Abstract
OBJECTIVES The influence of ADRB2 gene polymorphisms on pharmacodynamics of ritodrine and the relationship between genotypes of sulindac metabolizing enzymes and pharmacokinetics of sulindac. METHODS Sixty six patients with preterm labor diagnosed between 20 and 37 weeks' gestation received ritodrine as an intravenous infusion. The time (hr) to delivery or therapeutic failure, the percentages of patients who were successfully treated up to 24 hr, 48 hr, and 7 days after the start of acute intravenous therapy, change in uterine contraction intensity, the mean gestational age at delivery, and neonatal birth weight were assessed. Patient demographics, maternal and perinatal outcomes were collected. In order to investigate the influences of genetic polymorphisms on the pharmacodynamics of ritodrine, 5 single nucleotide polymorphisms (SNPs) including ADRB2 rs1042713, rs1042714, rs1042717, rs1042718, and rs1042719 were analyzed. For sulindac study, nineteen patients diagnosed with preterm labor between 16 and 37 weeks' gestation were enrolled and given a 200mg sulindac by oral route twice a day for 2days. Blood samples were collected at the following times after first dosing: 0.5, 1.5, 4, 8, and 12 hr. And then the plasma concentrations of sulindac and its metabolites were determined using a validated HPLC method. In order to investigate the influences of genetic polymorphisms on the pharmacokinetics of sulindac, 8 single nucleotide polymorphisms (SNPs) including AOX1 rs2293528 and rs2248130, FMO3 rs2075992 and rs909530, MSRA rs4841331, rs9329221, rs6981758 and rs9644682 were analyzed. RESULTS In ritodrine study, mean age of subjects was 31.42 ± 4.62 years. The number of patients enrolled at <26 weeks' gestation, 26 to <28 weeks' gestation, 28 to <32 weeks' gestation, and ≥32 weeks' gestation was 10 (15.2 %), 6 (9.1 %), 18 (27.3 %), and 32 (48.5 %), respectively. For the ADRB2 rs1042717 G/A allelic variation, it took much shorter time to delivery or therapeutic failure in patients with AA genotype (83.7 ± 81.3 hr) than those with GG or GA genotype (174.8 ± 205.7 hr, P = 0.014). For the ADRB2 rs1042719 G/C allelic variation, it took much shorter time to delivery or therapeutic failure in patients with CC genotype (80.4 ± 88.2 hr) than those with GG or GC genotype (167.0 ± 197.9 hr, P = 0.028). From Cox's proportional hazards modeling, of the ADRB2 rs1042717, patients who had homozygote were 100 times faster to delivery or therapeutic failure than those who had heterozygote. On the other hand, the heterozygotes of the ADRB2 rs1042718 took 488 times shorter time for delivery or therapeutic failure than the homozygotes. In patients with heterozygotes, the proportions of successfully treated were significantly greater (75.0 %, at all the end points) than those with GG and AA genotype at all the end points (P = 0.013, 0.013, and 0.042, respectively at 24 hr, 48 hr, and 7 days) within the ADRB2 rs1042713 G/A allelic variation. From multiple linear regression model, ADRB2 rs1042718, gestational age at admission, and ADRB2 rs1042719 had significant influence on neonatal birth weight. Gestational age at admission, ADRB2 rs1042717, and ADRB2 rs1042718 had significant influence on gestational age at delivery. In sulindac study, mean maternal age was 32.05 ± 3.79 years and the number of patients enrolled at <28 weeks' gestation, and ≥28 weeks' gestation was 11 (57.9 %), and 8 (42.1 %), respectively. The retention times for sulindac, sulindac sulfone, sulindac sulfide, and IS were about 4.1 min, 