Abstract
The purpose of this study was to contribute to the future improvement of children's nutrition by means of evaluating the effects of nutrition education to mothers and of feeding children in the two groups, one who participated nutrition education and supplementary feeding, and another who unparticipated. 1. One group was those of 30 day care centers (unban area 15 and rural 15), randomly selected nation-wide, who were given nutrition education and supplementary feeding, and another was those of 15 day care centers (urban 7 and rural 8). 2. The nutrition education was given to mothers of 4 to 6 years age of children in 10 to 11 occasions annually for the period of 1 to 2 years. 3. The food, consisting of daily ration of 150 grams of flour, 70 grams of corn-soy blend and 25 grams of vegetable oil, was supplied to those children in 6 meals a week as lunch and sidedish for the period of 1 to 2 years. 4. Indicators in this study: a. Status of recipients (1) Schooling records of mothers (2) Occupations of mothers (3) Occupations of fathers b. Evaluation of the effect of nutrition education to mothers (1) Mothers' knowledge on nutrition (2) Practices of the knowledge (3) Changes in the children's physical conditions c. Evaluation of the effect of feeding services (1) Surveillance in taking nutrients (2) Measuring of children's physical standards 5. This study was based on feeding practices at Day Care Center for the period of June 1 to June 30, 1975 and was to figure out the nutrition value and amount taken by children for 5 days. The measuring of children's height and wight was made for the period of November 1 to November 15, 1975 and the statistics were made in comparisons by indicators such as participants, non-participants, urban and rural for the average standard values. The questionnaires were sent to all the parents and non-participants, and responses were received for the period of November 1 to December 20, 1975. Comparisons were made by giving 4 points in evaluating each criterion. A. Effect of nutrition education: The scores in nutrition education to mothers were participants-3.59 versus non-participants-2.70 and the evaluation ratings were "Excellent" versus "good". Thus, the considerable attainment of the knowledge by participants was found from this result. On the other hand, the evaluation ratio of nutrition education appeared to be urban-3.45 versus rural-3.39 and the ratings were "Very good" versus "Very good". But remarkable difference between them was found. 1. Evaluation of the knowledge on nutrition a. The scores in the knowledge on feeding period of mother's milk was participants-3.52 versus non-participants-2.79 and the ratings were "Excellent" versus "Good". b. The scores in the knowledge on weaning method were participation centers-3.71 versus non-participating centers-3.19 and the ratings were "Excellent" versus "Very good". c. The scores in the knowledge on nutritional food were participating centers-3.71 versus non-participating centers-2.77 and the ratings were "Excellent" versus "Good". d. The scores in the roles of nutrition were participating centers-3.57 versus non-participating centers-2.21 and the ratings were "Excellent" versus "Fair". e. The scores in the knowledge on food for patients and pregnant women were participating centers-3.46 versus non-participating centers-2.52 and the ratings were "Very good" versus "Good". From the above scores we found the facts that the participants were still poor in the knowledge on patients and pregnant women and that non-participants responded not at all "Excellent" to 5 questions, to be poor, on the nutrition knowledge much more poor on the roles of nutrients. Thus, we found the remarkable difference between ones given nutrition education and the others not given. 