Abstract
최근 우리나라에서도 여성음주자가 급증하면서 여성알코올 중독자에 대한 관심이 증가하고 있다. 본 연구의 목적은 가족력 유무에 따른 여성알코올 중독자와 남성알코올 중독자와의 비교를 통해 인구통계학적 요인, 음주력, 가족력, 자아의 강도와 성격요인 등이 어떤 차이가 있는지를 조사하고 여성 알코올 중독자들의 예후 예측인자를 사회 문화적, 심리적 변인을 중심으로 알아보고자 하였다. 연구 대상은 DSM-IV의 진단기준에 따라 알코올 의존으로 진단받은 알코올 중독자들을 대상으로 2003년 3월∼4월까지 입원치료에 참여하고 있는 남, 여 알코올 중독 환자들을 각각 60명씩 선별하여 설문조사를 실시하였고 무성의한 답변을 보인 응답자는 제외되어 최종적으로 남자 59명, 여자 52명이 통계처리 되었다. 연구에 사용된 척도는 가족력을 측정하기 위해서는 Jones(1983)이 개발한 문제성 음주자의 자녀변별검사(Children of Alcoholic Screening Test: CAST)를 손영균(1994)이 번안하여 한국화 한 것을 사용하였다. 성격요인검사에 사용된 설문지는 타당성과 신뢰도 등을 고려하여 Catell과 동료들이 개발한 16개 성격요인검사(16PF Questionaire)의 Form-c를 서울대학교 부설 학생생활연구소에서 한국인에 알맞게 표준화한 SPF(Sixteen Personality Factor Test)식 한국판 성격요인 검사지를 사용하였다. 자아강도척도에 대한 측정도구의 구성은 Barron(1953)의 자아강도 척도가 68문항으로 구성되어 있으나 본 연구에서는 우리나라의 정신과 임상에서 가장 널리 쓰이고 있는 단축형 MMPI에 포함되어 있는 31개 문항으로 자아강도를 측정하였다. 수집된 자료에 대한 실증분석방법은 빈도분석, (Chi-square)검증, 일원 변량분석(One way Anova), t-검증(t-test), 상관분석(Correlation Analysis), 신뢰도분석(Reliability analysis)을 활용하였으며 유의수준 p<.05에서 검증했으며 통계처리는 SPSS / WIN 10.0 프로그램을 사용하여 분석하였다. 연구결과에 따라 먼저 가족력 유/무에 따른 여성군을 비교 분석한 결과를 보면 다음과 같다. 음주력에 있어서는 처음 술을 마셔 본 시기는 가족력 있는 여성군의 경우 21세, 가족력이 없는 여성군의 경우가 22세로 나타나 가족력 있는 경우 보다 가족력 없는 경우 처음 술을 마셔 본 시기가 더 늦은 것으로 나타났다. 술을 마시는 이유는 전체적으로 가정, 사회, 경제적인 스트레스가 63.5%로 가장 많은 응답을 보였으나 가족력이 있는 경우보다 가족력이 없 는 경우 습관적으로 술을 마시는 경우가 더 높은 것으로 나타났으며 통계적으로 유의한 차이를 보였다(p<.05). 단주를 지속시켜 준 요인에 대해 살펴보면, 가족력 있는 여성군의 경우에는 신체 건강상태에 대한 두려움이 33.3%, 가족력 없는 여성군의 경우에는 가족에 대한 책임과 의무가 높은 것으로 나타났다. 단주에 실패하는 이유로는 가족력이 있는 여성군의 경우에는 가정에서의 불화가 40.7%, 가족력 없는 여성군의 경우에는 음주에 대한 욕구가 32.0%,와 불안감과 우울감이 각각 24.0%로 높게 나타났다. 자아강도에 대해 살펴보면, 가족력 있는 여성군의 경우가 16점, 가족력 없는 여성군이 18점으로 가족력 있는 경우보다 가족력 없는 경우 자아가 강하게 나타났으나 통계적으로도 유의미한 차이를 보이지는 않았다.(p>.05) 여성군내 가족력 유무에 따른 자아강도와 성격요인간의 관계를 알아보기 위하여 상관분석을 실시한 결과를 살펴보면, 가족력이 있는 여성군에서 자아강도는 유아성-원숙성(r=-.425), 소심성-위험성(r=-.479), 충동성-통제성 (r=-.507), 긴장성-안정성(r=-.559)과의 관계에서 유의미한 부적(-)인 상관 관계를 보이는 것으로 나타났다. 가족력이 없는 여성군에서 자아강도는 소심성-위험성(r=-720), 방종성-인습성(r=-.702), 긴장성-안정성(r=-.770)과의 관계에서 유의미한 부적(-)인 상관관계를 보이는 것으로 나타났다. 둘째, 여성군과 남성군을 비교 분석한 결과를 보면 다음과 같다. 처음 술을 마셔 본 시기에 대해 살펴보면, 남자가 17.8세, 여자가 21.9세로 남자보다 여자가 더 늦은 것으로 나타났으며, 이는 통계적으로도 유의미한 차이를 보였다.(p<.001) 술을 마시는 이유에 대해서는 전체적으로 가정, 사회, 경제적인 스트레스 때문에가 61.3%로 가장 많은 응답을 보였다. 알코올 중독으로 인하여 정신과에 입원한 횟수에 대해 살펴보면, 남자가 5.2회, 여자가 4.0회로 여자보다 남자가 더 많은 것으로 나타났으며, 통계적으로는 유의미한 차이를 보이지 않았다. 입원방법에 대해서는 남자의 경우 67.9%, 여자의 경우에는 53.8%로 비자발적 입원이 높게 나타났다. 단주를 지속시켜 준 요인에 대해서는 전체적으로 가족들의 적극적인 협조가 가장 많은 응답을 보였다. 단주실패의 원인으로서는 남성군에서 음주에 대한 욕구가 33.9%가 여성군에서도 음주에 대한 욕구가 32.7%, 가정에서의 불화가 28.8%, 불안감과 우울감이 21.2%로 나타났으며, 이는 통계적으로도 유의미한 차이를 보였다.(p<.05) 자아강도를 비교해보면, 남자가 20.2점, 여자가 17.7점으로 여자보다 남자가 더 자아가 강하게 나타났으며, 이는 통계적으로도 유의미한 차이를 보였다.(p<.05) 즉, 남성군의 경우는 자아강도가 강할수록 대상자 성격의 참여성, 자신감, 자립성이 높게 나타난다는 것을 알 수 있으며, 여성군의 경우는 자아강도가 강할수록 대상자 성격의 위험성, 인습성, 자신감, 자립성, 통제성, 안전성이 높게 나타난다는 것을 알 수 있었다. 이상과 같은 연구결과를 통해 살펴본 임상적 의의는 다음과 같다. 첫째, 여성알코올 환자는 남성알코올 환자와 중독특성과 임상 양상이 다르다는 것을 알 수 있었다. 