간질환자의 특성과 의료사회사업적 접근의 필요성에 대한 연구

Abstract

본 논문의 연구목적은 간질환자의 사회기능의 문제를 야기시키게 되는 문제들을 검토해 보고, 한국의 간질환자 현황을 파악하며, 간질환자의 사회심리적 문제들에 대한 해결방법을 연구하여 간질문제에 대한 사회사업적 의의를 찾는 데 있다. 본 연구의 주된 내용은 간질에 대한 의학적, 심리사회적 측면에서의 이해를 비롯하여, 장미회를 중심으로 간질환자의 실태 파악에 중점을 두었으며, 의료사회사업적 접근방법으로 개별사회사업과 집단사회사업, 지역사회조직사업을 사례 적용하여 간질환자의 사회심리적 문제해결 방법을 연구하였다. 본 연구를 통하여 얻어진 간질환자의 사회심리적 문제와 그에 대한 해결방법을 요약하면 아래와 같다. A. 간질환자들은 간질에 대한 그릇된 사회인식으로 인하여 여러 가지의 사회심리적 문제들을 갖고 있는데, 옛날부터 지속되어 온 간질에 대한 편견, 오해, 무지 등의 그릇된 사회인식은 간질환자의 사회 생활면에 사회적 낙인으로 작용되어 간질환자들은 사회에서 소외되거나 거부되고 있다. 따라서 간질환자의 직장, 결혼, 진학문제 등 장래문제에 심각한 장애가 있는 것으로 나타났다. 간질환자는 그의 심리적 적응에 문제가 있는데, 간질에 대한 그릇된 사회인식을 경험하고 내면화시키게 되며, 흔히 간질의 진단에 저항하거나 부정한다. 그리고 간질을 갖고 있는데 대한 심각한 불안이 있으며, 간질을 외부에 숨기려는 경향이 있다. B. 간질환자의 사회기능의 장애요인이 되는 사회심리적 문제에 대하여 의료사회사업적 접근방법으로서 개별사회사업과 집단사회사업, 지역사회조직사업을 사례 적용한 결과, 개별사회사업으로 간질환자의 자아의 강화를 시킴으로서 심리적 부적응 문제, 즉 간질에 대한 불안이나 긴장을 해소시키고, 간질의 진단에 대한 저항이나 부정의 문제를 해결할 수 있었다. 집단사회사업으로 간질을 숨기려는 경향이 많은 환자들에게 간질이 있음을 노출시키는 기회가 제공되어 사회적치료의 중요한 의미가 있었다. 특히 집단내에서 간질환자들은 자신의 문제들을 자연스럽게 표현하고, 문제의 완화 및 해결이 되었다. 지역사회조직사업으로 간질환자와 그 가족, 지역사회 등을 치료의 대상으로 포함하여 간질에 대한 그릇된 사회인식 및 태도의 변화를 가져오고 간질문제에 대한 관심과 참여를 높일 수 있었다. 본 연구논문의 결과에서 개별사회사업과 집단사회사업, 그리고 지역사회조직사업을 병행 활용한 의료사회업적 접근 방법으로서 간질환자의 사회심리적 문제의 해결은 물론, 그가 속해 있는 가정이나 지역사회의 안녕과 균형을 유지함으로서 간질환자의 문제에 대한 통합적 효과를 나타내었다. 따라서 본 연구자는 간질환자의 심리사회적 문제 해결에는 의료사회사업적 접근방법이 사용되어야만 한다고 보며, 이는 궁극적으로는 간질환자나 그 가정, 지역사회, 국가의 안녕과 복지에 공헌하는 것이 되리라고 본다. ;The purpose of this study is to seek for, 1) Psycho-social problems of epileptic patients, 2) Present situation of epilepsy in Korea, 3) To find solving method of the problems in the epilepic patient's life. The content of this study is to understand medical and psycho-social aspect of epilepsy, and to grasp of the situation of epileptic patients in Korea. And cases were studied in order to find solving method of their psycho-social problems by using case work, group work and community organization. This study summarized as follows, A: This study shows that the patients is faced with many social and psychological problems caused by public opinion concerning epilepsy. First, the longstanding discrimination about epiliepsy such as social prejudice,misunderstanding, ignorance influence their social life as a social stigma. Therefore, they have obstacles in planning the future such as getting a job, marriage, schooling. They are often isolated or rejected by their family and society. Second, epilepsy has a difficulity in adjusting their psychological environment. They experience social attitude about epilepsy and may come to internalized views which other people hold about them. Epilepsy tend to deny both fact and the significance diagnosis of epilepsy. They feel great deal of anxiety having epilepsy and have a tendency to hide their symptom. B. To solve the psycho-social problems of the epileptic patients, the cases were studied by using casework group work and community organization. Casework can help the patients to adjust their psychosocial environment. It helps to strengthen the ego, to solve resistance and denial about the diagnosis of epilepsy. Group work has the effect as a social treatment providing the opportunity to expose their symptom which they have a tendency to hide. But in the group, their problems are diminished and solved easily. Community organization had treated epilepic patients, his family and his community, it was able to change public opinion and attitude about epilepsy which cause the problems of epilepsy. Medical social work approach using casework,groupwork, community organization could solve not only psycho-social problems but also integrate the welfare of his family and community. As these results, the author believes that in order to solve the psycho social problems of epilepsy, medical social work approach must be used, it may come to be contribute well-being and welfare of epilepsy and his family and community and nation.
