dc.description.abstract목 적: 무세포 인간 진피는 성형 재건 외과 분야에서 매우 유용한 재료이나, 시간이 지남에 따라 흡수되어 부피가 줄어드는 단점이 있다. basic fibroblast growth factor (bFGF) 는 콜라겐 합성과 혈관신생을 촉진시키는 물질로, 본 연구에서는 bFGF를 이용하여 무세포인간진피 (SureDerm??)의 이식 후 혈관신생을 증대시켜 흡수율을 낮출 수 있는지에 대해 연구하였다. 방 법: 40마리의 흰쥐를 bFGF군과 대조군으로 각각 20마리씩 두 그룹으로 나누었다. 흰쥐의 등에 두 개의 피하 터널을 만든 후, 2mm 두께의 SureDerm?? 을 15 × 5mm 크기로 잘라 각각의 터널에 이식하였다. bFGF군은 이식 전 이식편에 bFGF 용액을 분무하였고, 이식 후에는 bFGF 용액을 2주 동안 3일마다 이식 부위에 주사하였다. 대조군은 bFGF 용액 대신 phosphate-buffered saline(PBS)을 사용하였다. 4일, 1, 4, 12주 후 이식편을 꺼내, 육안 및 현미경적 검사를 통해, 염증 정도, 이식편의 두께, 신생 콜라겐 밀도, 신생혈관 수를 측정하여 비교하였다. 결 과: bFGF군은 높은 신생혈관 및 신생콜라겐 밀도, 더 빠른 염증세포의 침착을 보였다. 이식 12주 후, 이식편의 두께는 이식 전 두께와 비교하여 대조군은 75.15 ± 4.80%, bFGF군은 81.79 ± 5.72% 로 유의하게 bFGF 군의 흡수가 적었다(p<0.05). 결 론: bFGF는 혈관신생을 증가시키고 생착 과정을 촉진시켜, 무세포인간진피 이식의 흡수를 줄이는 데 효과적이다. 따라서, bFGF는 무세포 인간 진피 이식 시행 시, 유용한 첨가물이 될 수 있을 것으로 사료된다. ;Purpose: Acellular human dermis is very useful implant for use in plastic and reconstructive surgery. However, the volume of acellular human dermis graft is known to decrease for a long time. Basic fibroblast growth factor(bFGF) is a polypeptide that enhances the collagen synthesis and angiogenesis. In the current study we determined whether bFGF could improve the survival of acellular human dermis(SureDerm) by increasing angiogenesis of the graft. Methods: Forty rats were divided into two groups(control and bFGF). A 2-mm thick piece of SureDerm was cut into smaller pieces that were 15 × 5 mm in size. Two subcutaneous pockets were made on the back of each rat. Grafts sprayed with bFGF were implanted in the bFGF group and injected with bFGF after transplantation every 3 days for 2 weeks. In the control group, the grafts were treated with phosphate-buffered saline(PBS) instead of bFGF. Four days, and 1, 4, and 12 weeks after implantation, the grafts were harvested and gross and histologic examinations were performed. Inflammation grade, graft thickness, neocollagen density, and neocapillary count were measured. Results: The bFGF group displayed more rapid accumulation of inflammatory cells with a higher density of neocapillaries, and increased active collagen synthesis. After 12 weeks, the thickness of the grafts in the control and bFGF groups was 75.15 ± 4.80% and 81.79 ± 5.72%, respectively, in comparison to the thickness before transplantation. There was a statistically significant difference between both groups(p<0.05). Conclusion: bFGF was effective in reducing the absorption of acellular human dermal grafts by increasing angiogenesis and accelerating engraftment. In conclusion, bFGF may be a good tool for use in acellular human dermal graft transplantation for reconstructive surgery involving soft-tissue defects.
