dc.description.abstractThe major objective of the present study was to investigate the pharmacokinetic characteristics of levodopa and carbidopa when administered by the alternative administration routes such as nasal and transdermal delivery. The concentrations of levodopa and carbidopa in plasma and brain were determined using high-performance liquid chromatography (HPLC) and the data were subjected to non-compartmental kinetic analyses. For the determination of levodopa and carbidopa in rat plasma and brain, to blood and brain samples were added internal standard (I.S.), methyldopa, and perchlorate to precipitate of protein, and then the samples were centrifuged. The supernatant was added to potassium citrate buffer to neutralize perchlorate, and the sample solution was injected onto a C18 column. The levodopa, carbidopa and I.S. were eluted around 4, 11-12 and 8 min, respectively. This method was validated with respect to linearity, specificity, precision and accuracy. Substantial differences in the resultant kinetic data were obtained by comparing the plasma and brain concentration profiles by each administration route. When the levodopa (80 mg/kg) was coadministered with carbidopa (20 mg/kg) orally, a short half-life was observed. It was found that levodopa and carbidopa after nasal administration produced a high peak in brain as well as plasma and the higher systemic bioavailabilities (80.4%) than after oral administration (19.8%). However the elimination rate constant by nasal route was significantly faster than that by oral route (P < 0.05). The transdermal iontophoretic administration of levodopa and carbidopa revealed sustained concentration in the plasma although the absolute bioavailability with 0.4 and 0.6 mA/㎠ was very low which were 4.96% and 6.61%, respectively. In brain, the nasal route produced a faster and higher peak of levodopa, and the iontophoresis route led to the gradually increasing peak of levodopa. These results suggested that intranasal and transdermal iontophoresis therapy may be effective in the majority of patients with dysphasia in emergency situation, where an oral or intravenous administration is not feasible. In conclusion, this study showed the feasibility of new levodopa delivery systems such as nasal or transdermal delivery.;Levodopa는 Parkinson’s diseae의 임상적으로 가장 흔히 사용되는 약물로서 decarboxylase에 의하여 dopamine으로 분해되어 효능을 나타내는 약물이다. Levodopa는 dopamine이 통과하지 못하는 뇌혈관관문을 통과하는 장점이 있지만, 경구 투여 시 위배출시간에 따라 개체내 및 개체간 변동이 심하며 흡수 부위가 십이지장으로 제한되어 있고, 위장관 통과시 말초에서 decarboxylas에 의하여 대부분이 분해되어 뇌에 도달하는 levodopa의 양은 투여량의 1% 정도가 되는 것으로 알려져 있다. 또한 위장관 통과시 대사되어 생긴 dopamine으로 인한 현기증, 구역, 부정맥 등 여러 가지 부작용이 나타나게 되며 levodopa를 장기간 사용하는 경우 뇌중 dopamine 농도의 변동이 심해지고 운동성이 악화되는 등의 문제점이 나타난다. 이처럼 경구 투여 시 발생하는 문제점이 많은 약물임에도 불구하고 대체 경로에 대한 연구는 거의 없는 실정이다. 본 연구에서는 levodopa의 생체이용률을 높이기 위해 새로운 투여 경로로서 비강과 피부를 통한 약물 송달의 약물동태학적 특징을 검토하고자 하였다. 쥐의 혈장과 뇌에서 levodopa를 정량적으로 검출하기 위하여 기존에 보고된 HPLC 분석법을 참고하여 분석법을 개발 및 검증하였다. 쥐 혈장과 뇌으로부터 levodopa 및 carbidopa와 내부표준물질인 metyldopa을 추출하기 위하여 단백질을 perchloric acid를 사용하여 침전시키고 원심분리로 상층액을 분리하여 potassium citrate buffer로 중화시켰다. 이 시료를 Gemini 5u C18 컬럼에 주입하여 high pressure liquid chromatography (HPLC)로 검출하였다. Levodopa 및 carbidopa와 내부표준 물질은 쥐의 혈장에서 각각 4.3분 과 11.0-12.5 분, 8.0분에 검출되었다. 각각의 검량선은 혈장과 뇌에서 양호한 직선성을 나타내었으며 이 분석법은 충분한 특이성과 정확성 및 정밀성을 가지고 있었다. 비경구제제의 약동학과 생체이용률을 평가하기 위한 대조군으로서 levodopa와 carbidopa의 정맥 및 경구투여를 실시하였다. Levodopa와 carbidopa의 비경구제제를 투여하였을 때 경구 투여하였을 때와 비교하여 흡수와 배설 부분에서 상당한 차이를 보였다. Levodopa (80 mg/kg) 를 carbidopa (20 mg/kg) 와 병용 투여하였을 때 levodopa와 carbidopa의 최고 혈중농도(Cmax)와 혈장 중 약물농도-시간곡선 하 면적(AUC)은 문헌에 보고된 바와 마찬가지로 짧은 반감기가 관찰되었다. 대조적으로 경비흡수제제 (2.5 mg/kg levodopa 와 0.64 mg/kg carbidopa) 에서는 적은 용량에도 불구하고 경구투여 시와 유사한 Cmax값과 상대적으로 더 높은 생체이용률을 확인할 수 있었다 (80.4%). 하지만 혈중 및 뇌중에서 약물의 농도가 경구 제제보다 더 빠른 속도로 소실되었다 (P < 0.05). 반면 iontohoresis 경피흡수제제 (97 mg/kg levodopa 와 32.5 mg/kg carbidopa)를 적용하였을 때는 경구 투여하였을 때보다 느린 속도로 흡수되어 천천히 소실되었다. 경피흡수제제를 사용하였을 때 levodopa와 carbidopa의 소실속도는 감소하고 개체 변동성은 감소한 것으로 관찰되었으나 생체이용률은 경구 투여 시보다 감소한 것으로 나타났다 (iontophoresis 전류의 세기에 따라 0.4 mA/㎠ 일 때 4.96%, 0.6 mA/㎠ 일 때 6.61%). 이 결과들을 바탕으로 쥐의 혈장 및 뇌 중 levodopa와 carbidopa의 HPLC 분석법은 성공적으로 확립되었음을 확인하였고, 제제 조성 및 구성의 조절이 필요하나, 본 연구에서 제시된 비경구제제는 각각의 약동학적 특성 따라 정맥 또는 경구 투여가 불가능한 환자를 대상으로 임상 현장에 실용화될 가능성이 있는 것으로 판단되었다.-