둘째, 가족력이 세대간에 전달되어 예후에 유전적 소인이 관여함을 예측해 볼 수 있었다. 셋째, 여성알코올 중독환자의 긍정적 예후에는 개인뿐 아니라 가족들의 적극적 관심과 환경적 개입이 필요하다는 것이다. 향후 여성알코올 중독환자의 특성을 예방과 치료, 사후 관리에 반영하고 이들에 대한 세심한 배려가 있어야 할 것이다.;Recently, we have increased interests in the female alcoholics as the number of drinking women has jumped up. The purpose of this study was to find any difference among demographic factors, alcohol experience, family history, self-power, and personality by comparing the female with male alcoholics with and without family history, and to examine the prognosis prediction factors of women alcoholics around sociocultural, and psychological variables. The subjects were the alcoholics who diagnosed as being dependent on alcohols by the diagnosis criterion of DSM-Ⅳ and who hospitalized for treatment during March∼April, 2003, from which both 60 men and 60 women were selected and asked to answer a questionnaire. Those who replied insincerely were excluded, and the response from 59 men and 52 women was used for statistical processing. The measures used was revised Children of Alcoholic Screening Test(CAST) that Son, younggyun(l994) adapted and localized from the original Jones(1983)'s version. The questionnaire for personality factors test was revised SPF test(Sixteen Personality Factor Test) that Student Counseling Center under Seoul National University standardized the Form-c of 16 PF Questionnaire, which was developed by Cattell and colleagues, in consideration of validity and reliability. for Koreans. Measuring tool for self-power consists of 31 items that were included in abridged MMPI, which is currently used for psychiatry practices in Korea, although Barron(1953)'s tool consisted of 68 items. A SPSS/WIN 10.0 program was used for data processing with frequency analysis, x^(2)(Chi-square)-test, One way Anova, t-test, correlation analysis, and reliability analysis, at the significance level of p<.05. First, the result of comparing and analyzing female group with and without family history is as follows; For alcohol uses, female group with family history began drinking at the age of 21, while the group without family history did at 22. This means those with family history tend to drink alcohols earlier than those without family history. For the reason to drink, 63.5% of respondents said because of family, social, and economic stress. But those without family history tend to have more habitual drinking than those with family history, showing a significant difference(p<.05). For the factors of giving up drinking, female group with family history responded a concern to physical health, 33%, whereas the group without family history said a responsibility and obligation for family as a main reason. For the reason to fail to stop drinking, female group with family history mentioned 'family conflict', 40.7%, and group without family history did 'desire to drink', 32.0%, with anxiety and depression to be 24.0%, respectively. For