당뇨병환자의 당뇨검사와 Insulin 치료에 대한 간호원의 지식 및 실천에 관한 조사 연구

Abstract

It is essential for diabetics to try to control their disease throughout their lives by continuous changing life-style Thus it is natural that nurses taking care of them should make efforts to half their patient to adapt themselves to changing environments. However without sufficient knowledge of disease, such effort will be fruitless. The Purpose of Study lies in investigating nursing activities in terms of Glycosuria test and Insulin therapy, and clarifying the correlations between the both nursing activities by the subject's characteristics, and confirming whether there exist any significant difference between the subject's characteristics and their nursing activities for diabetics. This study will be contribute to improving nursing for diabetics by providing new basic datas which can usefully utilized in inservice education. The subjects for this study were 108 nurses at 6 general hospitals in Seoul. Data was collected from August 13. to September 31. 1980, using the questionnaire method and analyzed by the use of mean, standard deviation, coefficient of correlations, analysis of variance. The results obtained are as follows: 1. The subjects answered only 59.5% of the total nursing activities for diabetics correctly. In the both nursing activities, only 50.0%f nursing activities in terms of Glycosuria test was answered correctly, while 70.0% of nursing activities in terms of Insulin therapy. 2. As for the correlation between the both nursing activities by subject's characteristics, there are significant positive correlation in four variable such as nurse's clinical experience ("above 4-year"), nurse's teaching experience for diabetics("regularly", "occasionally"), the number of hospitalized diabetics("4-5", "above 6"), the route in acquiring the knowledge of diabetes ("periodicals", "self-education", "in school"). 3. As for the relation between the subject's characteristics and their nursing activities for diabetics, there are only significant difference in the route in acquiring the knowledge of diabetes(P＜0.01).;당뇨병환자는 일상동안 생활(life-Style)을 변화시면서 질병을 조절하는 것이 필요하다. 따라서 간호원은 그들의 환경에 대한 적응을 위해 환자교육을 중요시해야 한다. 효과적인 환자교육을 위해서는 간호원의 당뇨병에 대한 지식과 실천이 선행되어야 할 것이다. 본 연구의 목적은 간호원의 당뇨검사와 Insulin치료에 대한 간호활동을 조사하고, 간호원의 일반적 특성과 당뇨병환자에 대한 전체 간호활동사이에 유의한 차이가 있는지 알아 봄으로서, 간호원 실무 교육에 기초자료를 제공하고 효과적인 당뇨병환자간호를 하기 위함이다. 본 연구는 1980년 8월13일부터 9월31일에 걸쳐 실시 되었으며, 서울 시내에 있는 6개 종합벼원의 내과병실에 근무하는 간호원 108명을 대상으로 조사하였다. 본 연구도구는 Etzwiler, Beaser등이 사용한 질문지를 기초로 하여 본 연구자가 작성하였고, 질문내용은 당뇨검사에 대한 간호활동 16문항과 Insulin치료에 대한 간호활동 17문항으로 구성되었다. 조사된 문항은 점수로 산출하였으며 사용된 분석방법은 평균 표준편차 r-검증 F-검증이었다. 본 연구의 주요 결과는 다음과 같다. 1. 연구대상 간호원의 당뇨병환자에 대한 전체 간호활동에서 59.5%가 올바르게 활동하고 있었고 간호활동 중에서 당뇨검사에 대한 활동 50.0%, Insulin치료에 대한 활동은 70.0%가 올바르게 활동하고 있었다. 2. 연구대상 간호원의 일반적 특성에 따른 간호활동간의 상관관계에서, 임상경력별로 "4년이상"의 집단에서만 양의 상관이 있었고(r=0.6617), 당뇨병환자 교육경험별로는 "정기적으로 했다"는 집단(r=0.5623)과 "경우에 따라 했다"는 집단에서 양의 상관이 이었다(r=0.2340) 근무하는 병실의 당뇨병환자 입원수별로는 입원환자수 "4-5명"(r=0.3726)과 "6명이상"에서 양의 상관이 있었고(r=0.2576), 당뇨병에 대한 지식습등 경로별로는 "의료정기간행물을 통해서" 얻은 집단(r=0.4496)과 "학교에서 배웠다"는 집단(r=0.3508) "스스로 책을 보고 공부했다"는 집단에서 양의 상관이 있었다(r=0.2753) 3. 연구대상 간호원의 일반적 특성과 당뇨병환자에 대한 전체 간호활동의 관계에서, 당뇨병에 대한 지식습득경로별에서만 유의한 차이가 있어다.(p＜0.01)
