간호학생을 위한 임상실습평가도구의 개발

Abstract

The Ultimate purpose of nurse education is to enable the students who finish the nurse education course to play a good role of a nurse, and in the achievement of this purpose clinical practice education is very important. Clinical practice education evaluation helps to estimate how much the achievement of the purpose of clinical practice education is advanced ,and helps the students study and the teachers improve their practice guiding method. Therefore it is necessary to develop a valid and reliable clinical practice evaluation tool. This study is to develop a evaluation tool for the efficient clinical practice evaluation based on the nursing process, in which the content and evaluation in the clinical practice education are not contradict, in order to evaluate exactly how the purpose of the education is attained. This study is made devided into the developing process of a tool and the verifying process of the validity and reliability of the developed tool. The subjects in the tool developing process are two expert groups. And those in the verifying process are ten nurse professors who guide and evaluate the students in the clinical practice, and one hundred head nurses working in the hospitals, which are placed in Seoul, Sungnam, and Suwon. The collection of materials for the tool development has been done from Feb. 26 to Apr. 15 in 1996, and for the verification of the validity and reliabilty of the developed tool has been done from Apr. 16 to 24 in 1996. In the analysis of the materials collected, to inspect the reliability we, using S.A.S worked out Cronbach a coefficient, and to verify the validity we worked out average, standard deviation, and factors. The tool developing process in this study is as follows; 1. To collect the tools for clinical practice evaluation which are being used in nurse colleges in Korea, and to analyze the theoretical frame and the evaluation content. 2. To refer to related books, and to write out the prelimenary lists for the evaluation items and content of the clinical practice ,evaluation tool, to which we applied the nursing process advised by two nurse professors. 3. To evaluate the validity of the prelimenary tool through the first group of 2Q experts, and to revise and supplement the content. 4. To evaluate the validity of the prelimenary tool through the second group of 10 nurse professors, and to revise and supplement the content. 5. To verify the validity and reliability of the developed tool. The results of this study is as follows; 1. To develop a clinical practice evaluation tool, in which the nursing process was applied. The developed tool consists of six stages, adding the professional behaviour to five stages of nursing, which are material collection, nursing diagnosis, nursing plan, nurse implementation, and evaluation. Each stage consists of three to eight question items in the content, and so the sum total of evaluation items is thirty one. 2. To examine the validity and reliability of the developed tool. 1) The verification of the internal coherence As the result of the verification of the internal coherence of the developed tool, the Cronbach α, coefficient was worked out 0.9454 in 31 evaluation items, the range of the Cronbach α coefficient of each evaluation items was worked out from 0.9414 to 0.9458. Its relative high numerical value verifies the great internal coherence. 