백혈병 환자의 조혈모세포이식 후 재발체험에 관한 연구

Abstract

우리나라에서 백혈병으로 인한 사망자수는 1983년에 830명이었으나 2001년에 1,455명으로 해마다 증가하고 있는 상태이며(통계청, 2002), 백혈병은 연령이 증가함에 따라 발생빈도가 증가하기 때문에, 고령인구의 비율이 증가하는 추세에 따라 보다 중요한 건강문제로 대두되고 있다(박재갑, 박찬일, 김노경, 2003). 백혈병은 관해 유도를 위한 복합 항암화학요법을 시행하고, 관해(remission)가 오면 재발 방지를 위해 공고-강화요법을 1∼2차례 더 시행하며, 조혈모세포이식을 통하여 치유율을 높이고 있다. 급성 백혈병의 재발은 대부분 이식 후 3년 이내에 발생하게 되는데, 1차관해 상태에서 이식을 시행 받은 경우에는 재발율이 10∼30%인데 비하여, 이보다 진행된 상태에서 이식한 경우에는 재발율이 20∼70%정도 된다(이제환, 2002). 조혈모세포이식 후 완치에 대한 기대를 가지고 있던 대상자에게, 재발은 큰 충격으로 받아지고 있지만 지금까지 조혈모세포이식 후 재발한 환자에 대한 연구는 거의 없는 상태로, 이들에 관한 심층적 이해가 부족한 상태이다. 따라서 본 연구는 백혈병 환자의 조혈모세포이식 후 재발체험의 본질과 의미를 이해하고자 시행하게 되었다. 연구의 방법은 벤 매넌의 해석학적 현상학적 연구 방법을 적용하여 4단계를 통하여 접근하였다. 4단계의 연구 과정은 다음과 같다. : 체험의 본질에 집중, 실존적 탐구, 해석학적 현상학적 반성 및 글쓰기이다. 본 연구의 참여자는 서울시내 S의료원에서 조혈모세포이식 후 3년 이내에 재발한 백혈병 환자 5명으로, 남자 4명, 여자 1명이었다. 자료수집은 2003년 4월부터 8월까지, 지속적이고 심층적인 면담과 관찰을 통해 이루어졌고, 면담 시 개방적이고 반 구조화된 질문 형식을 사용하였다. 자료수집을 위해, 연구 참여자의 동의를 얻어 면담 내용은 모두 녹음하였으며 면담 즉시 녹음내용을 여러 번 반복하여 들으면서 참여자가 표현한 언어 그대로 필사하였다. 본 연구의 주제분석은 수집한 자료 속에서 의미있는 주제를 분류하고 의미를 밝히면서 본질적 주제를 이끌어냈으며, 이때 어원과 문학 및 예술작품 그리고 현상학적 문헌들을 참조하여 조혈모세포이식 후 재발한 백혈병 환자의 체험에 대한 통찰력과 이해를 높이고자 하였다. 참여자의 체험이 본질적인 주제로 '온몸으로 거부하고 잊고 싶은 참담한 심적 고통', '어쩔 수 없이 현실을 받아들여야 하는 운명', '죽음을 선택하고 싶을 만큼의 극심한 육체적 고통', '또 한 번 절망으로 내몰리게 한 경제적 고통', '계속되는 격리가 가져온 외로움', '기적처럼 살고 싶은 마지막 삶에 대한 기대'를 확인하였다. 본 연구의 간호학적 의의는 다음과 같다. 간호 실무의 측면에서는 조혈모세포이식 후 재발한 백혈병 환자를 보다 넓은 이해의 폭을 가지고 접근할 수 있을 것이고, 신체적, 정신적 측면을 포함하여 간호중재를 포괄적으로 수행할 수 있을 것이다. 간호 연구의 측면에서는 지금까지 재발한 환자에 대한 관심과 연구가 없었기 때문에, 본 연구가 재발한 환자에 대한 연구를 하기 위한 계기가 될 것이라 생각한다. 간호 교육의 측면에서는 조혈모세포이식 후 재발한 백혈병 환자의 체험의 본질과 의미를 바탕으로 실제적인 임상 교육에 근거로 사용될 수 있을 것이라 생각한다. 더 나아가 백혈병 환자 이외에 다른 암환자의 재발에 관한 현상학적 연구를 함으로써, 재발에 관한 본질을 계속 탐색할 필요가 있으며, 이는 간호 대상자를 심층적으로 이해하는데 도움이 될 것이라 생각한다. ;The purpose of this study was to understand the meaning and essence of relapsed leukemia after stem cells transplantation. For this purpose, this study is a van Manen's hermeneutic phenomenological research, focusing on the essence of experiences, existential survey and interpretive, phenomenological reflection and writing. For this study, 5 leukemia patients who had been hospitalized at 'S' hospital in Seoul to have hematopoietic stem cells transplanted but had their disease recurred were sampled. 4 were males and one was female. The data were collected from April to August, 2003, while patients continued to be interviewed in depth and observed. For the interview, a semi-structured questionnaire was used. Based on their agreement, the contents of the interview were all recorded to be transcribed by listening to the recorded statements repeatedly. The collected data were analyzed to classify some significant themes and determine fundamental ones. In this course, etymologies, literary works, art works and phenomenological literature were reviewed to get some insights into patients' experiences of suffering from relapsed leukemia after transplantation of stem cells. The sample patients' suffering experiences were expressed in the following phrases: 'a miserable mental pain which I wish to refuse and forgot for the life of me', 'a dammed doom forced to accept the reality anyhow', 'a extreme pain of body urging me to die', 'a financial hardship driving me into a despair again', 'a loneliness caused by continued separation', 'a hope of the life which I wish to live like a miracle'. Finding from article and creative inquiry further was validated and meaning was discovered. The study illuminated meaning and simultaneously validated the phenomenological research process. Essential themes for understanding experience of relapsed leukemia after hematopoietic stem cells transplantation, implications, for education, research and practice, direction and need for continuing inquiry were identified.