2. Criteria in the practice of the knowledge on nutrition a. The ratio in feeding children under 1 year age mother's milk or cow's milk supplemented with some other nutrients was participants-88% versus non-participants-56%. b. The ratio in feeding children under 6 months to 1 year age the milk and supplements were participants-81% versus non-participants-61%. c. The ratio in feeding children the sidedish "Very often" and "Often" was participants-80% versus non-participants-61%. d. The ratio in feeding children the balanced food "Always" and "Often" was participants-80% versus non-participants-60%. from the above ratios we found the fact that the participants responded "Excellent" or "Very good" and the non-participants "Good" in 4 questions on the nutritional practices. 3. Changes in physical conditions The participants responded "Very healthy" being better than the non-participants who responded "Healthy". a. The ratio in becoming healthier after participated was participating centers-89% versus non-participating centers-53%. b. The responses on the present physical conditions were "Very healthy" and "Healthy", and the ratio was participants-89% versus non-participants-69%. c. The responses on how often the child has caught bad cold during the last 2 years were "None" and "Seldom", and the ratio was participating centers-82% versus non-participating centers-51%. d. The responses on how often the child has had stomach trouble during the last 2 years were "None" and "Seldom", and the ratio was participants-86% versus non-participants-56%. From the above ratio shown we found the fact that the physical conditions of participants were better than non-participants. e. The ratio in experiencing serious diseases of children was participants-9% versus non-participants-22.6%, and from this ratio we found healthier participants according to the statistics made for the last 2 years. B. Effect of feeding children: Considering that lunch and sidedish for children are equivalent to one of 3 meals a day, we feed the standard amount of 500 Kilo Calorie the children of 4 to 5 years age, of which protein 15 grams and animal protein 5 grams were contained. 1. Status of feeding nutrients a. Comparisons of the nutrients taken by children as lunch and sidedish were as follows: Calorie: participants-669 versus non-participants-463. Both were highly significant. Protein: participants-23.8 grams versus non-participants-15.3 grams, both satisfactory to the standard. Both were highly significant. Animal protein: participants-4.5 grams versus non-participants-3.9 grams, both under the standard whereas this is one of most essential nutrients for children's growth. Both were not significant. Carbohydrate: participants-107.5 grams versus non-participants-82.2 grams, both were more than 60% of Total Calories. Both were significant. Fat: participants-18.6 grams versus non-participants-8.2 grams less than half of participants'. Both were highly significant. From the above figures shown we found the participants feed much more than the non-participants. b. Comparisons of the nutrients taken by urban and rural participants were as follows; both were not significant. Calorie: urban-693 grams versus rural-644 grams (The standard: 500 grams) Protein: urban-24.9 grams versus rural-22.8 grams (Both satisfactory) Animal protein: urban-4.8 grams versus rural-4.2 grams (Both under the standard) Carbohydrate: urban-112.8 grams versus rural-102.3 grams Fat: urban-18.1 grams versus rural-19.1 grams 2. Comparisons of physical standards of children a. In order to evaluate the effect of feeding children, we measured 1900 children's physical standards and found the participants were better than the non-participants. Height: participating centers-105.88cm versus non-participating-104.10cm. Both were highly significant. (The standard: 105cm) Weight: participating centers-17.40 Kg versus non-participating-17.04 Kg. Participating centers and non-participating centers were significant. (Both satisfactory) Chest circumference: participating centers-53.9cm versus non-participating-51.66cm. Both were highly significant. (Participants 2.24cm bigger) b. Comparisons of physical standards by urban and rural participants were as follows: Height: urban 108.21cm versus rural 104.25cm Weight: urban 18.23 Kg versus rural 16.82 Kg Chest circumference: urban 53.38cm versus rural 54.42cm. (Rural's 1.04cm bigger);본 연구는 어머니에 대한 영양교육을 하고 아동들에게는 보충급식을 제공한 집단과 그렇지 못한 집단을 비교하여 어머니에 대한 영양 교육의 효과와 아동들에 대한 급식의 효과를 측정하여 앞으로의 국가의 영양개선 사업에 도움이 되고져 하였다. 1. 조사대상은 어린이집에 나오는 4~6세 아동으로 전국에 있는 어린이 집중 보충급식을 제공하고 그 어머니에게는 영양교육을 실시하고 있는 어린이집 중에서 30개소를 추출하여 참고집단이라 칭하고 그렇지 못한 어린이집을 15개소를 추출하여 비 참여집단이라 칭하였다. 2. 영양교육의 내용은 <부록1>에 수록 되었고 횟수는 10~11회에 걸쳐서 교육을 실시했으며 기간은 1년~2년간이었다. 3. 급식의 내용은 참여집단은 점심 이외에 1일 밀가루 150g, 식물성유 25g, 혼합분 70g을 제공 받아서 아동들에게 보충식을 급식시켰고 비 참여집단은 보충식의 제공을 받지 못하고 단지 점심만을 급식시켰다. 기간은 1년~2년간이었다. 4. 조사내용과 분석방법은 다음과 같다. 가. 조사대상자의 일반상황조사 나. 영양교육의 효과 측정 (1) 어머니의 영양지식 (2) 영양지식의 실천도 (3) 아동의 현재 건강상태 변동 이상의 내용을 질문지를 이용하였고 각 변수의 평점측정 기준을 4점 간격 척도로, 최상은 4점 최하는 1점으로 하여 통계 하였고, T Test에 의한 유의성 검증을 하였다. 다. 급식의 효과 측정 (1) 영양섭취 실태는 실제로 아동이 먹는 점심과 간식의 양을 통계하여 참여별, 지역별, 기준량과의 비교와 측정하여 영양가를 산출하여, 평균, T Test 검증을 하였다. (2) 체위의 측정은 평균 통계 처리하여 참여별, 지역별, 기준치와의 차이를 비교하고 T Test에 의한 유의성 검증을 하였다. 이상의 결과를 요약하면 다음과 같다. 1. 어머니에 대한 영양교육의 효과 가. 어머니에 대한 영양지식의 평가 수유기간에 대한 지식, 이유방법에 대한 지식, 영양과 식품에 대한 지식, 영양소의 역할에 대한 지식에서, 참여집단은 “가장 좋다”로 척도가 나왔다. 그러나 환자와 임산부를 위한 지식에서 약간 빈약했다. 비 참여집단은 위의 다섯 가지 지표 모두 빈약한 상태의 척도가 나왔다. 참여집단과 비 참여집단 간에는 고도로 유의성이 있었다. 나. 영양지식의 실천도 1세 미만의 아기에게 급식한 식품, 젖 이외에 다른 식품을 먹이기 시작했을 때의 아기의 연령, 자녀에게 만들어 주는 간식의 횟수, 자녀에게 균형식을 급식시키는 빈도에서 참여집단은 “매우 좋다”에서 “가장 좋다”로 척도가 나왔다. 그러나 비 참여집단은 “좋다”로 척도가 나와서 참여집단보다 낮은 실천도를 보였다. 참여집단과 비 참여집단 간에는 고도로 유의적인 차이가 있었다. 다. 신체 건강상태의 변동 아동이 참여 후 더 건강해진 과정, 아동의 현재 건강상태, 아동들의 감기감영 빈도, 아동들의 배탈이 나는 빈도에서 참여집단은 “좋다”에서 “매우 좋다”로 척도가 나왔다. 그러나 비 참여집단은 “보통이다” 또는 “좋다”는 척도가 나왔다. 신체의 건강상태 변동에도 참여집단이 훨씬 우수했다. 참여집단과 비 참여집단 간에는 고도로 유의적인 차이가 있었다. 2. 아동에 대한 급식의 효과 어린이집에서의 점심과 간식은 1일 3식 중에 한 끼에 해당한다. 4~6세 아동의 한국인 영양권장량(14)은 1,500 칼로리, 단백질 45g 이다. 이 중에 1/3인 500 칼로리와 15g의 단백질과 5g의 동물성 단백질을 기준량으로 하였다. 가. 영양소의 섭취 실태 참여집단과 비 참여집단의 영양소 섭취실태를 비교하면 칼로리의 섭취비는 669 : 463 칼로리로 참여집단의 섭취량은 충분하였다. 참여집단과 비 참여집단 간에는 고도로 유의적인 차이가 있었다. 단백질의 섭취비는 23.8g : 15.3g 으로 참여집단과 비 참여집단 간에 고도로 유의적인 차이가 있었다. 기준량에 비해서는 양 집단이 만족된 상태였다. 동물성 단백질의 섭취비는 4.5g : 3.9g 이었고 성장기 아동의 발육을 위하여 섭취량을 높이도록 노력하여야 하겠으며 참여집단과 비 참여집단 간에 유의성은 없었다. 탄수화물의 섭취비는 107.5g : 82.2g 이었고 참여집단과 비 참여집단 간에 유의성은 없었다. 참여집단의 섭취율은 충분하였다. 지방의 섭취비는 18.6g : 8.2g 이었고 참여집단과 비 참여집단은 고도로 유의적인 차이가 있었다. 따라서 칼로리 단백질 지방질의 섭취비는 참여집단이 비 참여집단보다 월등히 높음을 볼 수 있었다. 나. 아동의 체위 비교 급식의 비교측정을 위해서 아동 1,900명의 신장 체중 흉위를 측정하여 참여집단과 비 참여집단을 비교하였다. 신장의 참여집단과 비 참여집단의 비는 105.88cm :104.10cm 이었고 양 집단은 한국인 영양권장량의 체위 기준치 105cm에 비해서 만족된 상태였으며 참여집단과 비 참여집단 간은 고도로 유의성이 있었다. 체중의 참여집단과 비 참여집단의 비는 17.40 Kg : 17.04 Kg 으로서 양 집단이 기준치에 비해서 만족된 상태였으나 통계적으로 참여집단과 비 참여집단 간에는 유의적인 차이가 있었다. 흉위의 평균비는 53.90cm : 51.66cm 로서 참여집단이 비 참여집단보다 2.24cm가 더 컸다. 참여집단과 비 참여집단 간에는 고도로 유의성이 있었다. 3. 이상의 결과로 보아서 어머니에 대한 영양교육의 효과와 급식의 효과면에서 참여집단이 비 참여집단보다 월등히 우수하였음을 알 수 있었다. 따라서 어머니에 대한 영양교육과 보충식이 아동의 건강에 미치는 영향은 매우 크다는 것을 알 수 있었다.