self-power, female group with family history took 16 points, while the group without family history did 18 points. This indicates that those without family history have stronger self-powers, but there is no signifiant difference(p>.05) Correlation analysis was used to find the relations between self-powers and personality factors depending on family history. The findings shows that there is significant negative con-elations between youngness and maturity(r=.-425), cautiousness and dangerousness(r=-.479), impulse and controls(r=-.507), strain and stability(r=-.559) in the self-power of female group with family history. For female group without family history, self-powers had significant negative correlations between cautiousness and dangerousness(r=0.720), looseness and convention(r= - .702), strain and stability (r= - .770). Second, the result of comparing and analyzing female group with male group is as follows; For the time to begin drinking, men began drinking at the age of 17.8, and women did at the age of 21.9, showing that women are found to drink later than men, with statistically significant difference(p<.00l). For the reason to drink, 61.3% responded because of family, social, and economic stress, in general. For the number of hospitalizing in psychiatry due to alcoholism, it was found to be 5.2 times for men, and 4.0 times for women. It indicates men tend to hospitalize more than women, with no significant difference statistically. For how to hospitalize, 67.9% of men, and 53.8% of women were forced to hospitalize. For the factors to keep them from drinking, family's active cooperation was the most frequently responded. For the reason to fail to stop drinking, there were desire to drink, to which 33.9% of men and 32.7% of women responded, family conflict, 28.8%, and anxiety and depression, 21.2%, with significant difference statistically (p< .05). For comparison of self-power, men took 20.2 points, and women did 17.7 points, showing men have more strong self-power than women, with significant difference(p<.05). In other words, the stronger self-powers men have, the higher involvements, self-confidences, independences. While for women, the stronger self-powers lead to the higher dangerousness, conventions, self-confidences, controls, and stabilities. Based on the findings, the study has clinical implications as follows; First, the nature and clinical aspects of women alcoholics are different from that of men alcoholics. Second, it is expected that family alcohol history conveyed from generation to generation results in hereditary dispositions in prognosis. Third, it needs both family and individual's active interests and environmental interventions for the positive prognosis of women alcoholics. From now on, we will have to consider the characteristics of women alcoholics in the prevention, treatment, and follow-ups, and pay close attention to them.
