감각 증상으로만 발현한 척수염의 임상 양상 분석

Abstract

Background : In a clinical field, myelitis with only sensory deficit is relatively rare but definitely exist. This, so called, “sensory myelitis” is not recognized as a single disease entity before. Commonly, sensory deficit and neuropathic pain of sensory myelitis lasts considerably long, although it isn’t devastating. Objective : The aim of this study is to investigate the clinical charactericstics of the myelitis which only presented sensory symptom. Methods : We report clinical, electrophysiologic and radiologic characteristics of 13 cases with sensory myelitis. Results : Most of them were men and showed subacute to chronic courses of asymmetric sensory symptoms with ascending patterns. The lesions on MRI were 1 to 4 segments and invaded usually central part of the cords with subtle enhancement. Pleocytosis of cerebrospinal fluid was not observed. Conclusion : This study might propose a foothold and the direction of future studies. Clinical and immunological studies of these unique type of myelitis may provide possible clues for the understanding of each immulogical charicteristics of sensory and motor pathways in the spinal cord.;임상에서 감각 증상만을 보이는 척수염을 가진 환자는 흔하게 접할 수 는 없으나 분명히 존재해왔다. 이 논문의 목적은 감각 증상만으로 발현한 척수염 환자들에 대한 임상 양상을 분석하여 이 질환이 일반적 척수염과 다른 임상적 특징이 있는지를 알아보기 위한 것이다. 우리는 13명의 감각 척수염 환자들의 임상 양상, 전기 생리검사 결과, 그리고 영상의학적 특성을 분석하였다. 이러한 환자들은 대부분 아급성이나 만성의 경과를 보였고 초기에는 상행하는 감각 증상을 특징으로 하였다. 척수MRI 촬영에서의 병변은 한 분절에서 네 분절까지 다양하였고 주로 척수의 중앙 부위를 침범하였으나 광범위한 경우도 많았고 조영 증강이 되는 경우가 많았다. 내원 초기 시행한 뇌척수액 검사에서 백혈구 증가는 없는 경향을 보였다. 본 연구에서 의도하는 감각척수염에 대한 인지와 임상양상에 대한 분석은 감각증상만을 호소하는 환자의 병소가 말초신경에만 국한되는 것이 아니라 중추신경 특히 척수에 있을 수 있다는 사실을 주지하기 위한 것이다. 이를 통하여 감각척수염 환자들의 진단과 치료가 보다 신속히 이뤄질 수 있는 계기가 되기를 바란다.
