癌患者 家族의 問題와 그에 對한 醫療 社會事業的 接近

Abstract

우리나라 國民의 成人病 가운데 癌疾患은 漸次的으로 그 數가 增加하고 있다. 本 硏究者는 癌患者의 治療에 도움이 되기 爲한 社會事業的 方法의 一環으로 癌患者가 屬한 家族의 社會心理的 問題를 알아보고 그 解決을 爲한 對策을 模索하였다. 먼저 患者의 心理的 特性과 家族들에게 미치는 影響을 文獻에 依해 調査했다. 그 結果 患者는 診斷 初부터 治療 終了까지 複雜한 心理的 葛藤 속에 있는 것을 알았다. 그리고 家族들은 그에 따른 影響을 받고 있다. 또한 實際로 患者로 因해 어떤 反應과 變化를 보이고 있으며, 患者로 因해 생긴 問題를 어떻게 處理하고 있는 가를 알아보기 爲해 質問 調査를 가졌다. 調査 對象者는 廷世 醫療院 癌 센타에 다니는 外來 患者의 家族으로 하였고, 그들이 어떤 問題를 어떻게 處理하고 있는 가에 對한 內容을 中心으로 하여 50事例를 調査했다. 調査 結果 다음과 같은 것을 알 수 있었다. 첫째, 患者로 因해 家族들은 精神的으로 疲勞한 狀態에 있다. 그들은 病에 對한 知識 不足과 癌에 對한 그들 自信의 衝擊的 反應으로 患者를 對하는데 心理的 어려움을 갖고 있다. 그리고 患者로 因해 家族에게는 役割의 變化와 家庭 雰圍氣의 變化를 가져왔다. 둘째, 家族들은 그들이 患者로 因해 생긴 心理的 問題를 病院에 와서 解決하고 싶어하나 그들이 만날 수 있는 對象은 醫師에 限定되어 있으며 醫療的 知識, 經驗에 對해서만 局限되어 滿足을 얻고 있으며, 實際로 當面한 心理的 어려움은 滿足스럽게 解決을 얻지 못하고 있다. 이러한 問題 發見에 對해 다음과 같은 解決 方法을 模索 하였다. 첫째, 治療者가 患者와 家族에 對한 知識을 가지고 治療에 臨 할 수 있도록 하기 爲한 制度的인 面의 提案으로써, ① 患者의 心理社會的 評價가 包含된 醫務記錄을 患者의 記錄紙에 添附하여야 한다. ② 醫療的인 面에 對한 知識과 相談 能力을 갖춘 專門要員을 둠으로써 患者와 家族에게 心理的 서비스를 提供하게 한다. 둘째, 解決 方法의 理論的 適用의 提案으로 診斷 初부터 患者와 家族에게 介入하여 그들이 心理的 均衡을 잃지 않고도 危機狀況을 잘 適應해 나갈 수 있도록 하기 爲해 危機 介入理論을 適用시킨 接近 方法을 提示 하였고, 家族을 대상으로 일하는 方法의 一環으로 家族治療理論을 應用하였다.;Among adult illnesses, Cancer and Cancer-related diseases are gradually increasing in number in Korea. In order to provide better assistance for the cancer patient, this thesis is an attempt to find out solutions to their emotional and family problems using social work methods. This study was first intended to clarify the psychological nature of the cancer patient and his family a review of the literature. This study showed that the patient is faced with many psychological problems, and that these problems gave an effect on the patient's family. In addition, a questionnaire survey was carried out to discover the practical effects on the patient's family, the response of the family, and how changes in the roles and structure of the family occurred. Individuals participating in the research were limited to fifty persons comprised of the family members of out-patients of Yonsei Cancer Center, Seoul, Korea. In this research the findings are as follows: First, a patient I s family members are emotionally exhausted because of the patient. Because of lack of knowledge about cancer, and the shock caused by the disease, they have difficulty in dealing with the patient. As a consequence of having a cancer patient in the family, role changes among family members occur and the atmosphere of the home is changed. Secondly, the family of the patient wants to solve the urgent psychological problems caused by the patient, but they cannot do this problem satisfactorily because there are few doctors who can meet such needs due to limition of time. Families find they can only depend on doctors for medical knowledge and experience, not for psychological support. To meet these unmet needs, following a re recommended. First, patient's psychosocial evalution must be systematically inclued in the medical chart, so that the physician can treat the patient having sufficient information concerning the patient end his family situation. Second, to help the cancer patient's family to the crisis situation without psychological disturbance crisis intervention and family therapy approach are suggested, and the medical social worker who have medical knowledge and can do counseling are needed in Cancer Center as a resident medical team professional. Third, the health professional who related to cancer patient must be understand the emotional needs of patient and his family. To do this, medical team conference and seminar should be held regularly in hospital, and the medical social worker should be entrusted group program for the patients and their families for the unification of treatment.