2) The analysis of the question items The result of the question item analysis of the developed tool shows that the correlation to each item is high, with the range of 0.3100 to 0.7600 in the correlation coefficient with total. 3) The verification of the validity of the content To examine the validity of the clinical practice evaluation tool developed by nursing process range we gave, respectively, 5 grades to "very valid", 4 to "valid, 3 to "ordinary", 2 to "not valid", and 1 to "very unvalid". The validity grades of 31 question items range from 3.72 to 4.49 and the average is 3.96. It shows that the validity of the content is high. 4) Factor analysis In the results of factor analysis of the developed tool, we can see factor one is nursing diagnosis and nursing plan, factor two is nurse implementation, factor three is material collection, factor four is professional behaviour, and factor five is evaluation. The eigen value of each factor ranges from 1.1807 to 11.863. From the above results, we can see that the clinical practice evaluation tool developed in this study shows high validity and reliablity. We hope this evaluation tool will be much used in the evaluation of clinical practice education.;간호교육의 궁극적인 목표는 간호학생들이 교육과정을 이수한 후 간호사로서 그들의 역할을 층분히 발휘할 수 있도록 하는 데 있으며 이러한 목적을 달성하는데에 임상실습교육이 큰 비중을 차지하고 있다. 임상실습교육 평가는 임상실습교육의 목표달성이 어느정도 이루어졌나를 측정하며 학생의 학습활동을 돕고 실습지도를 개선토록 하는 것이며 이를 위해서는 타당하고 신뢰성 있는 임상실습평가도구의 개발이 필요하다. 이에 본 연구는 임상실습교육에서 교육되고 있는 내용과 평가내용이 일치되어 교육의 목적달성도를 정확히 평가할 수 있도록 간호과정을 근거로 한 평가도구를 개발하여 효율적인 임상실습평가에 기여하고자 시도하였다. 본 연구는 도구의 개발과정과 개발된 도구의 타당도 및 신뢰도 검증과정으로 나누어진다. 도구개발과정에서의 대상은 2개그룹의 전문가 집단이었으며 개발된 도구의 신뢰도, 타당도 검증 대상은 간호학생의 임상실습 지도 및 평가를 맡고 있는 간호학 교수와 서울 및 수원, 성남등에 소재한 대학부속및 종합병원등에서 근무하고 있는 수간호사등 100명이었다. 도구개발을 위한 자료수집은 1996. 2. 26일 부터 4월 15일까지 이루어졌으며, 개발된 도구의 타당도, 신뢰도 검증을 위한 자료수집은 1996. 4. 16일 부터 4. 24일까지 이루어졌다. 수집된 자료의 분석은 S.A.S를 이용하여 신뢰도를 검증하기 위해 Cronbach α계수를 산출하였고, 타당도를 검증하기 위해 평균, 표준편차 및 요인분석을 산출하였다. 본 연구의 도구개발과정은 다음과 같다. 1. 전국 간호대학에서 사용하고 있는 평가도구를 수집하여 내용을 분석하고, 문헌고찰 및 간호대학 교수 2명의 자문을 받아 간호과정을 기초로 한 임상실습평가 도구의 평가항목 및 내용의 예비목록 작성 2. 20명으로 구성된 1차 전문가 집단을 통한 예비도구의 내용타당도 측정 및 내용 수정, 보완 3. 10명의 간호학 교수로 구성된 2차 전문가 평가단을 통한 예비도구의 내용타당도 측정 및 내용 수정, 보완 4. 개발된 도구의 신뢰도 검증을 위하여 내적일관성 검사 및 문항분석 5. 타당도 검증을 위해 내용타당도 조사 및 요인분석 본 연구의 결과는 다음과 같다. 1. 간호과정을 기초로 한 임상실습평가도구의 개발 개발된 도구는 간호과정의 5단계 즉, 자료수집, 간호진단, 간호계획, 간호수행, 평가와 그리고 전문직 행동을 추가하여 6개의 평가항목으로 구성되고 있으며 각 항목은 다시 3개에서 8개의 평가 내용으로 구성되고 있어 총 평가내용은 31개이다. 2. 개발된 도구의 신뢰도 및 타당도 검증 가. 내적 일관성 검사 개발된 도구에 대해 내적일관성에 의한 신뢰도를 검증한 결과 31개 평가문항에서는 Cronbach α계수가 0.9454로 나타났고 각 문항의 Cronbach α 계수의 범위는 0.9419∼0.9458로 나타나 비교적 내적일관성이 높았다. 나. 문항분석 개발된 도구의 31문항의 문항분석 결과를 살펴보면 correlating with total이 0.3100∼0.7600로 나타나 각 문항의 상관관계가 대체로 높았다. 다. 내용 타당도 검증 간호과정을 기초로 하여 개발한 임상실습평가도구의 타당도를 측정하기 위해 5점척도로 '매우 타당하다'고 보면 5점, '타당하다' 4점, '보통이다' 3점, '타당하지 않다' 2점, '매우타당하지 않다'라고 보면 1점으로 평가하도록 작성하였다. 31개의 문항의 내용 타당도는 평균 3.72점에서 부터 4.49점까지 분포하고 있으며 31개 문항의 평균점수는 3.96점이었다. 평가내용이 모두 타당도가 높음을 알 수 있었다. 라. 요인분석 본 연구에서 개발된 도구의 6개 평가항목 및 31개 평가내용에 대하여 요인분석을 실시한 결과 요인1은 간호진단. 간호계획으로 나타났고, 요인2는 간호수행, 요인3은 자료수집, 요인4는 전문직 행동, 요인5는 평가로 분리되어 나타났다. 각 요인간에 eigen value는 1.1807∼11.863으로 나타났다. 이상과 같은 결과에서 볼 때 본 연구에서 개발된 임상실습평가도구는 신뢰도 및 타당도가 높은 것으로 나타났으며 이 평가도구가 임상실습교육평가에 많이 이용되기를 기대한다.
