실험적으로 유도된 가토의 면포에 수종 좌창 국소도포제의 면포용해 능력에 관한 비교연구

Abstract

좌창은 모남 상피세포의 과각화에 의해 생성되는 면포를 특징으로 하는 입모 피지낭의 만성 염증성 질환으로 면포는 모낭의 과도한 정상 각화에 의해서 발생되는 것이 아니라 각질 세포사이의 점착력의 증가로 각질 세포가 피지와 함께 모낭관 밖으로 배출되지 못하고 하부 모낭 누두에 축적되므로써 형성된다. 저자는 좌창 국소치료제의 면포 용해능력을 알아보기 위해서 paraffin유에 용해시킨 50% oleic acid를 가토의 양쪽 외이도에 3주간 도포하여 면포를 형성시킨후 좌측귀를 대조군으로하고 우측귀에 5종의 좌창 국소도포제를 4주간 도포하여 면포의 수와 면적의 변화를 관찰하고 다음과 같은 결과를 얻었다. 1. 면포 수의 변화 1) 치료 1주에 20% azelaic acid도포로서 면포 수가 50% 감소되어 가장 빠른 반응을 보였다. 2) 치료 4주에 면포 수의 감소는 0.1% 비타민 A산이 83.3%였고 나머지 4종의 약제는 87.5%로 같은 감소율을 나타내었다. 2. 면포 면적의 변화 1) 치료 1주에 유의한 면포 면적의 감소를 보인 약제는 benzoyl peroxide와 3% salicylate였다. 2) 치료 4주에 가장 큰 면포 용해 능력을 보인 약제는 0.1% 비타민 A산(68.5%)이었고 그 다음이 5% benzoyl peroxide(67.8%)이었으며, 5% sulfur와 3% resorcinol의 복합제제(37.8%), 3% salicylate(37.3%), 20% azelaic acid (34.8%)의 순이었다. 이상의 결과로 면포 용해능력은 0.1% 비타민 A산과 5% benzoyl peroxide가 비교적 월등했으며, 5% sulfur와 3% resorcinol복합 제제, 3% salicylate, 20% azelaic acid는 비슷한 효과를 보였다.;Acne vulgaris is a chronic inflammatory disease of pilosebaceous follicles characterized by comedo formation produced by the impaction and distension of the follicles with tightly packed horny cells. Many agents are effectively used for the treatment of acne, but the comparative studies on comedolytic effects of such drugs are nearly absent. In present work comedolytic effects of 5 commonly used topical acne remedies, 5% sulfur and 3% resorcinol in lotion, 5% benzoyl peroxide, 0.1% vitamin A acid, 3% salicylate and 20% azelaic acid, were evaluated in 15 rabbits on whose ear comedones had been experimentally induced by application of 50% oleic acid in paraffin oil. The number and size of comedones were measured weekly for 4 weeks after daily application of 5 comedolytic agents on comedon bearing rabbit ears. The results are as follows: 1. 20% Azeloic acid decreased numbers of comedones by half at first week, but there was no significant difference in number of comedones between drugs at final week. 2. Benzoyl peroxide and 3% salicylate decreased mean comedon size significantly from first week, and the others did from 2nd week. (P＜0.05) 3. In comparison of maximal capacity to decrease comedon size, 0.1% vitamin A acid was the greatest and 5% benzoyl peroxide, 5% sulfur and 3% resorcinol, 3% salicylate and 20% azelaic acid in decreasing orders.0.1% Vitamin A acid decreased mean comedon size by 68.5% at fourth week.