5.4 min, 12.2 min, and 6.6 min, respectively, and their corresponding peaks were well separated from endogenous interfering peaks in human plasma. The calibration curves were linear over the concentrations ranging from 0.5 ㎍/mL to 10 ㎍/mL, 0.3 ㎍/mL to 5 ㎍/mL, and 0.3 ㎍/mL to 5 ㎍/mL for sulindac, sulindac sulfone, and sulindac sulfide, respectively (r=0.9993, 0.9981, and 0.9997, respectively). The Intra- and Inter-day precisions were in the ranges of 4.1 - 14.9% and 3.0 - 14.6%, the ranges of 6.3 - 9.0% and 7.3 - 13.0%, the ranges of 2.9 - 10.6% and 5.7 - 14.4%, for sulindac, sulindac sulfone, and sulindac sulfide, respectively. The mean AUCall values for sulindac/sulindac sulfone were highly dependent on the FMO3 rs909530 allelic variation (P = 0.036) and the MSRA rs9644682 allelic variation (P = 0.016). CONCLUSIONS In this study, we suggest that ADRB2 have significant effects on pharmacodynamics of ritodrine therapy and pharmacokinetics of sulindac is attributable to the genetic polymorphisms of FMO3 and MSRA in subjects with preterm labor. ;연구 목표 본 연구는 조기진통 환자에서 β-2 아드레날린 수용체의 유전적 다형성이 ritodrine의 약물동태에 미치는 영향과 sulindac 대사관련 효소들의 유전적 다형성이 sulindac의 약물동력과의 연관성을 평가하고자 하였다. 대상 및 방법 임신 주수 20-37주의 66명의 조기진통 환자에게 ritodrine을 정맥 주입하였다. 분만 또는 치료실패까지 걸리는 시간, 약물주입을 시작한 후 24시간, 48시간, 7일까지 성공적으로 치료된 환자의 백분율, 환자의 기본적인 정보들이 수집되었고 β-2 아드레날린 수용체 유전자의 유전적 다형성이 ritodrine의 약물동태에 미치는 영향을 평가하기 위하여 채집한 혈액으로부터 5 종류의 단일염기다형성 (SNP, single nucleotide polymorphism)을 분석하였다. 분석에 포함된 유전자들은 ADRB2 (rs1042713, rs1042714, rs1042717, rs1042718, rs1042719 이었다. Sulindac 연구를 위해서는 임신 주수 20-37주의 19명의 조기진통 환자에게 sulindac 200mg을 하루에 두 번 경구 투여하였다. 혈액시료는 약물투여 후 1.5, 4, 8, 12 시간 이후에 채혈되었다. Sulindac과 그 대사체들의 혈중농도는 HPLC 방법을 이용하여 분석되었다. 대사관련 효소들의 유전적 다형성이 sulindac의 약물동력에 미치는 영향을 평가하기 위하여 채집한 혈액으로부터 8 종류의 단일염기다형성 (SNP, single nucleotide polymorphism)을 분석하였다. 분석에 포함된 유전자들은 AOX1 (rs22293528, rs2248130), FMO3 (rs2075992, rs909530), MSRA (rs4841331, rs9329221, rs6981758, rs9644682) 이었다. 결과 Ritodrine 연구결과, 모집된 환자들의 평균나이는 31.42 ± 4.62 이었다. 일변량 분석 결과, ADRB2 rs1042717 G/A 의 유전적 다형성 분석결과 AA 유전자형을 가진 환자 군에 있어서 GG 혹은 GA 유전자형을 지닌 환자군 에 비해 더 짧은 시간 안에 분만 또는 치료실패에 도달하였다 (83.7 ± 81.3 hr, P = 0.014). 또한 rs1042719 G/C 유전자에 대하여는, GG 또는 GC유전자형을 가진 환자 군에 비해 CC 유전자형을 가진 환자 군에서 분만 또는 치료실패에 이르는 시간이 더 짧았다 (80.4 ± 88.2 hr, P = 0.028). 콕스의 비례 위험성 모델 분석결과, ADRB2 rs1042717 유전자에서 동형접합 변이형 유전자를 지닌 군이 이형접합 변이형 우전자를 지닌 군에 비해 더 빨리 분만 또는 치료실패에 이를 위험성이 100배로 나타났다. 이와는 반대로, ADRB2 rs1042718 유전자에서 이형접합 변이형 유전자를 지닌 군은 동형접합 변이형 유전자를 지닌 군에 비해 더 빨리 분만 또는 치료실패에 이를 위험성이 488배로 나타났다. ADRB2 rs1042713 유전자의 이형접합 변이형 유전자를 지닌 환자들은 24시간, 48시간, 7일 내에 성공적으로 치료된 확률이 GG 또는 AA 유전형을 지닌 환자들에 비해 통계적으로 높았다. 다중선형회귀분석 결과, ADRB2 rs1042718와 내원 당시 분만 주수, ADRB2 rs1042719 가 신생아의 태아몸무게에 유의적인 영향을 미치는 것으로 나타났다. 또한 내원 당시 분만 주수, ADRB2 rs1042717, and ADRB2 rs1042718 이 분만 당시 분만 주수에 유의적인 영향을 미치는 것으로 나타났다. Sulindac 연구에 있어서 환자의 평균 나이는 32.05 ± 3.79 이었다. Sulindac, sulindac sulfone, sulindac sulfide와 내부표준물질의 유지시간은 각각 4.1분, 5.4분, 12.2분, 6.6분 이었으며, 이 분석조건에서 네 물질 모두 다른 혈장 성분들로부터 잘 분리되었다. Sulindac, sulindac sulfone, sulindac sulfide 의 표준 검량선은 각각 0.5 - 10 ㎍/mL, 0.3 - 5 ㎍/mL, 0.3 - 5 ㎍/mL 농도 범위에서 직선성을 나타내었다. 이 분석법은 특이성, 직선성, 정확성, 정밀성, 감도 측면에서 모든 기준에 적합함을 검증하였다. 단변량 분석결과 sulindac/sulindac sulfone의 평균 AUCall 값이 FMO3 rs909530 유전자와 MSRA rs9644682 유전자의 유전적 다형성과 매우 유의한 결과를 나타내었다. (P = 0.036, P = 0.016). 결론 조기진통환자에서 ADRB2 의 유전적 다형성이 ritodrine의 약물동태에 유의적인 영향을 미치며, sulindac의 대사효소인 FMO3 와 MSRA 의 유전적 다형성이 sulindac의 약물동력에 영향을 주는 인자임을 확인 할 수 있었다. 이 연구의 결과는 ritodrine과 sulindac의 효과와 부작용을 예측하고 더 나아가 조기진통환자의 환자맞춤형치료 개발에 기여 할 수 있을 것이다.
