손 마사지가 암환자의 성격유형별 수술 전 상태불안과 수면에 미치는 영향

Abstract

The purpose of this study was to compare the effects of hand massage as a nursing intervention on preoperative the anxiety and sleeping disturbance in personality type among patients. The subjects were twenty-three patients for A type personality group and twenty-five for B type personality group, who were admitted at S hospital for gastrointestinal cancer surgery. This study was carried out from September, 2003 to November, 2003. The data were selected from at S hospital in Seoul. Research instruments were Framingham A, B type Personality, Visual Analogue Scales of Pain, State-Trait Anxiety Inventory, and the VSH sleep scale. The collected data were analyzed by means of frequency, percentage, standard deviation, chi-square test, t-test, ANOVA and Pearson correlation with SAS 8.2 program. The results of this study were as following 1. The level of state anxiety was decreased very significantly in A type Personality group after hand massage (P= .001). The state anxiety level decrease significantly in B type Personality group after hand massage (P= .027). 2. No significance shows sleep quality difference in A type Personality or B type Personality after hand massage(P=0.924). In conclusion, hand massage for 10 minutes is a benefical nursing intervention that alleviates the anxiety of A type personality patients in immediately prior to surgery, therefore it is recommended to use the hand massage as a nursing intervention for patients undergoing anxiety. But the result of no significant difference sleep quality in personality type before surgery was suggested as following. It means that the study need to compare the experimental group with control group. The results of this study appear promising, additional research is recommended to further the appropriate uses of hand massage in nursing practice for this and other patient population. ;2002년도 통계청 자료에 따르면 암이 사망원인 중 1번째이며 암으로 사망하는 인구는 점차 증가하고 있다. 암의 장기별 발생빈도는 폐암이 1위이고 위암, 간암, 대장암 순이며 소화기계 암이 사망원인중의 큰 부분을 차지하고 있다. 수술 전 환자들은 수술과 마취에 대한 염려와 불안이 높고 수술 불안감과 함께 수면 장애를 경험하며 심한 경우 수면장애가 동반되고 회복을 지연시키게 되므로 수술 전 환자의 불안을 감소시키는 간호는 중요하다. 대상자의 성격유형에 따라 스트레스 상황에 직면했을 때 대처하는 형태가 달라진다. 따라서 본 연구는 수술 전 대상자에게 손 마사지 실시하여 성격 유형에 따른 상태불안 감소가 차이가 있는지를 알고, 수면 질의 차이가 있는지를 검증해 보고 간호실무에서 활용할 수 있도록 하기 위함이다. 서울에 소재한 S병원에서 소화기계 암으로 복부 수술을 실시한 환자 48명을 대상으로 하였다. Framingham의 성격유형에 따라 A형 23명, B형 25명을 무작위 임의 추출하여 시행하였고 자료수집기간은 2003년 9월부터 11월까지였다. 연구도구는 일반적 특성 10문항, Framingham(1978)의 A유형 행동검사, Spielberger의 State-Trait Anxiety Inventory로 김정택(1978)이 번역한 상태불안 측정도구 20문항을 사용하였으며, 수면의 질 측정은 Verran & Snyder-Halpern(1987)이 개발한 8문항의 도구를 이용하여 측정하였다. 자료분석은 SAS 8.2 프로그램을 이용하였으며, 두 집단간의 일반적 특성은 빈도, 백분율, 평균, 표준편차, t-test, χ^(2)-test, ANOVA를 사용하였으며 성격유형에 따른 손 마사지의 상태불안 감소효과와 수면의 질 차이는 Wilcoxon's Singled Rank Test, Wilcoxon's Two-Sample test를 사용하여 분석하였다. 본 연구의 결과는 다음과 같다. 1. 손 마사지 실시 후 A형 성격유형에서 상태불안 감소는 B형 성격유형의 상태불안 감소보다 유의하게 낮았다. 2. 손 마사지 실시 후 수면의 질은 A형 성격유형과 B형 성격유형의 두 군간의 차이가 유의하지 않았다. 본 연구에서 B형 성격유형보다 스트레스에 더 민감하게 반응하는 A형 성격유형에서 손 마사지 실시 후의 상태불안이 더 유의하게 감소되었다. 따라서 임상에서 소화기계 암 수술을 하기 위해 입원한 대상자들에게 손 마사지 실시는 상태불안 감소를 위한 간호중재로 이용될 수 있을 것이다. 하지만 수면 만족도 차이는 두 군간의 차이가 유의하지 않은 것으로 나타난 것으로 볼 때 손 마사지 중재를 하지 않은 대조군과의 비교 연구가 더 필요할 것으로 사료된다.
