임상수행능력에 효과적인 교육프로그램임이 검증되었다. 시뮬레이션기반 응급기도관리 교육 프로그램은 복잡하고 어려운 여러 가지 상황을 시뮬레이터를 통해 직접수행 해 봄으로써 임상현장에서 문제해결에 필요한 임상수행능력을 습득하고 응급기도관리에 대한 자기 효능감, 간호사의 전문적 역량을 키우는 데 도움이 되는 교육임을 확인하였다.;This study was conducted to compare the self efficacy and clinical performance ability of nurses in education of lecture type and simulation-based education. A comparative study design was used. Data were collected from April 28, 2012 to May 12, 2012. A total of 60 nurses, 30 nurses for the simulation-based education group and 30 nurses for the lecture group, applied the education program of simulation-based emergency airway management education program. Data profiling was executed through preparatory investigation about measurement variable, simulation-based education group was educated lecture, small group workshop, team simulation for emergency airway management and lecture group was educated lecture for emergency airway management. Data were analyzed by the statistical method such as frequency, ratio, t-test, Mann-Whitney Test, Chisquare by the SPSS WIN 18.0. The simulation-based education group showed significantly higher self efficacy of emergency airway management compared with the lecture group(t=5.985, p<.001). The simulation-based education group showed significantly higher clinical performance ability of emergency airway management compared with the lecture group(t=5.532, p<.001). According to the results above, simulation-based education was verified to be an effective teaching method to improve the self efficacy, clinical performance skills of nurses in the learning of emergency airway management. Based on the findings of this study, following recommendations are made. First, it is recommended that simulation based emergency airway management programs applicable to nursing education be developed and distributed to the field of education, so that these are generalized. Second, it is recommended that more studies on the effectiveness of simulation-based training programs fostering self-evaluation and objective evaluation by the observer to measure simultaneously be conducted. Finally, it is recommended that many follow-up studies to verify these effects.
